It should be up to insurance companies to be honest. They definitely should have option to not cover deliberately risky accounts. It is similar for any other insurance. If you are driving drunk your accident insurance is also void.

Covid treatment wont be covered by healthcare system and people can be free of restrictions.
Vaccines just protect you anyways so unvaccined wont have any responsibility by spreading nor causing problems for healthcare system.

At least half the people who are in hospital are their because of their own actions if you want to go down this route. Most of the people with liver cancers were drinkers, drug abusers or fatties. Most of the people with lung cancer were smokers. Most of the people with type 2 diabetes were fatties. The list goes on.
