I got this in my mail today.
I don’t live anymore in the apartment mentioned.
The contract for the mentioned house was transferred to my friend’s name in December.
And I also left that house to move in another apartment.
Also I did not do my city registration on the mentioned home. Just lived there for a month and moved.
The house was on my name from November till December- January.
They have two papers to fill.
How do I tackle this. Am I in trouble?

No you are not in trouble but you need to answer them either way with the correct information: you used to live there (did not register your residence there which is not great but cannot be changed now) and then update them that you are not renting the mentioned house anymore and have your one and only residence somewhere else (attach your Meldebescheinigung or rental contract of your current home).
