I got the mswt refund form, but a bit lost with this section. For the EUR – do I put the total amount or without the mswt? What is Ct?

You should know that they don’t refund you at the German border. You demonstrate/declare the items exported to German border control and have them stamp the form. Stamped form goes to the business you paid VAT to, and that business refunds you.
If you ship to CH from amazon.de, the swiss MWST is already calculated by Amazon, no need of a form: https://www.amazon.de/gp/help/customer/display.html?nodeId=201910940
If the seller is Amazon Eu Sarl just let the German border officer put a stamp on the printed out invoice you got directly. No need for any form.
You then mail all the stamped invoices to Amazon.

For Amazon you just need the invoice with your Swiss address stated as receiver. Show it at the German customs and send the invoice with „Ausfuhrstempel“ to Amazon.
I think you put the total amount with VAT.
At the same time I’d like to know how it goes for you. I usually do not claim the VAT for things I buy and they do not come with a form to get it back.
