SOUTH BEND, Ind. (WNDU) – Patients at a dentist office in South Bend are being told that their personal information may have been leaked in a recent data breach.
Jefferson Dental Center says it discovered on Nov. 15 that its computer network had suffered a cyber attack and that an unauthorized user had gained access to patient information.
That stolen information may include the following:
NamesSocial Security NumbersAddressesDriver’s License NumbersDate of Birth
Jefferson Dental Center says it has notified the Indiana Attorney General’s Office and Federal Law Enforcement about the data breach.
The company suggests that you take the proper steps to protect yourself if your information was exposed, including placing an initial or extended fraud alert and monitoring your bank account statements.
