The survey isn’t well designed; you should consider spending more time on that.
People should know what kind of input you want, but you shouldn’t bias the answers. Be more aware of which questions should be multiple choice, etc.
This is a terrible survey. Your 5 minute estimate is more like 30 minutes, 10 of which would be spent trying to interpret what your questions really ask.
