You have to declare that on your next tax return (and every following one). The tax office will want this confirmation once, then not necessarily in the following years.
You can deduce a certain sum from your tax declaration, cheaper prices for sime museums, parks,…
The Most benefits start at a gdb of 50% but with 40% you have a „Behindertenpauschalbetrag“ of 860€ per year. That means your taxable income is lower. So you have more „netto vom brutto“.
Some companies do offer some benefits but These are Most of the time their free will to do so. There is no Special rule for That.
Edited the typos I saw.
How long did it take your application? I have been waiting for almost 4 months.
First things first: This is basically a certificate that your specific diabetic condition is considered a disability. You don’t have to publicly disclose that status anywhere and it does not bring you any disadvantages.
It depends on lots of things, but I believe that everywhere in Germany a GdB of 40% qualifies you for an extended protection from being laid off and potentially an additional benefits package to cover medical bills as well as tax deductibles. You’ll only get most of that if you decide to disclose the disability to your employer.
Given that that is a bit of a sensitive thing, think about that decision hard. Your employer might welcome a disclosure if you have more than 60 colleagues, as companies above that siylze pay fees for not employing enough disabled in Germany.

You can claim an extra deduction on your income tax return. It’s 860€ with GdB 40.

Tax deductibles, mainly.

[This Checklist](https://www.bih.de/fileadmin/user_upload/Formulare/Checklisten_Arbeitshilfen/Fachlexikon_Offenbarung_Checkliste.pdf) might be helpful – unfortunately, I can’t seem to find it in English.
You might also want to check out [this page](https://sag-ichs.de) (unfortunately I can’t find an English translation right now, either) for help on deciding about disclosing the disability.
EDIT: added a link to the Pro/Con Checklist which might be the better first link to follow.
