I received this yesterday on PEU ZUS.

I signed a contract with NFZ as a student. I pay 55,80zl a month. They asked me to email them a scan of DRA every month (by the 20th of the next month) which I’ve been doing for a year. I forgot to do that the past month. I did not forget the payment (because the month before I payed for two months in advance), I only forgot to submit DRA and send it to NFZ.

Google translate didn’t do a great job translating this. Anyone knows what should I do now if anything?

Ofc first thing came to mind is to go ans ask, but my Polish is very limited so better to have some context. I really appreciate the help

  1. The message says they didn’t get the confirmation of payment. Just send the scan you forgot and it should be ok

  2. Well that’s exactly what the letter is telling you. They did not get the monthly document from you so they added a charge of 55 zl for your health insurance to your account.

    As far as official letters go this one is super friendly and clear. I’m surprised that Google translate failed you.

    Oh and it also says you are legally required to send those so don’t forget next time.

  4. Generally, they wrote that they did not receive the documents, so according to the law they charged you a fee in the amount of 55,80. Just send the documents you forgot to send .
