I honestly don’t care about this, it doesn’t forbitt to give only the name of the fader and doesn’t exclude automatically him, so I think it will not be a problem for anybody.
Surname of both parents, with the order being agreed on is how it works in Spain (it used to be both surnames but the father’s going first)
Put a ? as their middle name until they reach 18, then they can choose for themselves.
