This absolutely can be real. That is called pedal edema. When your heart is no longer capable of pumping efficiently, fluid stays in the interstial spaces and follows gravity, collecting in the lower extremities instead of being returned to the circulatory system via the lymphatics.
Typically,fluid in right-sided heart failure collects in the lungs and left-sided heart failure collects in the extremities abdomen and scrotum. A cardiologist may point out that the this whole business is more complicated than I am stating here but K.I.S.S.
Cause can be from the heart enlarging from untreated high blood pressure, damage to the heart from a heart attack or other heart disease processes.
It is progressive and generally irreversible but can be treated, slowed and mitigated to some extent using diuretics to remove the edema, medications to alter the force of contraction, as well as to alter the heart rate and blood pressure and reduce the changes in the heart muscle that reduce contractility. These medications work to improve the efficiency of the heart and remove excess fluid.
When those interventions fail , a mechanical pump (LVAD) is sometimes used to assist blood flow through the left side of the heart.
It is important to remember this CHF frequently occurs in addition to other chronic disease processes such as diabetes and kidney disease, making treatment challenging as medications that improve one condition can exacerbate another.
