1. I’d love to help, but that handwriting is absolutely horrible and I can’t read any of it.

4. So based on u/Wolff_Hound’s deciphering:

   The document was issued based on criminal record (a document confirming that he didn’t commit a crime, I think that’s what it is) issued by municipality office in Smíchov no. 4?6/19 on 20th October 1919.

   The leave was allowed by (some military units) on 25th April 1919 no. 174.
