A recent large-scale analysis of working-age populations across 204 countries, published in JAMA Ophthalmology, found more than 437 million people were living with visual impairment in 2019—a figure that has nearly doubled since 1990.

These are not abstract numbers. They represent a vast segment of the global workforce whose productivity is constrained by something as basic and treatable as poor vision. As we invest billions in advanced technologies, we are still failing to address one of the most fundamental drivers of human performance: the ability to see clearly.

I call this the billion-dollar blind spot. Each year, unaddressed vision impairment costs the global economy an estimated $411 billion in lost productivity. This is not a theoretical model. It reflects real people excluded from education and employment because they cannot access something as basic as an eye exam or a pair of glasses.

The most striking part is how solvable this problem is. Data gathered by the International Association for the Prevention of Blindness shows that at least one billion people are living with vision impairment that could have been prevented or has yet to be addressed. The interventions are well understood. Glasses, cataract surgery and basic eye care systems can restore sight quickly and affordably.

Yet policymakers, with understandable competing priorities, continue to treat eye care as a charitable health issue rather than what it truly is: economic infrastructure.

When a road is built, we understand it enables commerce. When electricity expands, we recognize its impact on productivity. Vision works the same way. If people cannot see, they cannot fully participate in the economy. They cannot learn effectively in school. They cannot perform skilled work. They cannot contribute at their full capacity.

We take healthy vision for granted, and that is part of the problem. A billion people are unnecessarily living with vision impairment because of the lack of affordable access to care, and the consequences ripple through every development goal we claim to prioritize. If we fail to meet global education targets by 2030, we should ask a simple question: how many children could not see the board in their classroom?

A 2023 economic analysis by the World Health Organization found that every dollar invested in selected eye health interventions could generate a median of $36 in social and economic benefits, highlighting eye care as an important investment in human productivity and development. That return is not generated by one-time interventions. It comes from building systems that ensure people receive the right care at the right time.

Too often, the global response defaults to transactional solutions. Count the number of glasses distributed. Count the number of surgeries performed. These metrics are easy to report, but they miss the larger picture. Real impact comes from sustained systems that train local providers, create jobs, establish clinics and ensure follow-up care.

We have seen what happens when that system exists. In Guatemala, we partnered with economists and researchers to study coffee workers whose livelihoods depend on precise near vision for tasks like sorting beans. When workers received simple reading glasses, productivity increased by 7.9 percent, and they could recover the cost of their glasses with a single day’s work. But the deeper lesson is not about a pair of glasses. It is about building systems that make this level of access routine.

This is why I argue that eye care should be integrated into national development strategies. It belongs alongside investments in transportation, energy and digital infrastructure. It is foundational to everything else we hope to achieve.

Some will argue that we are in a moment that demands bold technological investment. They are right. Artificial intelligence will reshape industries and improve lives. I am not against innovation. In fact, we actively use technology to expand access to care and reach communities that traditional systems cannot.

But innovation alone cannot unlock the potential of people who are excluded from education and work because they cannot see. AI cannot compensate for a child who cannot read. It cannot replace the productivity lost when a worker struggles with untreated vision impairment. Technology amplifies human capability. It does not substitute for it.

What makes this moment particularly frustrating is that we are not waiting for a breakthrough. We already have the knowledge, the delivery models and decades of evidence. Our network now serves more than nine million people each year through integrated systems of care built over 50 years. These systems are designed to be locally led, sustainable and scalable.

Education outcomes improve when students can see. Workforce participation increases when adults can perform their jobs effectively. Health systems become more efficient when preventable conditions are addressed early. Economic growth accelerates when millions of people can contribute more fully.

Governments and global institutions must integrate eye care into economic development strategies. They must prioritize vision care within public health funding. In addition to technological advancement, we must measure prosperity by whether people have the basic capacity to participate in society.

We live in an age of extraordinary artificial intelligence. Yet for millions of people, the most life-changing technology is still an ordinary pair of glasses.
