I’m guessing medical devices (stints) from Boston Scientific & Medtronic
Strong links between Ireland and Puerto Rico pharma companies.

Med devices, Indiana would be orthopaedics such as hips look at DePut Synthes, Stryker and Henry Schein having orthopaedic manufacturing plants.
Pureto Rico would also be med devices but the likes of stents from Boston Scientific and Medtronic or contact lenses/intraocular lenses from J&J.
According to the US Census Bureau, from Indiana to Ireland:
1. Organic Chemicals ($7.1 billion)
2. Pharmaceutical Products ($6.5 billion)
3. Optic, Photo Etc, Medic Or Surgical Instrments Etc ($943.1 million)
Most of Puerto Rico’s exports are similar, so I would guess the same things, but I cannot find information specific to Ireland.
