Your bank. Or you do it with online banking
Just in case you’re not aware: you can do this online as well.
You “send” neither of them. You bring the top one to your bank to pay and the bottom one is to change your data.
Those are two different things.
The one on top is for a bank transfer. You would send this to your bank, or copy the required information into your online banking. If you use only this, you will continue to have to make manual payments on time.
The bottom one is an authorisation for direct debit. You would send that one to the Beitragsservice, so they could then automatically deduct the amounts in time. They cannot use it like that though, as you haven’t filled out crucial parts like the payment interval.
