In French we call it *un décompte de Sécurité sociale* and I used Google translate but couldn’t find any samples.

The *décompte de sécurité sociale* is a statement from your health insurer detailing which of your health care costs they have reimbursed, and how much.
If you have public health insurance in Germany (90% of Germans do), then the system works differently here: the health care provider (e.g. doctor, hospital etc) deals directly with your insurance, you don’t have to pay for the care only to have it reimbursed later. The only exception are the small co-pays for certain services (e.g. 5 € for medication, 10 € per day of hospital stay, etc), which you pay directly to the pharmacy/hospital/etc.
If you have private health insurance, then the system works similarly to what you’re used to. You pay all your health care costs up front, and then send the bill to your insurance provider who then reimburses you all of or part of the cost (depending on the terms of your insurance). You generally get a document called a *Leistungsabrechnung* which details what costs they have reimbursed.
