If you want to be registered put your info at the top and the person you want to send at the bottom
The Adress should be your old one if I get that correctly. It is the one you are currently registered at (as opposed to the one you want to register).
This is a form granting a third party the power to register your address on your behalf. Is this really what you want?
Power of attorney: you are Person 1, your authorized person is NR2.
On top goes your current place of residence.
