There’s no standard form for this, but what you’ve got seems to be fairly reasonable? Your place of education/employment shouldn’t be mandatory but I guess is there to confirm your identity – polish tenancy laws can make it super difficult to evict non-paying tenants, so landlords with any degree of experience will want to double-check that you’re legit.
The only thing I’d be concerned about is the “departure date is not an automatic contract end date”, you probably want to establish what is the contract end date then.
