So I worked 6 months in Poland, then moved to Germany. Now German tax authorities is requesting for income tax confirmation in Poland. I tried to submitted my PIT-37 (along with the confirmation of online transmittal), but they wanted the stamped document instead.
Now since I’m in Germany already, and **will only go to Poland (Warsaw) specifically for this**, I would like to know, if possible, the step-by-step procedure on how to request the stamp for this certificate. I saw this (https://www.podatki.gov.pl/zaswiadczenia/o-dochodach-w-tym-o-przychodzie/) but its too broad, and the form needed to submit there is different from the one given by tax authorities (https://cdn-cgkpp.nitrocdn.com/uTgUHANhNhMNMBQQegQtJnGptaafzOKD/assets/images/optimized/rev-a39123d/wp-content/uploads/2023/02/Zaswiadczenie_EWR_kraje_UE-EOG-723×1024.jpg)
**I would like to plan it as much as possible so that I will not waste time (will take 1 day off) and resources (for booking 1 night accomodation, and roundtrip train ticket)** Because I had a not-so-good experience dealing with them (Warszawa-Ursynow) when I was new – the staff just keeps repeating the same (in Polish), as if that would help. (Had to return before I got what I need)
Or if you know someone who can help (be it paid, or free (better)), I would appreciate recommendations – preferably a person who I can contact directly (not just company email or phone) for faster exchange.
Thank you in advance!

You shared a link with a precisely described procedure on how to apply in person for the cerificate you need. What else is confusing you about it? Why do you claik its too broad?
