Just a few tips, your scales from 1 to 5 don’t indicate which number is best, your question about frequency doesn’t include “more than never but less than 1 time per week”, and “Prepared Food” isn’t defined; is it about frozen, or prepared food? Or frozen prepared food? I didn’t answer because I’m actually not sure what you mean.

    Also, the last question doesn’t make sense: I am the interviewee, but I am none of these persons. And my name shouldn’t be a requirement.

    I think you need to put a little more thought into your poll. Besides that, presenting it in Dutch here instead of English will mostly get you answers from Flanders, not the rest of Belgium.
