I don’t understand what date of service is
does that mean the date I applied to become a Polish citizen or does that mean today’s date when I am signing this paper?
The backed out parts were filled out already by the consulate.

I recommend just simply contacting the consulate/embassy and surely they will assist you.
If you translate directly … “Signature of the receiver as well as place and date of the delivery” … but delivery to who? You? Consulate? IMO consulate would make more sense.
It is probably better to call them.
