To be honest, if you aren’t mentally ill at the time of acquiring the guns, you won’t be denied getting them.
And once you get the license, it’s just up to renewal just like a car license. Just like a car there are no more checks if you are mentally unwell, go blind or something. The same applies to guns in Malta. There is no system to inform the police yet.
The only reason that the police remove a license is through conviction/ order of the court or not updating of license.
In all the above cases, their system flags it and they take action. If there is no integration, the police are effectively blind
