When I was getting it out of the envelope. Fucking hell. Should I just ust a transparent tape or ask for a copy?

  2. I think the most pragmatical thing to do would just be to fill it as normal, and attach a paper apologizing for having accidentally torn the paper. Maybe use some transparent tape to hold it together, but don’t cover barcodes with it.

    If it’s not OK, they’ll just send a new one.
