Police have apologised to the family of a mum who died from a gunshot wound to the head after it was initially treated as a suicide.<\/p>\n
Perth mum Amy Wensley, 24, died at her Serpentine home in June 2014, and has been the subject of multiple police investigations since.<\/p>\n
A coronial inquest into her death years later found there was insufficient evidence to determine how she died.<\/p>\n
The coroner presiding over the inquest found premature decisions made by detectives who attended her death resulted in lost forensic opportunities, before referring the matter to the Department of Public Prosecutions and the Cold Case unit to review.<\/p>\n
A Western Australia Police spokesman said dedicated investigators have continued to examine all available information relating to Ms Wensley\u2019s death for the past two years.<\/p>\n
Police Commissioner Col Blanche met with Ms Wensley\u2019s mother, Nancy Kirk, and members of her family in January this year.<\/p>\n
The Commissioner acknowledged shortcomings in the initial police investigation and apologised to the family during the meeting.<\/p>\n
\u201cAmy\u2019s death has had a profound impact on those who loved her, and questions surrounding the circumstances of her death have remained unanswered for more than a decade,\u201d a spokesman said.<\/p>\n
\u201cThe WA Police Force remains committed to establishing what occurred and pursuing all available lines of inquiry.\u201d<\/p>\n
Police are offering a $1m reward for information that leads to the identification and conviction of the person or persons responsible for Ms Wensley\u2019s death.<\/p>\n
\u201cThe reward reflects the WA Police Force\u2019s ongoing commitment to resolving the unanswered circumstances surrounding Amy\u2019s death.<\/p>\n
\u201cInvestigators believe there may be people who hold information about Amy\u2019s death who have not yet come forward, or who may now be prepared to speak with police. <\/p>\n
\u201cEven information that may appear insignificant could assist the investigation.<\/p>\n
\u201cAmy\u2019s family have shown extraordinary determination in seeking justice for Amy. The WA Police Force remains committed to the investigation and to ensuring every reasonable investigative avenue is pursued.\u201d<\/p>\n
