It also revealed a higher number of deaths among people who took its therapy in the two Phase 3 trials in relapsing MS, which prompted one analyst, Michael Leuchten of Jefferies, to question the drug\u2019s approval prospects. Analysts have forecasted peak sales of more than 3 billion Swiss francs, or $3.8 billion,\u00a0for fenebrutinib, Leuchten wrote.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">Fenebrutinib is part of a new crop of \u201cBTK inhibitors\u201d that drugmakers are now positioning as potential autoimmune disease treatments. The effort has yielded multiple setbacks in MS, though, as the Food and Drug Administration <a href=\"http:\/\/www.biopharmadive.com\/news\/novo-launches-wegovy-pill-argenx-to-swap-ceos\/808685\/\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:recently rejected a BTK drug from Sanofi;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">recently rejected a BTK drug from Sanofi<\/a>, and other developers like Merck KGaA and Biogen <a href=\"http:\/\/www.fiercebiotech.com\/biotech\/biogen-bails-btk-race-just-19-months-after-placing-125m-bet\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:have given up;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">have given up<\/a>\u00a0on prospective drugs.<\/p>\n<p>Dive Insight:<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">BTK, or \u201cBruton\u2019s tyrosine kinase\u201d inhibitors, have proven successful at treating blood cancers like leukemia and lymphoma. But they\u2019ve been seen as helping with autoimmune conditions, too, as they work by suppressing the activity of the B cells that go haywire in those diseases. Other B cell-targeting therapies, like Roche\u2019s Ocrevus and Novartis\u2019 Kesimpta,\u00a0have helped people with MS control relapses and disability progression.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">Some BTK drugs have already made inroads against immunological disorders. Sanofi\u2019s Wayrilz and Novartis\u2019 Rhapsido have been approved in immune thrombocytopenia and a form of hives, respectively. But progress has been slower-going against MS. Sanofi appeared poised to bring its drug, tolebrutinib, to market for a less common form of the disease. But the rejection it received was <a href=\"https:\/\/www.biopharmadive.com\/news\/sanofi-multiple-sclerosis-btk-inhibitor-troubles\/809443\/\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:based around the kind of liver concerns;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">based around the kind of liver concerns<\/a>\u00a0that now hang over fenebrutinib.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">Roche reported Monday that in a trial called \u201cFENhance 1,\u201d people with the relapsing form of the disorder who got fenebrutinib had a 51% lower yearly relapse rate than those who got Aubagio over the course of at least 96 weeks. That result was consistent with <a href=\"https:\/\/www.biopharmadive.com\/news\/roche-ms-fenebrutinib-clinical-trial-results-data-relapsing-primary\/805152\/\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:findings from an earlier trial in relapsed disease;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">findings from an earlier trial in relapsed disease<\/a>, \u201cFENhance 2.\u201d Roche said the combined results \u201cequate to approximately one relapse every 17 years.\u201d The drug also succeeded in another study in primary progressive disease.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">However, in FENhance 1, Roche said there was one case in the fenebrutinib group, along with one in the Aubagio group, of liver enzyme elevations that met a threshold called <a href=\"https:\/\/www.gastrojournal.org\/article\/S0016-5085(14)00675-1\/fulltext\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:\u201cHy\u2019s law.\u201d;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">\u201cHy\u2019s law.\u201d<\/a>\u00a0That threshold equates to an increased risk of death or liver transplantation, and often draws attention from U.S. drug regulators.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">Additionally, in the combined FENhance 1 and 2 populations, there were eight deaths among fenebrutinib\u00a0recipients \u201cwith various causes and at different points in treatment,\u201d compared with one in Aubagio-treated patients. \u201cFurther analyses are ongoing to better understand these findings,\u201d Roche said in a statement.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">The new findings, then, may not help fenebrutinib\u2019s already shaky case to the Food and Drug Administration, Leuchten wrote in a note to clients. Liver-related side effects were already a known issue from earlier fenebrutinib studies as well as trials of other BTK inhibitors. But the imbalance in patient deaths is a new finding that will likely stimulate further scrutiny, he wrote.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">\u201cThe tolebrutinib\u00a0[rejection letter] makes clear the FDA is looking at a risk\/balance profile especially if treatment alternatives are available,\u201d he wrote. \u201cCombined with the liver safety we are not sure the fenebrutinib profile clears the bar.\u201d<\/p>\n","protected":false},"excerpt":{"rendered":"This story was originally published on BioPharma Dive.
