A new extended trailer for Casualty‘s upcoming 40th anniversary spectacular has revealed what exactly the team will be up against when disaster strikes. The medical drama will air a special double-bill of episodes early next month.
The above clip confirms that a brutal explosion will strike St James’ Hospital, the nearest hospital to Holby. It has previously been regularly mentioned in episodes.
Paramedics will be seen searching through the rubble at the site of the catastrophe, while staff back at Holby City Hospital desperately try to cope with the fallout.

Official teaser spoilers reveal that Dylan (William Beck) is set to face personal tragedy while he leads his team through the extremely stressful situation, while Stevie (Elinor Lawless) and Flynn (Olly Rix) will fight for survival in what’s billed as an “emotionally charged storyline that promises drama on an unprecedented scale, unlike anything Casualty has tackled before”.
Other familiar faces slated to appear are Melanie Hill (Siobhan), Barney Walsh (Cam), Neet Mohan (Rash), Sammy Dobson (Nicole), Kirsty Mitchell (Faith), Anna Chell (Jodie), Sarah Seggari (Rida) and Aron Julius (Matty).
The paramedic team are also at the heart of the action, with Di Botcher (Jan), Michael Stevenson (Iain), Milo Clarke (Teddy), Charles Venn (Jacob) and Naomi Wakszlak (Indie) also featuring.
The double-bill arrives on 5 September, almost exactly 40 years – minus one day – since the show debuted on 6 September, 1986.

The most recent cluster of episodes, subtitled Lethal Legacy, aired from April to July of this year, finishing with a tragic death.
The BBC put Casualty out to tender in 2025, but in December it was confirmed that BBC Studios (who produce it) emerged victorious in the bidding process and will continue for at least three more series.
