Casualty spoilers follow.
Casualty is celebrating its 40th birthday, and the medical drama’s landmark occasion has been marked by a compelling double bill episode – an explosive spectacular and an impressive televisual feat that highlights all that is good about the show and makes a bold statement about what is to come.
The show wastes no time jumping into the action. Within the opening couple of minutes, St James’ Hospital has exploded, and Holby ED is preparing to treat an overwhelming number of victims of the blast. The first 20 minutes are a fascinating insight into how NHS professionals come together to respond to a major incident with many fatalities.

However, the large-scale disaster suddenly became all too personal for Dylan when his ex, Sophia – now a psychiatric nurse at St James’ – was brought into the ED with horrific burns. Kellie Shirley’s return to the show was sadly all too brief, as Sophia’s injuries were not survivable despite Dylan’s very best efforts.
Killing off former character Sophia wasn’t exactly the boldest move the show could have made, but it was a twist that led to a disturbing breakdown for Dylan.
As he spiraled out of control, he put his life at risk by carrying a bag which potentially contained a bomb outside the ED. While the bag was ultimately harmless – and the explosion at St James’ deemed an accident – Matty was devastated by his dad’s recklessness.
Meanwhile, clinical lead Flynn was at the scene of the blast and was soon joined by Stevie. But the pair found themselves in huge peril when a shocking chain of events resulted in their ambulance plummeting off the edge of a cliff and plunging into deep water.
In an unbelievable underwater sequence, just one of director Steve Hughes’ impressive achievements in these episodes, Stevie deliberately drowned while Flynn brought them to the surface. But things went very wrong for Flynn, and when he attempted to resuscitate Stevie on land, he also collapsed.
When the ED got word of Stevie and Flynn’s accident, it all got too much for Dylan, and he retreated to a store cupboard to hide. It wasn’t only about the night’s traumatic events, but also about his recent feud with Stevie, and Dylan was distraught that she might die without them making amends.

Show stalwart Will Beck truly gave an all-time great performance in these scenes, and there was a deeply emotional pay-off too, as Dylan eventually returned to the frontline and acted quickly to save his dear friend’s life.
It really feels like Stevie will be the driving force of the action to come beyond the anniversary – and rightly so, as Elinor Lawless is such a valuable asset to the show. While there could be a romance for Stevie and Flynn on the horizon, it’s Stevie’s relationship with Dylan that is arguably the most interesting.
While the characters share very little screen time in these episodes, their bond is at the very heart of the anniversary. Their touching moment together when Stevie woke up in hospital was interrupted before it could go any further, and it remains to be seen where this compelling dynamic will go next.

Of course, it wouldn’t be an anniversary without some returning characters, and it was a joy to see definitive Holby icon Charlie and long-serving doctor Ethan arrive to help out the struggling team. These weren’t just cameos but meaningful appearances that added so much value to the episodes.
Long-time viewers know that Ethan was in the early stages of Huntington’s disease when he left the show in 2023, and his condition has deteriorated in the years since. He and Charlie were caught up in the drama as Charlie was picking him up from St James’ – but Ethan was a patient in hospital, rather than a doctor.
While Ethan tried to be helpful in the ED, his shaking hands meant he was unable to play as big a role as he would have liked. But Charlie helped him see that there was more than one way to be a doctor, and Ethan’s career in medicine might not be over just yet.

Charlie himself was on hand to give much-needed pep talks to some other characters too. He used advice from his late wife Duffy to spur on a defeated Cam, and it was Charlie who comforted Dylan at his lowest ebb and ultimately encouraged him to save Stevie. Plus, he gave his thanks to hero paramedic Indie, who felt like she had been given a Hollywood handshake!
There are some more nods to the past throughout the episodes, including Flynn’s 1986-themed playlist from his daughters to celebrate his 40th birthday, some hilarious references to Anneka Rice and Martin Kemp, and a vehicle resembling Charlie’s classic yellow Beetle in the opening shot.
Perhaps there could have been more nostalgia flowing through the script, but more old faces probably wouldn’t have made sense in the context of the episodes and could have wasted what is an utterly stellar main cast.

The anniversary is a true ensemble effort, and every character has an important moment – from Rida calling out a patient’s racism, to Indie saving Stevie and Flynn, to Rash bravely stepping up in Dylan’s absence on his first day as a consultant.
These episodes are rooted firmly in the here and now, tackling very timely themes of unity and inclusion, and the importance of coming together in the face of external forces trying to tear us apart.
While it’s a dark double bill, the episodes ultimately leave you with nothing but optimism, and a heartwarming montage in the closing moments epitomises everything that Casualty has represented so proudly for the last four decades, as a powerful voice for the NHS.

There are big changes ahead for the show, with a move from Holby to a fictional Welsh hospital firmly in the pipeline.
But the team both in front of and behind the camera are adamant that this anniversary sets a solid intention for what the new era of Casualty will hold – and if these episodes are anything to go by, we’re in for an absolute treat.
