{"id":116970,"date":"2025-10-12T07:35:10","date_gmt":"2025-10-12T07:35:10","guid":{"rendered":"https:\/\/www.europesays.com\/ie\/116970\/"},"modified":"2025-10-12T07:35:10","modified_gmt":"2025-10-12T07:35:10","slug":"silence-and-evasion-as-family-seek-answers-over-fathers-death-in-ambulance-fire-the-irish-times","status":"publish","type":"post","link":"https:\/\/www.europesays.com\/ie\/116970\/","title":{"rendered":"\u2018Silence\u2019 and \u2018evasion\u2019 as family seek answers over father\u2019s death in ambulance fire \u2013 The Irish Times"},"content":{"rendered":"<p class=\"c-paragraph paywall b-it-article-body__text--left\">A family claims their nine-year quest for answers about their father\u2019s death in an ambulance fire has been met with \u201csilence\u201d, \u201cevasion\u201d and \u201cinstitutional indifference\u201d. <\/p>\n<p class=\"c-paragraph paywall \">\u201cSomething went catastrophically wrong, and no one has taken responsibility,\u201d the family of Christopher Byrne said.<\/p>\n<p class=\"c-paragraph paywall \">They expressed grief, anger and frustration when speaking to The Irish Times following another adjournment of an inquest into Mr Byrne\u2019s death. <\/p>\n<p class=\"c-paragraph paywall \">A widower and father of five from Suncroft, near Naas, Mr Byrne (79) died, and two paramedics were injured, when an ambulance in which he was a patient burst into flames shortly after arriving outside the emergency department at Naas General Hospital on September 22nd, 2016.<\/p>\n<p class=\"c-paragraph paywall \">It is understood the fire started after an oxygen cylinder in the vehicle ignited.<\/p>\n<p class=\"c-paragraph paywall b-it-article-body__text--left\">Mr Byrne, who had recovered from cancer, was being taken to hospital for respiratory difficulties as a result of an infection.<\/p>\n<p class=\"c-paragraph paywall b-it-article-body__text--left\">The inquest into his death was put on hold in 2017 pending an investigation by the <a href=\"https:\/\/www.irishtimes.com\/tags\/health-and-safety-authority\/\" target=\"_self\" rel=\"nofollow noopener\" title=\"https:\/\/www.irishtimes.com\/tags\/health-and-safety-authority\/\">Health and Safety Authority<\/a> (HSA), the State entity responsible for enforcing health and safety in the workplace. <\/p>\n<p class=\"c-paragraph paywall b-it-article-body__text--left\">A criminal prosecution against the <a href=\"https:\/\/www.irishtimes.com\/tags\/hse\/\" target=\"_self\" rel=\"nofollow noopener\" title=\"https:\/\/www.irishtimes.com\/tags\/hse\/\">HSE<\/a> by the HSA was listed for January 29th, 2025 but, two days before that, Mr Byrne\u2019s children (Thomas, Lil, Francis, Christopher and Sarah) were informed the <a href=\"https:\/\/www.irishtimes.com\/tags\/director-of-public-prosecutions\/\" target=\"_self\" rel=\"nofollow noopener\" title=\"https:\/\/www.irishtimes.com\/tags\/director-of-public-prosecutions\/\">Director of Public Prosecutions<\/a> (DPP) was withdrawing the case. <\/p>\n<p class=\"c-paragraph b-it-article-body__interstitial-link\">[\u00a0<a aria-label=\"Open related story\" class=\"c-link\" href=\"https:\/\/www.irishtimes.com\/crime-law\/courts\/2025\/02\/22\/we-never-got-to-say-goodbye-to-him-family-seek-answers-years-after-naas-hospital-ambulance-fire\/\" rel=\"noreferrer nofollow noopener\" target=\"_blank\">\u2018We never got to say goodbye to him\u2019: family seek answers years after Naas hospital ambulance fireOpens in new window<\/a>\u00a0]<\/p>\n<p class=\"c-paragraph paywall \">The inquest was then listed to proceed last month but was adjourned at the request of all parties after issues were raised about disclosure of documents. <\/p>\n<p class=\"c-paragraph paywall b-it-article-body__text--left\">Kildare county coroner Loretta Nolan criticised the HSA over delays in providing information and over the adequacy of its summary report about Mr Byrne\u2019s death, describing it as \u201ca very small report after a long investigation\u201d.<\/p>\n<p class=\"c-paragraph paywall \">Her predecessor as coroner, Denis Cusack, previously raised concerns about delays in receiving reports, particularly from the HSA. Its report landed last June, and Dr Nolan said she had to direct the authority to produce additional documents and exhibits, which she received just a week before the inquest date. <\/p>\n<p class=\"c-paragraph paywall \">She adjourned the inquest after lawyers for the family and other parties sought time to examine all relevant material and to identify necessary witnesses. <\/p>\n<p class=\"c-paragraph paywall \">Speaking on behalf of Mr Byrne\u2019s family following the adjournment, his son-in-law Michael Davis said they have \u201cendured nine years of grief, silence and unanswered questions\u201d.<\/p>\n<p class=\"c-paragraph paywall \">The ambulance fire and explosion that killed Mr Byrne was \u201can event so violent and so preventable that it should have triggered immediate accountability and reform\u201d but was instead met \u201cwith institutional indifference\u201d, he said. <\/p>\n<p class=\"c-paragraph paywall b-it-article-body__text--left\">\u201cFor nearly a decade, we have sought truth and compassion from the HSE, Health and Safety Authority, Health Products Regulatory Authority, BOC Gases, the ambulance service, and every investigative body involved,\u201d he said. <\/p>\n<p class=\"c-paragraph paywall \">Their efforts were met with \u201csilence\u201d, \u201cevasion\u201d and \u201cdelay\u201d. <\/p>\n<p class=\"c-paragraph paywall \">The DPP informed the family last January there would be no case to answer, as the labelling on the oxygen cylinders was deemed up to standard and in date, he said. \u201cWhile we accept the legal conclusion, it does not erase the deeper truth: something went catastrophically wrong, and no one has taken responsibility.\u201d<\/p>\n<p class=\"c-paragraph paywall \">Their legal team got just three weeks\u2019 notice of the inquest hearing, he said. <\/p>\n<p class=\"c-paragraph paywall b-it-article-body__text--left\">The coroner initially decided to proceed with the inquest but then had to adjourn for reasons including failure to produce some documents and delays in providing others.  <\/p>\n<p class=\"c-paragraph paywall \">Mr Byrne was \u201cdeeply loved\u201d and his death was \u201cnot just a tragedy, it was a systemic failure\u201d that was \u201ccompounded by the refusal of those responsible to face us, to explain, to acknowledge\u201d, Mr Davis said.<\/p>\n<p class=\"c-paragraph paywall \">The impact on the family is \u201cimmeasurable\u201d, he said. \u201cWe live with trauma, with the haunting image of how he died, and with the knowledge that no one has stood up to say: \u2018This was wrong.\u2019 We have buried Christopher, but not our questions.\u201d <\/p>\n<p class=\"c-paragraph paywall \">The family, he stressed, is not seeking to assign blame but \u201cto demand recognition that Christopher\u2019s life mattered\u201d.<\/p>\n<p class=\"c-paragraph paywall \">His family \u201cdeserves more than silence\u201d, he said.
