{"id":371494,"date":"2026-03-06T19:55:09","date_gmt":"2026-03-06T19:55:09","guid":{"rendered":"https:\/\/www.europesays.com\/ie\/371494\/"},"modified":"2026-03-06T19:55:09","modified_gmt":"2026-03-06T19:55:09","slug":"no-synthetic-opioid-cychlorphine-has-not-proved-narcan-ineffective","status":"publish","type":"post","link":"https:\/\/www.europesays.com\/ie\/371494\/","title":{"rendered":"No, Synthetic Opioid Cychlorphine Has Not Proved Narcan \u201cIneffective\u201d"},"content":{"rendered":"\n<p>A minor media frenzy has been forming around cychlorphine, a novel synthetic opioid described as the <a href=\"https:\/\/jacksontimesvoice.com\/stories\/next-killer-drug-makes-its-way-to-kentucky,62070\" rel=\"nofollow noopener\" target=\"_blank\">\u201cnext killer drug\u201d<\/a> taking over at an <a href=\"https:\/\/www.knoxcounty.org\/rfc\/pdfs\/mediarelease\/2026\/2026.02.05KnoxCountyRFCPreliminaryTests16DeathsTiedtoNewDrug.pdf\" rel=\"nofollow noopener\" target=\"_blank\">\u201cexponential rate\u201d<\/a> while standard naloxone has <a href=\"https:\/\/jacksontimesvoice.com\/stories\/next-killer-drug-makes-its-way-to-kentucky,62070\" rel=\"nofollow noopener\" target=\"_blank\">\u201cproven ineffective.\u201d<\/a> While there are some notable differences between cychlorphine and the various fentanyl analogs and nitazenes that have previously been called these things, in this case some of the warnings appear due to a mixup with a different drug.<\/p>\n<p>N-Propionitrile chlorphine, often referred to as cychlorphine, was first detected in the United States drug supply <a href=\"https:\/\/www.cfsre.org\/nps-discovery\/monographs\/n-propionitrile-chlorphine\" rel=\"nofollow noopener\" target=\"_blank\">in 2024<\/a> by the Center for Forensic Science Research &amp; Education (CFSRE).<\/p>\n<p>Orphines are their own subclass of synthetic opioid, like fentanyls and <a href=\"https:\/\/filtermag.org\/nitazenes-synthetic-opioids-fentanyl\/amp\/\" rel=\"nofollow noopener\" target=\"_blank\">nitazenes<\/a>. Most coverage of cychlorphine includes the claim that it\u2019s 10 times more potent than fentanyl. This is not wrong, but it should be noted that the estimate comes from lab studies of petri dishes, not humans.<\/p>\n<p>CFSRE has described the emergence of orphines as the result of China scheduling nitazenes in mid-2025. As fentanyl crackdowns opened the door for nitazenes, the inevitable nitazene crackdowns opened the door for orphines. However, CFSRE has so far identified cychlorphine in at least 25 overdose deaths\u201421 in the United States and <a href=\"https:\/\/drugchecking.community\/report\/september-20-october-3-2025\/\" rel=\"nofollow noopener\" target=\"_blank\">four in Canada<\/a>\u2014and\u00a0in 11 of those cases cychlorphine was the only opioid detected. This is a departure from how nitazenes and other novel synthetic opioids have entered the supply.<\/p>\n<p>\u201cI do think that\u2019s a little unusual for compounds we call opiates to be on their own,\u201d Erin Tracy, lead chemist at the UNC Street Drug Analysis Lab, told Filter. \u201cBut other [new psychoactive substances] and other drug classes are often found on their own, like benzos or bath salts.\u201d\u00a0<\/p>\n<p>The lab is a <a href=\"https:\/\/filtermag.org\/drug-checking-mail-kit-safe-supply\/amp\/\" rel=\"nofollow noopener\" target=\"_blank\">public drug-checking service<\/a> at the University of North Carolina at Chapel Hill, where harm reduction organizations around the country send residue samples by mail. Though five of the samples Tracy has identified as cychlorphine appeared on their own, the lab has only seen 11 such samples in total.<\/p>\n<blockquote>\n<p>To date, there is no publicly available evidence to suggest that naloxone would be any less effective for reversing an overdose of cychlorphine than of any other opioid. <\/p>\n<\/blockquote>\n<p>In late January, CFSRE put out a <a href=\"https:\/\/www.cfsre.org\/nps-discovery\/public-alerts\" rel=\"nofollow noopener\" target=\"_blank\">public alert<\/a> describing the increase in cychlorphine-involved fatalities. In February, CFSRE added cychlorphine to its <a href=\"https:\/\/frdd.cfsre.org\/frd-report?id=MzM1\" rel=\"nofollow noopener\" target=\"_blank\">Forensically Relevant Drug Database<\/a> and published a <a href=\"https:\/\/www.cfsre.org\/images\/content\/reports\/public_alerts\/Colombo_Plan_Orphine_Alert_Feb_2026_v2.pdf\" rel=\"nofollow noopener\" target=\"_blank\">health alert<\/a>, since which time the subject has been gaining more media attention.<\/p>\n<p>Of the 21 US overdose cases, seven were in Tennessee. This doesn\u2019t mean that cychlorphine\u00a0is concentrated in Tennessee; what often look like \u201chot spots\u201d on a map don\u2019t necessarily reflect the regions where a new substance is most prominent, but the regions where forensics laboratories are testing for it.<\/p>\n<p>\u201cWe do know it\u2019s more powerful than fentanyl and that naloxone, or Narcan, does not completely block the effects of the drug,\u201d Dr. Darinka Mileusnic-Polchan, chief medical officer at the Knox County Regional Forensic Center, stated in a February <a href=\"https:\/\/www.knoxcounty.org\/rfc\/pdfs\/mediarelease\/2026\/2026.02.05KnoxCountyRFCPreliminaryTests16DeathsTiedtoNewDrug.pdf\" rel=\"nofollow noopener\" target=\"_blank\">press release<\/a>. \u201c[M]ultiple doses may be needed to prevent an overdose.\u201d<\/p>\n<p>To date, there is no publicly available evidence to suggest that naloxone would be any less effective for reversing an overdose of cychlorphine than of any other opioid. In response to Filter\u2019s inquiry, the Knox County Regional Forensic Center said the information had been shared internally by the Drug Enforcement Administration.<\/p>\n<p>\u201cIn our discussions with toxicologists from the DEA we were informed that due to the potency of this particular drug that there is a possibility that multiple doses of Narcan would most likely be needed or possibly ineffective,\u201d said Chris Thomas, the Center\u2019s director and chief administrative officer.<\/p>\n<p>On March 2 the <a href=\"https:\/\/jacksontimesvoice.com\/stories\/next-killer-drug-makes-its-way-to-kentucky,62070\" rel=\"nofollow noopener\" target=\"_blank\">Jackson Times Voice<\/a> used similar language, reporting that the Kentucky Office of Homeland Security had \u201cwarned the public and officials that \u2018Naloxone was not able to block the antinociceptive effects of Cychlorphine completely\u2019 and that traditional use of the life-saving drug has proven ineffective.\u201d<\/p>\n<p>Naloxone does not work by \u201cblocking\u201d an opioid\u2019s effects, because it\u2019s administered after the effects have already taken hold.\u00a0In order to block them, it would have to be administered as a pre-treatment. Which it was, in the original study supporting an October 2025 claim in <a href=\"https:\/\/jacksontimesvoice.com\/stories\/next-killer-drug-makes-its-way-to-kentucky,62070\" rel=\"nofollow noopener\" target=\"_blank\">Clinical Toxicology<\/a> that \u201cNaloxone was not able to completely block the antinociceptive effects of chlorphine.\u201d<\/p>\n<p>N-Propionitrile chlorphine and cychlorphine are the same thing. Chlorphine is actually a different substance.\u00a0<\/p>\n<blockquote>\n<p>A pervasive misconception about naloxone is that it affects the opioid molecules themselves. It doesn\u2019t. <\/p>\n<\/blockquote>\n<p>The Kentucky Office of Homeland Security and the DEA did not respond to Filter\u2019s requests for comment, but it seems reasonably likely that the DEA has been feeding that claim to KOHS and the regional forensic centers and health departments. \u201cChlorphine\u201d could have become \u201ccychlorphine\u201d intentionally or without any of the government agencies responsible ever noticing, and neither would be surprising.<\/p>\n<p><strong>A pervasive misconception about naloxone is that it affects the opioid molecules themselves. It doesn\u2019t. It affects the receptors they bind to in the human brain and body, which are the same receptors regardless of the opioid\u2019s potency. It is unequivocally false to say that naloxone doesn\u2019t work well on higher-potency opioids. Using too much can in some cases <a href=\"https:\/\/filtermag.org\/naloxone-misuse-cops\/amp\/\" rel=\"nofollow noopener\" target=\"_blank\">be as dangerous<\/a> as not using enough.<\/strong><\/p>\n<p>\u201cTreatment should be supportive,\u201d CFSRE stated in its February cychlorphine alert. \u201cTreat the airway and breathing first, followed by reversal with cautious administration of an opioid antagonist (e.g. naloxone), being careful not to <a href=\"https:\/\/filtermag.org\/opvee-nalmefene-naloxone\/amp\/\" rel=\"nofollow noopener\" target=\"_blank\">precipitate acute withdrawal<\/a>.\u201d<\/p>\n<p>Though chlorphine is not the substance being increasingly detected in overdose deaths, it\u2019s still an opioid, so it would be concerning if naloxone really had proven ineffective, or less effective. Fortunately, the original <a href=\"https:\/\/linkinghub.elsevier.com\/retrieve\/pii\/S002839082400282X\" rel=\"nofollow noopener\" target=\"_blank\">study<\/a> showing that naloxone \u201cwas not able to completely block the antinociceptive effects of chlorphine\u201d had nothing to do with overdose, and was instead referring to naloxone as a pretreatment for chlorphine-dosed mice, to see if they were still sensitive to pain when their tails were pinched. It actually goes on to say that naloxone pretreatment \u201cfully\u201d blocked the decrease in respiratory rate brought on by chlorphine.
