{"id":49178,"date":"2025-09-07T13:04:07","date_gmt":"2025-09-07T13:04:07","guid":{"rendered":"https:\/\/www.europesays.com\/ie\/49178\/"},"modified":"2025-09-07T13:04:07","modified_gmt":"2025-09-07T13:04:07","slug":"could-we-catch-heart-failure-sooner","status":"publish","type":"post","link":"https:\/\/www.europesays.com\/ie\/49178\/","title":{"rendered":"Could We Catch Heart Failure Sooner?"},"content":{"rendered":"<p><a href=\"https:\/\/emedicine.medscape.com\/article\/163062-overview\" class=\"cl_ref_article\" rel=\"nofollow noopener\" target=\"_blank\">Heart failure<\/a> is often diagnosed after patients have already lost much of their function and quality of life, but emerging tools that noninvasively measure heart function may offer new ways to catch the disease sooner.<\/p>\n<p><a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S2772963X25004260\" rel=\"nofollow noopener\" target=\"_blank\">A recent study published in\u00a0JACC: Advances<\/a> tested one such tool, the FDA-cleared Vivio System, which estimates left ventricular end-diastolic pressure (LVEDP), in more than 2000 patients at high risk for heart failure who were seen at three primary care centers.<\/p>\n<p>Nearly 40% of these patients, all of whom had diabetes, <a href=\"https:\/\/emedicine.medscape.com\/article\/238798-overview\" class=\"cl_ref_article\" rel=\"nofollow noopener\" target=\"_blank\">chronic kidney disease<\/a>, or suspected heart failure, had elevated LVDEP, an indicator of heart failure. More than two thirds also reported symptoms, limitations, or impaired quality of life. However, none had a prior diagnosis.<\/p>\n<p>Many heart failure diagnoses occur in the <a href=\"https:\/\/www.ahajournals.org\/doi\/10.1161\/CIRCHEARTFAILURE.121.008538\" rel=\"nofollow noopener\" target=\"_blank\">emergency room or inpatient setting<\/a>, according to lead study author Omar Cantu-Martinez, MD, from Healthcare Institute for Innovations in Quality, University of Missouri-Kansas City, and Saint Luke\u2019s Mid America Heart Institute, Kansas City, Missouri.<\/p>\n<p>\u201cWaiting until patients present with significant symptoms leads to worse outcomes and misses the opportunity for earlier detection and treatment that could prevent these hospitalizations,\u201d he said in an interview.<\/p>\n<p><strong>The Diagnostic Challenge in Primary Care\u00a0<\/strong><\/p>\n<p>Early signs of heart failure are easy to miss, according to Gregg Fonarow, MD, an American Heart Association (AHA) national volunteer expert and director of the Ahmanson-UCLA Cardiomyopathy Center.<\/p>\n<p>\u201cThis may be especially true for older adults or those with overlapping chronic conditions,\u201d Fonarow, who was not involved in the study, told Medscape Medical News. \u201cMild exertional intolerance or vague malaise may be dismissed as aging or deconditioning.\u201d<\/p>\n<p><img decoding=\"async\" class=\"ck-editor__editable ck-editor__nested-editable\" src=\"https:\/\/www.europesays.com\/ie\/wp-content\/uploads\/2025\/09\/ht-250905-ambar-kulshreshtha-120x156.jpg\" alt=\"photo of Amar Kulshrestha, MD\" height=\"156\" width=\"120\" data-asset-description=\"Amar Kulshrestha, MD\" data-asset-id=\"b6351f8f-c72e-417b-9be6-1537e3bedcf0\" data-asset-title=\"ht-250905-ambar-kulshreshtha-120x156.jpg\" data-creditline=\"Emory School of Medicine\" data-source=\"N\/A\" data-keywords=\"\" data-path=\"\/vim\/live\/professional_assets\/medscape\/images\/thumbnail_library\/ht-250905-ambar-kulshreshtha-120x156.jpg\" data-asset-url=\"https:\/\/www.europesays.com\/ie\/wp-content\/uploads\/2025\/09\/ht-250905-ambar-kulshreshtha-120x156.jpg\" data-height=\"156\" data-width=\"120\" role=\"textbox\" tabindex=\"-1\" contenteditable=\"true\" loading=\"lazy\"\/>Ambar Kulshreshtha, MD<\/p>\n<p>Symptoms such as fatigue, dyspnea, and edema may also be attributed to <a href=\"https:\/\/emedicine.medscape.com\/article\/297664-overview\" class=\"cl_ref_article\" rel=\"nofollow noopener\" target=\"_blank\">chronic obstructive pulmonary disease<\/a>, <a href=\"https:\/\/emedicine.medscape.com\/article\/123702-overview\" class=\"cl_ref_article\" rel=\"nofollow noopener\" target=\"_blank\">obesity<\/a>, <a href=\"https:\/\/emedicine.medscape.com\/article\/198475-overview\" class=\"cl_ref_article\" rel=\"nofollow noopener\" target=\"_blank\">anemia<\/a>, kidney disease, or <a href=\"https:\/\/emedicine.medscape.com\/article\/286759-overview\" class=\"cl_ref_article\" rel=\"nofollow noopener\" target=\"_blank\">depression<\/a>, noted Ambar Kulshreshtha, MD, a family physician and an associate professor in the Department of Family and Preventive Medicine at Emory University School of Medicine in Atlanta.<\/p>\n<p>\u201cThe biggest issue in detecting heart failure early is that the symptoms can be very nonspecific,\u201d Kulshreshtha said. He noted that referral to cardiology typically happens when an individual presents with dyspnea on exertion, abnormal <a href=\"https:\/\/emedicine.medscape.com\/article\/1894014-overview\" class=\"cl_ref_article\" rel=\"nofollow noopener\" target=\"_blank\">ECG<\/a> results or elevated <a href=\"https:\/\/emedicine.medscape.com\/article\/2087425-overview\" class=\"cl_ref_article\" rel=\"nofollow noopener\" target=\"_blank\">B-type natriuretic peptide<\/a> (BNP) levels.<\/p>\n<p>Short clinic visits, which make it hard to explore nuanced symptoms or perform thorough assessments, and fragmented care also complicate diagnosis, according to Fonarow.<\/p>\n<p>\u201cFurther care can be fragmented when specialists treat individual conditions without synthesizing the whole clinical practice,\u201d he said. \u201cThe primary care physician may believe if the patient had heart failure, it would have been recognized by one of the specialists involved in the patient\u2019s care, even though it was not.\u201d<\/p>\n<p><strong>Filling a Gap in Current Guidelines<\/strong><\/p>\n<p>LVEDP measurement has traditionally required cardiac catheterization, limiting its use to advanced or unconfirmed cases.<\/p>\n<p>Current <a href=\"https:\/\/emedicine.medscape.com\/article\/163062-overview?_gl=1*9rqqbr*_gcl_au*MTQ3MzE1NTM4My4xNzU2MzA0NzUwLjcyNjE3NDMyMS4xNzU2OTIzMDQwLjE3NTY5MjMwMzk.\" rel=\"nofollow noopener\" target=\"_blank\">American College of Cardiology guidelines<\/a> recommend confirmatory testing such as a BNP test and <a href=\"https:\/\/emedicine.medscape.com\/article\/1820912-overview\" class=\"cl_ref_article\" rel=\"nofollow noopener\" target=\"_blank\">echocardiography<\/a> once heart failure is suspected. <a href=\"https:\/\/decisionpoint.medscape.com\/cardiology\/viewarticle\/929486\" rel=\"nofollow noopener\" target=\"_blank\">Cardiac catheterization<\/a> may also be required in some advanced or unconfirmed cases.<\/p>\n<p>\u201cUntil now, tools that could noninvasively measure LVEDP \u2014 the gold-standard invasive hemodynamic measure for heart failure detection \u2014 were not available,\u201d Cantu-Martinez said.<\/p>\n<p>Noninvasive LVEDP screening may give primary care physicians a way to identify possible heart failure in patients who they may not have suspected to have the condition.<\/p>\n<p>\u201cA positive screen could prompt confirmatory testing and earlier referral to a heart failure specialist, rather than waiting for disease progression,\u201d Cantu-Martinez said. \u201cThe Vivio System can detect estimated elevated LVEDP with <a href=\"https:\/\/www.ahajournals.org\/doi\/10.1161\/circ.148.suppl_1.16544\" rel=\"nofollow noopener\" target=\"_blank\">80% sensitivity and 83% specificity<\/a>.\u201d<\/p>\n<p>In the study, one third of patients with elevated LVEDP were asymptomatic, while two thirds already reported symptoms on the Kansas City Cardiomyopathy Questionnaire (KCCQ-12). Twenty-six percent of patients with elevated LVEDP had severe symptoms consistent with NYHA class II or higher.<\/p>\n<p><img decoding=\"async\" class=\"ck-editor__editable ck-editor__nested-editable\" src=\"https:\/\/www.europesays.com\/ie\/wp-content\/uploads\/2025\/09\/ht-250905-gregg-fonarow-120x156.jpg\" alt=\"photo of Gregg Fonarow, MD\" height=\"156\" width=\"120\" data-asset-description=\"Gregg Fonarow, MD\" data-asset-id=\"f2f33544-da32-4eda-857c-17f3d87b49bd\" data-asset-title=\"ht-250905-gregg-fonarow-120x156.jpg\" data-creditline=\"UCLA Health\" data-source=\"N\/A\" data-keywords=\"\" data-path=\"\/vim\/live\/professional_assets\/medscape\/images\/thumbnail_library\/ht-250905-gregg-fonarow-120x156.jpg\" data-asset-url=\"https:\/\/www.europesays.com\/ie\/wp-content\/uploads\/2025\/09\/ht-250905-gregg-fonarow-120x156.jpg\" data-height=\"156\" data-width=\"120\" role=\"textbox\" tabindex=\"-1\" contenteditable=\"true\" loading=\"lazy\"\/>Gregg Fonarow, MD<\/p>\n<p>\u201cNoninvasive devices using a brachial blood pressure cuff and single-lead ECG to estimate LVEDP in minutes offer a practical alternative for earlier screening,\u201d Fonarow said.<\/p>\n<p>Other tools, such as <a href=\"https:\/\/www.medscape.com\/viewarticle\/ai-enhanced-ecg-predicts-hypertension-related-risks-2025a10000h5\" rel=\"nofollow noopener\" target=\"_blank\">AI-enhanced ECGs<\/a>, are also being studied as possible ways to flag patients at risk earlier.<\/p>\n<p>Kulshreshtha said tools like these could help fill gaps in primary care.<\/p>\n<p>\u201cThe <a href=\"https:\/\/www.medscape.com\/viewarticle\/974076\" rel=\"nofollow noopener\" target=\"_blank\">current guidelines<\/a> offer a structured framework for evaluating heart failure in primary care,\u201d he said. \u201cHowever, there is a need for more precise diagnostic algorithms, including clear thresholds for tests, to detect and triage heart failure earlier.\u201d<\/p>\n<p>Even when heart failure is identified, many patients still fall through the cracks after hospitalization. <a href=\"https:\/\/www.ahajournals.org\/doi\/full\/10.1161\/CIRCHEARTFAILURE.121.008335\" rel=\"nofollow noopener\" target=\"_blank\">Nearly 20% of patients are readmitted within 30 days of discharge<\/a>, and <a href=\"https:\/\/www.ahajournals.org\/doi\/full\/10.1161\/CIRCHEARTFAILURE.124.012357\" rel=\"nofollow noopener\" target=\"_blank\">many patients do not receive<\/a> all four <a href=\"https:\/\/www.jacc.org\/doi\/10.1016\/j.jacc.2021.02.006\" rel=\"nofollow noopener\" target=\"_blank\">guideline-directed therapies<\/a>, including an angiotensin receptor-neprilysin inhibitor, a beta-blocker, a mineralocorticoid receptor antagonist, and a SGLT2 inhibitor.<\/p>\n<p>\u201cThe earlier guideline-directed medical therapy is started, the better the outcomes,\u201d Fonarow said.<\/p>\n<p>The <a href=\"https:\/\/newsroom.heart.org\/news\/novel-virtual-care-program-enhances-at-home-support-for-people-with-heart-failure\" rel=\"nofollow noopener\" target=\"_blank\">AHA recently launched a virtual program<\/a> that provides ongoing cardiometabolic and heart failure management at home. Participating hospitals can refer patients before discharge, and once enrolled, patients receive remote monitoring devices, vital-sign tracking, and 24\/7 clinical support. The purpose is to reduce readmissions and extend evidence-based care to patients in their homes, especially those in underserved communities.<\/p>\n<p><strong>Feasibility in Primary Care<\/strong><\/p>\n<p>Some physicians may worry about adding yet another test to already overburdened clinic visits, but Cantu-Martinez said the Vivio System is designed for efficiency.<\/p>\n<p>\u201cHeart failure screening with the Vivio System and the KCCQ-12 takes <\/p>\n<p>Kulshreshtha agreed that integration is possible but emphasized the need for further validation.<\/p>\n<p>\u201cWith proper training and education, noninvasive LVEDP has the potential to be integrated into primary care practices,\u201d Kulshreshtha said. \u201cHowever, further studies are needed in primary care to validate and assess the effectiveness of this approach, and also to develop reimbursement models that support these novel diagnostics.\u201d<\/p>\n<p>The study also found that women were more likely to have elevated LVEDP than men.<\/p>\n<p>\u201cThe underdiagnosis and delayed recognition of <a href=\"https:\/\/www.medscape.com\/viewarticle\/722326\" rel=\"nofollow noopener\" target=\"_blank\">heart disease in women<\/a> are well-documented,\u201d Cantu-Martinez said. \u201cOur finding of a higher proportion of women with elevated LVEDP underscores the importance of maintaining a high index of suspicion for heart failure in women.\u201d<\/p>\n<p>He said combining LVEDP testing with the KCCQ-12 gives patients a way to articulate symptoms and functional limitations that might otherwise go unrecognized.<\/p>\n<p>\u201cThese approaches can support earlier detection and treatment of heart failure across all patients, regardless of sex or race, and may help reduce sex-based biases in diagnosis and care,\u201d he said.<\/p>\n<p>Experts agree that earlier detection of heart failure has the potential to lower hospitalization and mortality rates.<\/p>\n<p>\u201cOur goal is to catch heart failure before it catches our patients off guard,\u201d Cantu-Martinez said.<\/p>\n<p>Fonarow reported consulting for Abbott, Amgen, AstraZeneca, Bayer, Boehringer Ingelheim, Cytokinetics, Eli Lilly, Johnson &amp; Johnson, Medtronic, Merck, Novartis, and Pfizer.
