In a few short years, these medicines have reshaped obesity treatment, helping millions achieve weight loss that once required a surgeon’s referral. Prescribing rates continue to climb, and the early results have been, by most measures, remarkable. Yet attention is beginning to shift – from what these drugs can do to a considerably thornier question: how are those results supposed to last?

US behavior change company Noom is seeking to address that challenge with the launch of a free version of its GLP-1 Companion program, making the digital support platform available at no cost to anyone taking a GLP-1 medication, regardless of where they receive treatment. The program is designed to provide guidance around nutrition, side-effect management, physical activity and habit formation alongside pharmacological treatment.

Longevity.Technology: The launch of a free GLP-1 companion platform reflects a broader realization that is beginning to ripple through obesity medicine: losing weight and keeping it off are not the same challenge. While the first wave of enthusiasm around GLP-1s centered on their remarkable ability to drive weight loss, attention is increasingly turning to what happens next – particularly as real-world data suggest many patients discontinue treatment within the first year. Behavioral support is being repositioned accordingly; no longer a premium adjunct to pharmacotherapy, but something closer to structural scaffolding. The growing emphasis on muscle preservation and protein intake sits in the same frame – for longevity practitioners, maintaining lean mass during weight loss may ultimately prove as important as the number on the scale. That digital tools are now stepping into the gap between appointments is less a sign of innovation than of necessity: healthcare systems were not built to provide intensive lifestyle coaching at the scale these medications now demand. Whether any particular platform can deliver durable outcomes remains a question that will require continued scrutiny and robust long-term data, but the direction of travel is becoming clear. The obesity conversation is evolving from treatment to maintenance, from weight loss to healthspan – and perhaps from the seductive simplicity of a weekly injection to the rather less glamorous, but ultimately more consequential, business of building habits that last.

Beyond the prescription

The companion platform targets several of the friction points that commonly accompany GLP-1 therapy: protein adequacy, gastrointestinal side effects and the preservation of lean muscle during what can be a period of rapid weight change. Tools include personalized protein targets, medication tracking, educational content and exercise programming oriented around muscle health.

The emphasis is deliberate. Within both obesity medicine and longevity science, there is growing unease about the assumption that weight loss and metabolic improvement are straightforwardly synonymous – particularly when lean mass is lost in the process. For older adults especially, the implications for physical function and resilience are not trivial.

Geoff Cook, CEO of Noom, told Longevity.Technology that concern over lean-mass loss was a central consideration in the design of the GLP-1 Companion program. “A meaningful portion of total weight lost is lean mass, which has real implications for metabolic health,” he said, noting that rapid weight regain following discontinuation may also shift body composition in an unfavorable direction if muscle mass is not preserved.

Geoff Cook is the CEO of Noom

While Noom currently emphasizes resistance training and personalized protein targets as protective strategies, Cook told us the company is still building its evidence base around body composition outcomes. Data collected through the platform’s body composition scanning tools are beginning to generate what Cook described as encouraging signals, although the company has yet to publish formal analyses demonstrating effects on lean-mass retention or body composition outcomes.

Building habits for the long term

Noom is also positioning the program as a resource for clinicians, many of whom face practical constraints when attempting to provide ongoing lifestyle support within routine consultations.

“The medication gets people started, but what happens between appointments matters enormously,” said Dr Jeffrey Egler, Chief Medical Officer at Noom. “Providers prescribing GLP-1s want their patients to have nutrition guidance, side-effect support, and behavioral tools – but the structure of primary care doesn’t always make that possible. The free GLP-1 Companion isn’t designed to replace that relationship; it’s designed to extend it.”

The company points to evidence from its digital behavior change programs suggesting that structured support may improve long-term outcomes. According to Noom, users combining behavioral support with GLP-1 treatment achieve greater weight loss than medication alone, while randomized trial data from its broader platform indicate that participants continued to lose weight for an extended period after the formal intervention ended.

More recently, the company has generated preliminary data suggesting that users who engaged with its GLP-1 programs maintained most of their weight loss after discontinuing medication. The issue has become increasingly important as researchers seek ways to address the weight regain commonly observed after treatment is stopped. While those findings have not yet entered the peer-reviewed literature and will require further validation, they do align with increasing interest across the field in understanding how behavioral interventions can influence post-treatment trajectories.

Members can now get a range of free tools from Noom, plus additional features designed to build healthy habits The support gap

Cook told Longevity.Technology that behavioral support should increasingly be viewed as part of the obesity-treatment infrastructure rather than a premium service layered on top of medication. While GLP-1 labels have long specified that the drugs should be used alongside diet and exercise, he suggests the healthcare system has struggled to deliver that support at scale as adoption has accelerated.

“What we’re seeing now is a collision between the rapid adoption of these medications and the reality that the care system doesn’t exist to provide that support to tens of millions of people,” he explained. “The discontinuation data makes the stakes clear: most patients stop within a year, and without behavioral habits in place, the weight comes back quickly.” Cook added that by making the GLP-1 Companion free, Noom aims to become a default source of behavioral support recommended by doctors and providers.

Put another way, the next chapter of obesity medicine may be shaped as much by infrastructure as pharmacology.

“If the next few years of obesity care are going to be defined by broader GLP-1 access, the question isn’t just who gets the medication, it’s who gets the support to maximize outcomes,” he said.

The observation flags a tension that runs through obesity medicine at the moment – one that better drugs have sharpened rather than resolved. Clinicians have access to pharmacological tools of real efficacy; what remains elusive is the infrastructure to support patients through the longer arc of treatment, discontinuation and whatever comes after. Behavioral support companies are, not surprisingly, positioning themselves as that missing layer.

The launch of Noom’s new platform also illustrates how obesity medicine is converging with broader preventive health strategies. Rather than focusing solely on weight reduction, many emerging programs are incorporating measures of muscle health, nutrition quality and sustained behavior change – metrics that sit comfortably within a healthspan framework.

Reframing the conversation

GLP-1 therapies are no longer novel. They are becoming, steadily, an established feature of how obesity is treated – and the conversation is shifting accordingly, from the drama of early results to the less telegenic business of what comes next. Metabolic health, physical function, durable behavior change: these are the metrics that will define the field’s next chapter. For companies, clinicians and health systems, the task is building care infrastructure capable of supporting patients across that journey. It is a rather less glamorous undertaking than the medication that made it necessary – and probably a more important one.

Photographs courtesy of Noom

Digital companion aims to help patients preserve muscle, manage side effects and maintain weight loss beyond medication.
