{"id":1147771,"date":"2026-08-14T08:34:28","date_gmt":"2026-08-14T08:34:28","guid":{"rendered":"https:\/\/www.europesays.com\/uk\/1147771\/"},"modified":"2026-08-14T08:34:28","modified_gmt":"2026-08-14T08:34:28","slug":"which-is-really-worst-for-your-health-smoking-vaping-or-nicotine-pouches","status":"publish","type":"post","link":"https:\/\/www.europesays.com\/uk\/1147771\/","title":{"rendered":"Which is really worst for your health, smoking, vaping or nicotine pouches?"},"content":{"rendered":"<p>\t\t<img fetchpriority=\"high\" width=\"646\" height=\"339\" src=\"https:\/\/www.europesays.com\/uk\/wp-content\/uploads\/2026\/08\/SEI_308186731-f6fe.jpg\" class=\"article-image wp-image-29369131\" alt=\"Which is worst for your health, smoking, vaping or nicotine pouches? getty\/ Metro\" decoding=\"sync\"\/><br \/>\n\t\tThe fact is, no nicotine use is truly benign (Picture: Getty\/Metro)<\/p>\n<p>Cigarettes, vapes and <a data-ico=\"hyperlink-article\" href=\"https:\/\/metro.co.uk\/2026\/05\/20\/a-slimy-new-dating-habit-making-singles-lose-horn-28429946\/\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">nicotine pouches<\/a> all deliver the same addictive drug, nicotine, but they do so in very different ways \u2014 and people can be pretty territorial about their method of choice.<\/p>\n<p>Some smokers are wary of the <a data-ico=\"hyperlink-article\" href=\"https:\/\/metro.co.uk\/2025\/12\/01\/shocking-photo-shows-teens-blackened-shrivelled-lung-vaping-three-years-25027823\/\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">long-term effects of vaping<\/a>, seeing it as a case of better the devil you know, whereas e-cigarette and pouch loyals often defend their habit with \u2018at least it\u2019s not a cigarette\u2019.<\/p>\n<p>But which is actually worst for your health?<\/p>\n<p>As Dr Rayyan Zafar tells <strong>Metro<\/strong>, none of these options are benign, so \u2018if you do not use nicotine, do not start.\u2019<\/p>\n<p>However, the Imperial College neuropsychopharmacologist and senior researcher at Drug Science says<strong> <\/strong>there\u2019s a clear answer as to what does the most harm: \u2018Cigarettes, by a very large margin.\u2019<\/p>\n<p>\t\t\t\t\tAsk Metro<\/p>\n<p class=\"metro-disclaimer\">Use AI to go deeper into the stories you care about \u2013 powered by Metro and trusted publications.<\/p>\n<p>The main reason for this is combustion. When tobacco is burned, it produces carbon monoxide, fine particles and thousands of chemicals, including numerous carcinogens.<\/p>\n<p>\t\t<img width=\"646\" height=\"431\" src=\"https:\/\/www.europesays.com\/uk\/wp-content\/uploads\/2026\/08\/GettyImages-1420509183-e7a7.jpg\" class=\"article-image wp-image-29369935\" alt=\"\" decoding=\"async\" loading=\"lazy\"\/><br \/>\n\t\tCigarettes are worst \u2018by a very large margin\u2019 (Picture: Getty Images)<\/p>\n<p>Smoking is linked to a huge range of <a data-ico=\"hyperlink-article\" href=\"https:\/\/metro.co.uk\/2024\/12\/30\/much-life-lose-every-cigarette-smoke-22269692\/\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">serious health problems<\/a>, including lung <a data-ico=\"hyperlink-article\" data-track=\"inline-tag-auto-link_article\" href=\"https:\/\/metro.co.uk\/tag\/cancer\/\" rel=\"nofollow noopener\" target=\"_blank\">cancer<\/a> and cancers of the mouth, throat, larynx, oesophagus, pancreas, bladder and kidney.<\/p>\n<p>It also dramatically increases the risk of coronary heart disease, heart attack, stroke and peripheral vascular disease, while being a major cause of chronic obstructive pulmonary disease (COPD), chronic bronchitis and emphysema.<\/p>\n<p>According to Dr Zafar, the majority of these issues <a href=\"https:\/\/www.nice.org.uk\/guidance\/ng209\/chapter\/treating-tobacco-dependence\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">\u2018are caused by<\/a> inhaling the toxic chemicals produced when tobacco burns, rather than by nicotine itself.\u2019<\/p>\n<p>He adds: \u2018There is no safe level of cigarette smoking. Even smoking relatively few cigarettes can cause disproportionate cardiovascular harm.\u2019<\/p>\n<p>How harmful is vaping?<\/p>\n<p>This certainly doesn\u2019t mean vaping is harmless.<\/p>\n<p>As vapes don\u2019t burn tobacco, users <a href=\"https:\/\/www.gov.uk\/government\/publications\/nicotine-vaping-in-england-2022-evidence-update\/nicotine-vaping-in-england-2022-evidence-update-main-findings\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">are exposed to<\/a> \u2018substantially lower concentrations\u2019 of many of the chemicals associated with cancer, respiratory illness and cardiovascular disease found in cigarette smoke.<\/p>\n<p>But the vapour they create <a data-ico=\"hyperlink-article\" href=\"https:\/\/metro.co.uk\/2024\/04\/29\/vaping-stunt-brain-growth-teenagers-toxic-chemicals-20736472\/\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">may contain<\/a> fine particles, metals, aldehydes and flavouring-related chemicals that irritate or inflame the airways, causing <a href=\"https:\/\/www.cochrane.org\/evidence\/CD010216_can-electronic-cigarettes-help-people-stop-smoking-and-do-they-have-any-unwanted-effects-when-used\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">common side effects<\/a> like coughing, dry mouth, sore throat, headaches and nausea.<\/p>\n<p>\t\t<img width=\"646\" height=\"431\" temporarily raise heart rate and blood pressure, with high-strength modern vapes capable of delivering enough of it to create significant dependence.<\/p>\n<p>Still, Dr Zafar stresses that even without the \u2018decades of evidence needed to calculate vaping\u2019s precise <a data-ico=\"hyperlink-article\" href=\"https:\/\/metro.co.uk\/2023\/07\/10\/man-claims-vape-left-him-with-a-collapsed-lung-19097161\/\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">lifetime risks<\/a>\u2019, what we do have indicates it\u2019s a lot less damaging than smoking.<\/p>\n<p>And what about nicotine pouches?<\/p>\n<p>Then there\u2019s tobacco-free nicotine pouches, which Dr Zafar says \u2018may be the least harmful\u2019 of the three since there\u2019s no carbon monoxide and combustion involved. <\/p>\n<p>While studies suggest they\u2019re likely to be substantially less harmful than cigarettes though, he highlights that because these products are relatively new, research is limited in terms of long-term regular use.<\/p>\n<p>\t\t\t\t\tWhat do you think is the most harmful nicotine-related gum irritation, dry mouth, ulcers and white lesions where the pouch is placed, nausea, hiccups, headaches and an increased heart rate or blood pressure.<\/p>\n<p>There\u2019s also a risk of nicotine poisoning if swallowed, while the fact they deliver as much or more nicotine than a cigarette (albeit usually more slowly) has the potential to lead to a similar level of dependence.<\/p>\n<p>\u2018Nicotine pouches should <a data-ico=\"hyperlink-article\" href=\"https:\/\/metro.co.uk\/2024\/05\/29\/took-snus-17-year-old-now-im-warning-parents-20930385\/\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">not be used by children<\/a>, pregnant people or adults who do not already use nicotine,\u2019 explains Dr Zafar.\u00a0\u2018I\u00a0am particularly concerned about marketing which encourages users to start low\u2026 this needs to be urgently looked at.\u2019<\/p>\n<p>The verdict<\/p>\n<p>In terms of least to most harmful, Dr Zafar\u2019s broad ranking goes: No nicotine use or licensed nicotine-replacement therapy, tobacco-free nicotine pouches or regulated vapes, snus (pouches which contain tobacco), cigarettes.<\/p>\n<p>However, he says that the precise order in the middle is uncertain and can depend on the particular product, nicotine dose and pattern of use.<\/p>\n<p>\t\t<img width=\"646\" For a child or an adult who has never smoked, starting to vape or use pouches provides no health benefit and introduces a risk of dependence.<\/p>\n<p>\u2018Dual use is another important issue. Somebody who vapes or uses pouches but continues to smoke may retain much more smoking-related risk than they realise. The greatest health benefit comes from switching completely.\u2019<\/p>\n<p>Essentially \u2014 and to reiterate \u2014 \u2018being free from both tobacco and nicotine carries the lowest risk\u2019.
