Paauw, MD<\/p>\n<p>A 45-year-old woman presents with increasing tremor in her arms and a new tremor in her legs. These have been present for the past 3 weeks. She was in a motor vehicle accident 6 weeks ago and sustained a neck injury as well as increased fear, anxiety, and <a href=\"https:\/\/emedicine.medscape.com\/article\/1187829-overview\" class=\"cl_ref_article\" target=\"_blank\" rel=\"noopener\">insomnia<\/a> since the accident. Her exam shows a postural tremor which is mild. Current medications: <a href=\"https:\/\/reference.medscape.com\/drug\/zoloft-sertraline-342962\" class=\"cl_ref_drugs\" target=\"_blank\" rel=\"noopener\">sertraline<\/a>, zolpidem, <a href=\"https:\/\/reference.medscape.com\/drug\/ultram-conzip-tramadol-343324\" class=\"cl_ref_drugs\" target=\"_blank\" rel=\"noopener\">tramadol<\/a>, and <a href=\"https:\/\/reference.medscape.com\/drug\/aleve-anaprox-naproxen-343296\" class=\"cl_ref_drugs\" target=\"_blank\" rel=\"noopener\">naproxen<\/a>.<\/p>\n<p>Which medication is the most likely cause of the tremors?\u00a0<br \/>\u2003A) Sertraline\u00a0<br \/>\u2003B) Zolpidem\u00a0<br \/>\u2003C) Tramadol\u00a0<br \/>\u2003D) Naproxen<\/p>\n<p>The likely cause of her tremor is sertraline, a selective serotonin reuptake inhibitor (SSRI). It is possible the patient has <a href=\"https:\/\/emedicine.medscape.com\/article\/2500075-overview\" class=\"cl_ref_article\" target=\"_blank\" rel=\"noopener\">serotonin syndrome<\/a> with the combination of sertraline and tramadol. Tremors are almost always present with serotonin syndrome. SSRIs can cause tremors separate from serotonin syndrome. The <a href=\"https:\/\/www.prd-journal.com\/article\/S1353-8020(01)00043-8\/abstract\" target=\"_blank\" rel=\"noopener\">tremor<\/a> is usually postural and resolves with discontinuation of the medication. In one case series of <a href=\"https:\/\/www.psychiatrist.com\/pcc\/exacerbation-essential-tremor-sertraline-use\/\" target=\"_blank\" rel=\"noopener\">fluoxetine-induced tremor<\/a>, it lasted for months after discontinuation.<\/p>\n<p>Hyponatremia<\/p>\n<p>Another important, underappreciated side effect of SSRIs is <a href=\"https:\/\/emedicine.medscape.com\/article\/242166-overview\" class=\"cl_ref_article\" target=\"_blank\" rel=\"noopener\">hyponatremia<\/a>. SSRI use is more common in patients who are admitted with severe (Na hyponatremia. A meta-analysis of 39 studies looking at antidepressant exposure risk and development of hyponatremia found the highest risk being in patients exposed to serotonin-<a href=\"https:\/\/reference.medscape.com\/drug\/levarterenol-levophed-norepinephrine-342443\" class=\"cl_ref_drugs\" target=\"_blank\" rel=\"noopener\">norepinephrine<\/a> reuptake inhibitors (7.44%) and SSRIs (5.59%); <a href=\"https:\/\/www.cambridge.org\/core\/journals\/european-psychiatry\/article\/risk-of-antidepressantinduced-hyponatremia-a-metaanalysis-of-antidepressant-classes-and-compounds\/3ABCD6CF7AD23D03003F93E4F648AEC0\" target=\"_blank\" rel=\"noopener\">mirtazapine<\/a> was much less likely to be associated with hyponatremia (1%). Hyponatremia due to SSRIs is <a href=\"https:\/\/journals.lww.com\/inpj\/fulltext\/2021\/30010\/a_study_to_understand_the_pattern_of_hyponatremia.20.aspx\" target=\"_blank\" rel=\"noopener\">elevated<\/a> in elderly patients and women. Simultaneous use of <a href=\"https:\/\/academic.oup.com\/ijnp\/article\/15\/6\/739\/760620?\" target=\"_blank\" rel=\"noopener\">diuretics<\/a> and angiotensin converting enzyme inhibitors further increases the risk of hyponatremia in patients who use SSRIs.<\/p>\n<p>Yawning<\/p>\n<p>Yawning is an interesting side effect of SSRIs. In a case report by Sinha and colleagues, a patient developed frequent <a href=\"https:\/\/www.cureus.com\/articles\/259946-yawning-as-a-rare-side-effect-with-increased-escitalopram-dose-a-case-report#!\/\" target=\"_blank\" rel=\"noopener\">yawning<\/a> (5 times every 15 minutes) when her <a href=\"https:\/\/reference.medscape.com\/drug\/lexapro-escitalopram-342961\" class=\"cl_ref_drugs\" target=\"_blank\" rel=\"noopener\">escitalopram<\/a> dose was increased from 5 mg to 15 mg. Roncero et al described a case of yawning in a patient on escitalopram that resolved when the <a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0213485312001880?via%3Dihub\" target=\"_blank\" rel=\"noopener\">dose of escitalopram<\/a> was reduced. In a review of <a href=\"https:\/\/www.sciencedirect.com\/science\/article\/abs\/pii\/S075549821400147X?via%3Dihub\" target=\"_blank\" rel=\"noopener\">drug-induced yawning<\/a>, SSRIs as a class were most frequently involved, and sertraline, <a href=\"https:\/\/reference.medscape.com\/drug\/prozac-fluoxetine-342955\" class=\"cl_ref_drugs\" target=\"_blank\" rel=\"noopener\">fluoxetine<\/a>, and <a href=\"https:\/\/reference.medscape.com\/drug\/paxil-brisdelle-paroxetine-342959\" class=\"cl_ref_drugs\" target=\"_blank\" rel=\"noopener\">paroxetine<\/a> were implicated.<\/p>\n<p>Bleeding Risk<\/p>\n<p>SSRIs have been linked to increased bleeding risk, especially upper gastrointestinal hemorrhage. Laporte and colleagues showed an association of SSRI use and <a href=\"https:\/\/www.sciencedirect.com\/science\/article\/abs\/pii\/S1043661816307769?via%3Dihub\" target=\"_blank\" rel=\"noopener\">risk of bleeding<\/a> in a meta-analysis of 42 observational studies, with an odds ratio of 1.41 (95% CI, 1.27-1.57; P NSAIDs alone. Make sure to think about patients\u2019 bleeding risks when starting SSRIs.<\/p>\n<p>Pearls: Yawning and tremors are common issues in primary care \u2014 consider SSRIs as a possible cause. Look at the medication list in patients with severe hyponatremia and consider SSRIs as a possible cause.
