{"id":126813,"date":"2025-05-24T02:19:09","date_gmt":"2025-05-24T02:19:09","guid":{"rendered":"https:\/\/www.europesays.com\/uk\/126813\/"},"modified":"2025-05-24T02:19:09","modified_gmt":"2025-05-24T02:19:09","slug":"is-the-inquest-into-the-bondi-junction-murders-further-stigmatising-schizophrenia-health","status":"publish","type":"post","link":"https:\/\/www.europesays.com\/uk\/126813\/","title":{"rendered":"Is the inquest into the Bondi Junction murders further stigmatising schizophrenia? | Health"},"content":{"rendered":"<p class=\"dcr-16w5gq9\">When Sandy Jeffs was diagnosed with schizophrenia in 1976, \u201cit was an absolute death sentence because schizophrenia and recovery weren\u2019t spoken of in the same sentence back in those days\u201d.<\/p>\n<p class=\"dcr-16w5gq9\">\u201cTo have your future taken away from you like that, at 23 years of age, was just awful,\u201d she says, \u201cand I bought into the pessimism of my diagnosis because that\u2019s what I was told by all those clinicians.\u201d<\/p>\n<p class=\"dcr-16w5gq9\">Jeffs went from being a university student to \u201cunravelling\u201d \u2013 graduating but not being able to get a job, and needing admission into the now-closed Larundel mental hospital in Melbourne<strong>.<\/strong><\/p>\n<p class=\"dcr-16w5gq9\">When <a href=\"https:\/\/www.tandfonline.com\/doi\/full\/10.1080\/14490854.2022.2028559\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\">Australia began closing these asylums in the late 1980s<\/a>, it was the start of Jeffs\u2019s career as a \u201cpublic loony\u201d \u2013 as she is proud to call herself \u2013 which placed her among the first wave of people to speak openly about living with mental illness.<\/p>\n<p class=\"dcr-16w5gq9\">As well as her work as a mental health advocate, Jeffs is a poet, author, hockey player and violinist. But despite being involved in several anti-stigma campaigns, she says complex mental illnesses like schizophrenia have never become normalised in the way anxiety and depression have. \u201cHollywood hasn\u2019t helped either depicting us either as mass murderers or mad geniuses,\u201d Jeffs said. \u201cNo one is ordinary like me.\u201d<\/p>\n<p class=\"dcr-16w5gq9\">Now, after the \u201crelentless negative spotlight\u201d cast by the <a href=\"https:\/\/www.theguardian.com\/australia-news\/2025\/apr\/28\/sole-cctv-operator-using-bathroom-when-bondi-junction-attack-began-sydney-court-hears-ntwnfb\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\">five-week Bondi Junction inquest<\/a> into the Joel Cauchi mass stabbing murders, she wonders \u201ccan we ever de-stigmatise schizophrenia?\u201d<\/p>\n<p class=\"dcr-16w5gq9\">The coronial inquest is investigating the circumstances surrounding Cauchi killing six people at Westfield Bondi Junction on 13 April 2024 before he was fatally shot <a href=\"https:\/\/www.theguardian.com\/australia-news\/2024\/apr\/13\/police-officer-who-shot-bondi-junction-attacker-wielding-a-massive-knife-hailed-as-a-hero\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\">by a police officer.<\/a><\/p>\n<p class=\"dcr-16w5gq9\">Cauchi, 40, was diagnosed with schizophrenia as a teenager. His condition had been appropriately managed but <a href=\"https:\/\/www.theguardian.com\/australia-news\/2025\/may\/13\/joel-cauchi-psychiatrist-schizophrenia-medication-westfield-bondi-junction-stabbings-inquest-ntwnfb\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\">the inquiry heard he was \u201clost to follow up\u201d<\/a> with healthcare professionals after he moved from Toowoomba in 2020.<\/p>\n<p class=\"dcr-16w5gq9\">Cauchi\u2019s diagnosis and care, and their potential link to the mass murders, has been heavily scrutinised during the inquest, which set to continue for another week.<\/p>\n<p class=\"dcr-16w5gq9\">The spotlight has caused distress among many people with schizophrenia, fearing they will be further judged and ostracised.<\/p>\n<p class=\"dcr-16w5gq9\">\u201cI just don\u2019t know how we\u2019re going to come back from this,\u201d Jeffs says. \u201cIt\u2019s going to be so difficult to even hold our heads high, let alone declare to an audience: \u2018I have schizophrenia. I am not a monster.\u2019\u201d<\/p>\n<p class=\"dcr-16w5gq9\">Many friends of Jeffs are now afraid to say they have schizophrenia, she says. She fears the damage will be \u201cirreversible\u201d for young people just diagnosed.<\/p>\n<p>\u2018A very sensational effect\u2019<\/p>\n<p class=\"dcr-16w5gq9\">Schizophrenia was already known to be <a href=\"https:\/\/www.cambridge.org\/core\/journals\/bjpsych-open\/article\/creating-a-hierarchy-of-mental-health-stigma-testing-the-effect-of-psychiatric-diagnosis-on-stigma\/3F7F87E0D4F50412B2A90072CFD8B995\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\">one of the most stigmatised mental health conditions<\/a>.<strong> <\/strong>The Bondi Junction inquest is likely to amplify that, especially as \u201cthey\u2019re so clearly using the diagnostic label\u201d, the deputy director of the University of Melbourne\u2019s Centre for Mental Health, Prof Nicola Reavley, says.<\/p>\n<p class=\"dcr-16w5gq9\">Because schizophrenia is an uncommon condition, people are more likely to be influenced by media reporting, Reavley says.<\/p>\n<p class=\"dcr-16w5gq9\">It could also be leading people living with schizophrenia to withdraw from activities and opportunities due to \u201canticipated discrimination\u201d, she says.<\/p>\n<p>Sandy Jeffs says people with schizophrenia \u2018have to have incentive to stay well\u2019. Photograph: Ellen Smith\/The Guardian<\/p>\n<p class=\"dcr-16w5gq9\">The CEO of Sane, Rachel Green, leads a national mental health organisation for people with complex mental health issues, says \u201cwe\u2019re hearing from people in our community \u2013 who live with schizophrenia and family members \u2013 who are just devastated by the scope and the scale of this inquiry.\u201d<\/p>\n<p class=\"dcr-16w5gq9\">She says the \u201congoing, intensive reporting\u201d on Cauchi and his schizophrenia during the inquest is \u201cobviously newsworthy\u201d and of real public interest, \u201cbut it is only telling one single particular story and the worst case scenario.\u201d<\/p>\n<p class=\"dcr-16w5gq9\">When coronial inquiries conclude, they can produce nuanced findings. But while the hearings are in progress, being reported piecemeal, \u201cit\u2019s got a very sensational effect,\u201d Green says.<\/p>\n<p class=\"dcr-16w5gq9\">Ever since Harry Jordan <a href=\"https:\/\/www.abc.net.au\/listen\/programs\/allinthemind\/my-brain-snapped-sudden-psychosis\/104522802\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\">experienced two episodes of psychosis as a teenager<\/a>, the 30-year-old is constantly educating people \u201cabout what it was and what it wasn\u2019t\u201d. \u201cA lot of people just are completely ignorant as to what a complex mental illness is. We use Google reCaptcha to protect our website and the Google <a data-ignore=\"global-link-styling\" href=\"https:\/\/policies.google.com\/privacy\" rel=\"noreferrer noopener\" class=\"dcr-1rjy2q9\" target=\"_blank\">Privacy Policy<\/a> and <a data-ignore=\"global-link-styling\" href=\"https:\/\/policies.google.com\/terms\" rel=\"noreferrer noopener\" class=\"dcr-1rjy2q9\" target=\"_blank\">Terms of Service<\/a> apply.<\/p>\n<p id=\"EmailSignup-skip-link-24\" tabindex=\"0\" aria-label=\"after newsletter promotion\" role=\"note\" class=\"dcr-jzxpee\">after newsletter promotion<\/p>\n<p class=\"dcr-16w5gq9\">The NSW chair of the Royal Australian and New Zealand College of Psychiatrists, Dr Pramudie Gunaratne, says \u201cone of the additional dangers of this tragedy is that it does further stigmatise people with severe mental illness, and particularly because these are people who are already so marginalised.\u201d<\/p>\n<p class=\"dcr-16w5gq9\">The inquiry draws a link between schizophrenia and criminality, which<a href=\"https:\/\/www.theguardian.com\/australia-news\/2025\/may\/22\/bondi-junction-inquest-told-most-people-with-treatment-resistant-schizophrenia-relapse-without-medication-ntwnfb\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\"> does not apply to the vast majority of people that have schizophrenia<\/a>, Gunaratne says.<\/p>\n<p class=\"dcr-16w5gq9\">Schizophrenia is a serious, long-term mental health condition, and one of the symptoms of schizophrenia is psychosis. During an episode of psychosis, a person\u2019s thoughts and perceptions are disrupted, so they may have difficulty recognising what\u2019s real and what\u2019s not, Gunaratne says.<\/p>\n<p class=\"dcr-16w5gq9\">\u201cIt\u2019s an illness, just like any other medical condition, it\u2019s the hand that you\u2019re dealt with in the lottery of life. These are people who need access to healthcare, but our systems are failing them.\u201d<\/p>\n<p>The missing voice<\/p>\n<p class=\"dcr-16w5gq9\">Before Dr Margaret Leggatt became the co-founder of Sane, her work in the 1980s was to ensure the people with mental illness being discharged from institutions had the support they needed.<\/p>\n<p class=\"dcr-16w5gq9\">But the funding from the institutions <a href=\"https:\/\/theconversation.com\/from-asylums-to-gp-clinics-the-missing-middle-in-mental-health-care-46345#:~:text=Most%20of%20Australia&#039;s%20asylums%20were,half%20a%20billion%20dollars%20annually.\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\">that was supposed to follow people<\/a> into community-based treatment never materialised, Leggatt says.<\/p>\n<p class=\"dcr-16w5gq9\">What\u2019s more, as mental health services have become more stretched, Gunaratne says the few available have shrunk to cover hospital emergency departments and inpatient units.<\/p>\n<p class=\"dcr-16w5gq9\">If someone becomes unwell when they\u2019ve been discharged into the care of their GP or a private service, there is no pathway for them to be referred back until they reach a crisis point, she says.<\/p>\n<p class=\"dcr-16w5gq9\">Leggatt says \u201cJoel Cauchi is the end result of our complete and utter failure to provide the sort of services in the community that somebody like Joel needed.\u201d<\/p>\n<p class=\"dcr-16w5gq9\">There has been no national public conversation about complex mental illness or schizophrenia, Green says, and the Bondi Junction inquest is problematically \u201csort of serving as a proxy for it.\u201d<\/p>\n<p class=\"dcr-16w5gq9\">While the inquest might produce good recommendations for the specific circumstances of the Bondi Junction tragedy, there won\u2019t be a discussion about broader systemic changes, Green says.<\/p>\n<p class=\"dcr-16w5gq9\">There is still no data on how many Australians live with schizophrenia, are employed, can access care or have trouble maintaining care because they move interstate, Green says \u2013 \u201cso how the hell are we planning services or responses?\u201d<\/p>\n<p class=\"dcr-16w5gq9\">She says there is a voice \u201cthat\u2019s missing\u201d in the big inquest story: \u201cWe\u2019re not hearing the voice of people who live well and why.\u201d<\/p>\n<p class=\"dcr-16w5gq9\">Jeffs says what keeps her in good health is having \u201cteam Sandy\u201d \u2013 made up of not just her clinicians, but also the people she has shared a safe, secure home with the last 15 years, her sport, music, and writing, as well as her friends and pets.<\/p>\n<p class=\"dcr-16w5gq9\"><a href=\"https:\/\/www.thelancet.com\/journals\/lanpsy\/article\/piis2215-0366(21)00252-2\/fulltext\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\">The gold standard of mental health care is the Trieste open door model<\/a> about supporting people, socially, financially and psychologically, to enhance their wellbeing, Jeffs says. She laments that Australia does not invest in that wrap around support.<\/p>\n<p class=\"dcr-16w5gq9\">\u201cIf you\u2019re homeless, and you\u2019re not cared for, and you have no team around you \u2013 where\u2019s the incentive to stay well?\u201d<\/p>\n<p class=\"dcr-16w5gq9\"> In Australia, the crisis support service <a href=\"https:\/\/www.lifeline.org.au\/\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\">Lifeline<\/a> is 13 11 14.
