Globally, more than 150 million people are projected to develop dementia by 2050.<\/p>\n<p>In the United States, this growing burden will increasingly strain families, caregivers, and the healthcare system\u2014especially in underserved communities.<\/p>\n<p>Researchers analyzed data from more than 25 million traditional Medicare beneficiaries between 2015 and 2021, focusing on trends in dementia incidence and prevalence across age, sex, race\/ethnicity, and socioeconomic groups.<\/p>\n<p>They made the following key findings:<\/p>\n<ul class=\"wp-block-list\">\n<li>Age- and sex-adjusted incidence of dementia fell from 3.5% in 2015 to 2.8% in 2021.<\/li>\n<li>At the same time, prevalence rose from 10.5% to 11.8%, with nearly 2.9 million beneficiaries\u2014about 12% of traditional Medicare enrollees\u2014living with a dementia diagnosis by 2021.<\/li>\n<li>Dementia incidence was highest among Black beneficiaries (3.1%), followed by White (2.8%) and Hispanic (2.6%) beneficiaries in 2021.<\/li>\n<li>Higher rates of dementia were consistently found in women, racial and ethnic minorities, and people living in socioeconomically disadvantaged neighborhoods.<\/li>\n<\/ul>\n<p><strong>Funding: <\/strong>This work was funded by the\u00a0Duke-UNC Alzheimer\u2019s Disease Research Center (ADRC)\u00a0under award P30AG072958 from the National Institute on Aging. BMJ<\/p>\n<p><strong>Abstract<\/strong><\/p>\n<p><strong>Incidence and prevalence of dementia among US Medicare beneficiaries, 2015-21: population based study<\/strong><\/p>\n<p id=\"p-2\"><strong>Objective<\/strong>\u00a0<\/p>\n<p id=\"p-2\">To determine the incidence and prevalence of dementia in a nationally representative cohort of US Medicare beneficiaries, stratified by important subgroups.<\/p>\n<p id=\"p-3\"><strong>Design<\/strong>\u00a0<\/p>\n<p id=\"p-3\">Population based study.<\/p>\n<p id=\"p-4\"><strong>Setting<\/strong>\u00a0<\/p>\n<p id=\"p-4\">Nationwide study between 2015 and 2021.<\/p>\n<p id=\"p-5\"><strong>Participants<\/strong>\u00a0<\/p>\n<p id=\"p-5\">Fee-for-service Medicare beneficiaries aged 66 or older with at least one year of continuous enrollment.<\/p>\n<p id=\"p-6\"><strong>Main outcome measures<\/strong>\u00a0<\/p>\n<p id=\"p-6\">Incidence and prevalence of dementia, calculated as percentage per person years or percentage of beneficiaries respectively. These metrics were also calculated in key subgroups defined by age, sex, race\/ethnicity, and neighborhood socioeconomic status.<\/p>\n<p id=\"p-7\"><strong>Results<\/strong>\u00a0<\/p>\n<p id=\"p-7\">A total of 5\u2009025\u2009039 incident cases of dementia were documented from 2015 to 2021. The overall age and sex standardized incidence decreased between 2015 and 2021 from 3.5% to 2.8%.<\/p>\n<p id=\"p-7\">Prevalence increased during this time from 10.5% to 11.8%. Male beneficiaries had a higher age standardized incidence than did female beneficiaries in 2015 (3.5%\u00a0v\u00a03.4%), a difference that widened by 2021 (2.9%\u00a0v\u00a02.6%; estimated difference-in-difference 0.94, 95% confidence interval (CI) 0.94 to 0.95; P<\/p>\n<p id=\"p-7\">Incidence was highest in 2015 for black beneficiaries (4.2%), followed by Hispanic beneficiaries (3.7%) and white beneficiaries (3.4%), and in 2021 for black beneficiaries (3.1%) followed by white beneficiaries (2.8%) and Hispanic beneficiaries (2.6%); the difference between white and black beneficiaries narrowed from 2015 to 2021 (difference-in-difference 0.92, 95% CI 0.91 to 0.93; P<\/p>\n<p id=\"p-8\"><strong>Conclusions<\/strong>\u00a0<\/p>\n<p id=\"p-8\">The incidence of dementia decreased from 2015 to 2021, but the prevalence increased.
