It is a white short haired cat, Elise is similing and wearing a long sleeved black and white top\" class=\"sc-d1200759-0 dvfjxj\"\/>Family handout<\/p>\n<p>The death of 16-year-old Elise Sebastian is being monitored by the Lampard Inquiry, a public inquiry into mental health deaths in Essex<\/p>\n<p class=\"sc-9a00e533-0 hxuGS\">Neglect at a mental health unit contributed to the death of a &#8220;happy and outgoing&#8221; 16-year-old, an inquest jury has heard.<\/p>\n<p class=\"sc-9a00e533-0 hxuGS\">Elise Sebastian, from Southminster near Maldon, Essex, was found unresponsive in her bedroom at the St Aubyn Centre in Colchester, in April 2021.<\/p>\n<p class=\"sc-9a00e533-0 hxuGS\">The teenager was supposed to have been receiving one-to-one care but, the inquest heard, an infrared alert system &#8211; which warned staff if she had been in her bathroom for too long &#8211; was muted, and she had been left alone for 28 minutes.<\/p>\n<p class=\"sc-9a00e533-0 hxuGS\">An inquest jury at Essex Coroner&#8217;s Court unanimously agreed that Elise&#8217;s death could have been prevented if it were not for multiple failures in her care.<\/p>\n<p class=\"sc-9a00e533-0 hxuGS\">The St Aubyn Centre, described in the hearing as &#8220;chaotic&#8221;, is run by the Essex Partnership University NHS Foundation Trust (EPUT), currently the subject of an ongoing public inquiry.<\/p>\n<p class=\"sc-9a00e533-0 hxuGS\">Pravin Fernando, representing EPUT, admitted that Elise&#8217;s room<a target=\"_self\" href=\"https:\/\/www.bbc.co.uk\/news\/articles\/c33zyg1y666o\" class=\"sc-f9178328-0 bGFWdi\" rel=\"noopener\"> should have been locked<\/a> to prevent the teenager entering alone, adding: &#8220;The trust accepts that these failings were causative of her death.&#8221;<\/p>\n<p><img decoding=\"async\" src=\"https:\/\/www.europesays.com\/uk\/wp-content\/uploads\/2025\/05\/1747930394_975_grey-placeholder.png\" class=\"sc-d1200759-0 dkIvM hide-when-no-script\"\/><img decoding=\"async\"   src=\"https:\/\/www.europesays.com\/uk\/wp-content\/uploads\/2025\/05\/d43ba7b0-3bc9-11f0-a7a6-addda7719864.png.webp.webp\" loading=\"lazy\" alt=\"Family handout Teenage girl with long dark shiny hair and a gentle smile. She is wearing cool green nail polish and holding a mobile phone.\" class=\"sc-d1200759-0 dvfjxj\"\/>Family handout<\/p>\n<p>Elise was found unresponsive in her room in the St Aubyn Centre in Colchester, in April 2021<\/p>\n<p class=\"sc-9a00e533-0 hxuGS\">The inquest heard Elise had secured a place at Writtle College to work with animals and her parents described her as a &#8220;happy, outgoing, funny little girl&#8221;.<\/p>\n<p class=\"sc-9a00e533-0 hxuGS\">She had been diagnosed with autism and her parents had become increasingly concerned about her anxiety and depression.<\/p>\n<p class=\"sc-9a00e533-0 hxuGS\">On 4 March 2021, after she had self-harmed a number of times, she was admitted for the second time to a ward at the unit.<\/p>\n<p>&#8216;Poor wi-fi&#8217;<\/p>\n<p class=\"sc-9a00e533-0 hxuGS\">Essex area coroner Sonia Hayes heard how Elise had asked to go to the toilet, and although some doors may have been left open, others needed a staff swipe card to let her through.<\/p>\n<p class=\"sc-9a00e533-0 hxuGS\">The ward had introduced an infrared monitoring system called Oxevision, two months earlier as part of a trial, to reduce the risk of patients self-harming in isolated areas.<\/p>\n<p class=\"sc-9a00e533-0 hxuGS\">Handheld monitoring tablets were not working because of poor wi-fi, and the unit&#8217;s clinical lead, Brian O&#8217;Donnell, did not think having someone allocated to monitoring the alerts on the main computer would be a &#8220;good use of their time&#8221;.<\/p>\n<p class=\"sc-9a00e533-0 hxuGS\">During the inquest, expert witness Prof Jasmeet Soar said that when Elise was found, she had been in cardiac arrest for nine minutes.<\/p>\n<p class=\"sc-9a00e533-0 hxuGS\">He said there had been a four to five minute delay in staff using an oxygen mask, and a defibrillator took seven minutes for staff to use, instead of the three minute maximum mentioned in guidelines.<\/p>\n<p class=\"sc-9a00e533-0 hxuGS\">&#8220;Someone should have got the defibrillator and started it earlier,&#8221; he added.<\/p>\n<p class=\"sc-9a00e533-0 hxuGS\">Prof Soar said the defibrillator also did not appear to have been used properly, from the readings he obtained, and had been switched on and off.<\/p>\n<p class=\"sc-9a00e533-0 hxuGS\">He said: &#8220;Some of the delays could have been avoided if staff would have known how to use the equipment.&#8221;<\/p>\n<p><img decoding=\"async\" src=\"https:\/\/www.europesays.com\/uk\/wp-content\/uploads\/2025\/05\/1747930394_975_grey-placeholder.png\" class=\"sc-d1200759-0 dkIvM hide-when-no-script\"\/><img decoding=\"async\"   src=\"https:\/\/www.europesays.com\/uk\/wp-content\/uploads\/2025\/05\/7287cbd0-3b50-11f0-bd2d-47954cf23778.jpg.webp.webp\" loading=\"lazy\" alt=\"Family handout A young girl holding a small chick and smiling at the camera. She is wearing a white top and has fringe over her face\" class=\"sc-d1200759-0 dvfjxj\"\/>Family handout<\/p>\n<p>Elise Sebastian was a &#8220;happy, outgoing, funny little girl&#8221; who dreamed of working with animals, her family said<\/p>\n<p class=\"sc-9a00e533-0 hxuGS\">The inquest also heard how the majority of staff in the unit were bank and agency workers who would not have known the children well.<\/p>\n<p class=\"sc-9a00e533-0 hxuGS\">Ms Hayes said she heard the ward &#8220;was clearly short staffed&#8221; and described how medical records appeared to be incomplete at times.<\/p>\n<p class=\"sc-9a00e533-0 hxuGS\">Mr O&#8217;Donnell said he had &#8220;constantly&#8221; raised concerns about staffing numbers with senior management, adding: &#8220;Our budget would always be overspent &#8211; my argument was that we were underfunded.&#8221;<\/p>\n<p class=\"sc-9a00e533-0 hxuGS\">The inquest heard how observations should have a therapeutic interaction but some thought that was impossible to achieve.<\/p>\n<p class=\"sc-9a00e533-0 hxuGS\">Consultant child and adolescent psychiatrist Dr Malcolm Bourne also told the hearing that &#8220;staff needed support to deal with children who were challenging and that autism could make depression more difficult to treat&#8221;.<\/p>\n<p><img decoding=\"async\" src=\"https:\/\/www.europesays.com\/uk\/wp-content\/uploads\/2025\/05\/1747930394_975_grey-placeholder.png\" class=\"sc-d1200759-0 dkIvM hide-when-no-script\"\/><img decoding=\"async\"   src=\"https:\/\/www.europesays.com\/uk\/wp-content\/uploads\/2025\/05\/0d7ea2a0-3b54-11f0-8d7e-8fd248e6bb27.jpg.webp.webp\" loading=\"lazy\" alt=\"EPUT The front of the st aubyn centre which is a red brick building with a cycle rack outside. It opened in 2012.<\/p>\n<p class=\"sc-9a00e533-0 hxuGS\">Shortly after Elise&#8217;s death, the ward at the St Aubyn Centre was closed to new admissions by the Care Quality Commission.<\/p>\n<p class=\"sc-9a00e533-0 hxuGS\"><a target=\"_blank\" href=\"https:\/\/www.cqc.org.uk\/provider\/R1L\/inspection-summary#mhadolescent\" class=\"sc-f9178328-0 bGFWdi\" rel=\"noopener\">The CQC report<\/a> found understaffing was a &#8220;significant concern&#8221; made worse by managers not ensuring staff had the appropriate skills and experience to look after vulnerable patients.<\/p>\n<p>&#8216;Deepest condolences&#8217;<\/p>\n<p class=\"sc-9a00e533-0 hxuGS\">The jury foreman told the inquest there were two main factors that probably caused the teenager&#8217;s death.<\/p>\n<p class=\"sc-9a00e533-0 hxuGS\">The first was &#8220;poorly administered observations due to poor staffing levels and falsified information on observation forms&#8221;.<\/p>\n<p class=\"sc-9a00e533-0 hxuGS\">He said the second was &#8220;Elise being able to gain access into her room and her observation level in an isolated area not being considered&#8221;, which the jury believed had directly led to Elise&#8217;s death.<\/p>\n<p class=\"sc-9a00e533-0 hxuGS\">The coroner said she would write a prevention of future death report which would recommend changes to make sure failures were not repeated.<\/p>\n<p class=\"sc-9a00e533-0 hxuGS\">Paul Scott, chief executive of EPUT, said: &#8220;I want to say sorry to Elise&#8217;s family and to everyone who loved her that she did not receive the care she deserved, and I offer my deepest condolences.&#8221;<\/p>\n","protected":false},"excerpt":{"rendered":"Nikki Fox BBC health correspondent, Essex Reporting fromEssex Coroner&#8217;s CourtShivani Chaudhari BBC News, Essex Family handout The death&hellip;\n","protected":false},"author":2,"featured_media":62294,"comment_status":"","ping_status":"","sticky":false,"template":"","format":"standard","meta":{"footnotes":""},"categories":[4317],"tags":[105,218,16,15],"class_list":{"0":"post-139705","1":"post","2":"type-post","3":"status-publish","4":"format-standard","5":"has-post-thumbnail","7":"category-mental-health","8":"tag-health","9":"tag-mental-health","10":"tag-uk","11":"tag-united-kingdom"},"share_on_mastodon":{"url":"https:\/\/pubeurope.com\/@uk\/114587578436214024","error":""},"_links":{"self":[{"href":"https:\/\/www.europesays.com\/uk\/wp-json\/wp\/v2\/posts\/139705","targetHints":{"allow":["GET"]}}],"collection":[{"href":"https:\/\/www.europesays.com\/uk\/wp-json\/wp\/v2\/posts"}],"about":[{"href":"https:\/\/www.europesays.com\/uk\/wp-json\/wp\/v2\/types\/post"}],"author":[{"embeddable":true,"href":"https:\/\/www.europesays.com\/uk\/wp-json\/wp\/v2\/users\/2"}],"replies":[{"embeddable":true,"href":"https:\/\/www.europesays.com\/uk\/wp-json\/wp\/v2\/comments?post=139705"}],"version-history":[{"count":0,"href":"https:\/\/www.europesays.com\/uk\/wp-json\/wp\/v2\/posts\/139705\/revisions"}],"wp:featuredmedia":[{"embeddable":true,"href":"https:\/\/www.europesays.com\/uk\/wp-json\/wp\/v2\/media\/62294"}],"wp:attachment":[{"href":"https:\/\/www.europesays.com\/uk\/wp-json\/wp\/v2\/media?parent=139705"}],"wp:term":[{"taxonomy":"category","embeddable":true,"href":"https:\/\/www.europesays.com\/uk\/wp-json\/wp\/v2\/categories?post=139705"},{"taxonomy":"post_tag","embeddable":true,"href":"https:\/\/www.europesays.com\/uk\/wp-json\/wp\/v2\/tags?post=139705"}],"curies":[{"name":"wp","href":"https:\/\/api.w.org\/{rel}","templated":true}]}}
