{"id":172200,"date":"2025-06-10T05:58:11","date_gmt":"2025-06-10T05:58:11","guid":{"rendered":"https:\/\/www.europesays.com\/uk\/172200\/"},"modified":"2025-06-10T05:58:11","modified_gmt":"2025-06-10T05:58:11","slug":"adhd-drug-mix-up-led-to-my-son-6-receiving-lethal-dose-of-highly-addictive-meds-my-warning-to-parents","status":"publish","type":"post","link":"https:\/\/www.europesays.com\/uk\/172200\/","title":{"rendered":"ADHD drug mix-up led to my son, 6, receiving lethal dose of highly addictive meds&#8230; my warning to parents"},"content":{"rendered":"<p class=\"author-section byline-plain\">By <a href=\"https:\/\/www.dailymail.co.uk\/profile-467\/sadie-whitelocks.html\" class=\"author\" target=\"_blank\" rel=\"noopener\">SADIE WHITELOCKS FOR DAILYMAIL.COM<\/a> <\/p>\n<p class=\"byline-section\"> Published:  23:34 EDT, 9 June 2025   |  Updated:  23:34 EDT, 9 June 2025   <\/p>\n<p class=\"mol-para-with-font\">A mix up at a pharmacy proved nearly fatal after a six-year-old&#8217;s <a style=\"font-weight: bold;\" target=\"_self\" href=\"https:\/\/www.dailymail.co.uk\/news\/adhd\/index.html\" id=\"mol-9a7520c0-4546-11f0-b22d-7382ae33e8e3\" rel=\"noopener\">ADHD<\/a> medication was accidentally swapped with a lethal dose of opioids.\u00a0<\/p>\n<p class=\"mol-para-with-font\">Sarah Paquin, a mother-of-three from British Columbia in <a style=\"font-weight: bold;\" target=\"_self\" href=\"https:\/\/www.dailymail.co.uk\/news\/canada\/index.html\" id=\"mol-9a78f150-4546-11f0-b22d-7382ae33e8e3\" rel=\"noopener\">Canada<\/a>, said her son Declan had been taking dextroamphetamine<a style=\"font-weight: bold;\" target=\"_self\" class=\"\" href=\"https:\/\/www.dailymail.co.uk\/health\/article-14755743\/ADHD-autism-diagnosis.html\" rel=\"noopener\">\u00a0to treat his attention-deficit hyperactivity disorder for years.<\/a><\/p>\n<p class=\"mol-para-with-font\">Paquin picked up his prescription as she always did from a local\u00a0Shoppers Drug Mart pharmacy, and didn&#8217;t notice anything unusual.<\/p>\n<p class=\"mol-para-with-font\">However, when her husband David went to give Declan his daily medication the next morning, he noticed the pills were a different color.\u00a0<\/p>\n<p class=\"mol-para-with-font\">When he checked the label on the bottle, he realized it contained a\u00a0high dose of the opioid hydromorphone and the name on the prescription was of a woman the family didn&#8217;t know.\u00a0<\/p>\n<p class=\"mol-para-with-font\">Hydromorphone is around four times more potent than morphine. The medication has the possibility of causing life-threatening breathing problems.<\/p>\n<p class=\"mol-para-with-font\">This risk is particularly high for children due to their smaller size and weight. &#8216;It&#8217;s just terrifying. I think this was entirely avoidable.&#8217;<\/p>\n<p class=\"mol-para-with-font\">Mr Paquin immediately returned the medication to the pharmacy, where the pharmacist gave him a refund but she was unable to\u00a0find the correct prescription.\u00a0<\/p>\n<p class=\"mol-para-with-font\">The order had to be refilled.<\/p>\n<p class=\"mol-para-with-font\">Commenting on the matter, a spokesperson for\u00a0Shoppers Drug Mart&#8217;s parent company, Loblaw PR, told DailyMail.com: &#8216;Upon review with the store, we have learned this was a case of human error, one that never should have happened.\u00a0<\/p>\n<p>Your browser does not support iframes.<\/p>\n<p class=\"mol-para-with-font\">&#8216;We have controls in place to minimize risks like this &#8211; where the patient was handed the wrong prescription bag &#8211; and the associate will review these with employees to avoid a similar situation in the future.\u00a0<\/p>\n<p class=\"mol-para-with-font\">&#8216;The owner of this location has reached out to the patient&#8217;s parents to apologize for any undue stress this may have caused, and to outline the corrective steps.&#8217;<\/p>\n<p class=\"mol-para-with-font\">The Paquin family say they have also been informed the pharmacist responsible for the error has been suspended.\u00a0<\/p>\n<p class=\"mol-para-with-font\">They now want to make their\u00a0story public in a bid to warn other parents about the importance of being vigilant.\u00a0<\/p>\n<p class=\"mol-para-with-font\">Mrs Paquin says: &#8216;[Patients and parents should] double, triple, quadruple-check every prescription you pick up, whether you&#8217;ve been going to that pharmacy for years, whether it&#8217;s a medication you&#8217;ve been on for years.&#8217;\u00a0<\/p>\n<p class=\"mol-para-with-font\">The Paquins have also filed a complaint with the College of Pharmacists of British Columbia about the incident.<\/p>\n<p class=\"mol-para-with-font\">The organization regulates all pharmacies in the province.<\/p>\n<p class=\"mol-para-with-font\">The FDA receives over 100,000 reports related to medication errors annually, with these contributing to up to\u00a09,000 deaths.<\/p>\n<p class=\"mol-para-with-font\">In 2022, a former Tennessee nurse was found guilty of criminally negligent homicide in the death of a patient who was accidentally given the wrong medication.<\/p>\n<p class=\"mol-para-with-font\">RaDonda Vaught, 37, was also convicted of gross neglect of an impaired adult in a case that fixed the attention of patient safety advocates and nurses&#8217;
