I\u2019m Dr F. Perry Wilson from the Yale School of Medicine.<\/p>\n<p>The issue I\u2019m discussing this week is bound to be sensitive, so let me try to focus just on the facts. Yes, there are <a href=\"https:\/\/jamanetwork.com\/journals\/jamanetworkopen\/fullarticle\/2825472\" target=\"_blank\" rel=\"noopener\">more diagnosed cases of autism now than there have been in the past<\/a>.<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"ck-editor__editable ck-editor__nested-editable\" src=\"https:\/\/www.europesays.com\/uk\/wp-content\/uploads\/2025\/06\/ou_250421_fig1_twin_study_689x388.jpg\" alt=\"photo of Autism Diagnosis Among US Children and Adults\" height=\"388\" width=\"689\" data-asset-description=\"Autism Diagnosis Among US Children and Adults\" data-asset-id=\"3eee3743-c4f2-4738-918d-25e40c29269b\" data-asset-title=\"ou_250421_fig1_twin_study_689x388.jpg\" data-creditline=\"JAMA Network\" data-source=\"JAMA Network\" data-keywords=\"\" data-path=\"\/vim\/live\/professional_assets\/medscape\/images\/thumbnail_library\/ou_250421_fig1_twin_study_689x388.jpg\" data-asset-url=\"https:\/\/www.europesays.com\/uk\/wp-content\/uploads\/2025\/06\/ou_250421_fig1_twin_study_689x388.jpg\" data-height=\"388\" data-width=\"689\" role=\"textbox\" tabindex=\"-1\" contenteditable=\"true\"\/><\/p>\n<p>The US Secretary of Health and Human Services (HHS), <a href=\"https:\/\/www.science.org\/content\/article\/claiming-autism-epidemic-rfk-jr-describes-nih-initiative-find-environmental-causes\" target=\"_blank\" rel=\"noopener\">Robert F. Kennedy, Jr, cites that fact and suggests that it implies the existence of an environmental cause of autism<\/a>\u2014 as of yet unidentified \u2014 that, through careful research, can be identified, perhaps as early as September. The focus on an environmental explanation for the increase runs counter to a <a href=\"https:\/\/www.cdc.gov\/mmwr\/volumes\/74\/ss\/ss7402a1.htm\" target=\"_blank\" rel=\"noopener\">recent Centers for Disease Control and Prevention (CDC) report<\/a> suggesting that the increase is due to better and earlier identification. RFK Jr referred to that line of evidence as a \u201ccanard\u201d \u2014 a useful excuse to protect entrenched interests that are actually the culprits. And although I think we know what <a href=\"https:\/\/www.npr.org\/sections\/shots-health-news\/2025\/04\/16\/nx-s1-5366676\/autism-cdc-rates-rfk-research\" target=\"_blank\" rel=\"noopener\">entrenched interests he may be referring to<\/a>, I\u2019ll give him credit that his public statements keep an open mind about what environmental factors might be at play.<\/p>\n<p>I thought, therefore, that it might be a good time to look at what we already know about environmental versus genetic triggers for autism to set expectations on what an HHS-sponsored investigation may be able to find. In other words, let\u2019s look at the data. Autism: genes vs environment.<\/p>\n<p>There\u2019s a pretty interesting way to quantify how much of a disease may be due to things external to our genetic makeup vs inherent to that makeup: twin studies. And the cool thing is, these studies can tell us how much of a disease may be due to an environmental trigger even when we don\u2019t know what that trigger might be.<\/p>\n<p>To show you how this works, I\u2019ll give two extreme examples.<\/p>\n<p>Let\u2019s look at a condition that is basically purely genetic \u2014 <a href=\"https:\/\/emedicine.medscape.com\/article\/1150165-overview\" class=\"cl_ref_article\" target=\"_blank\" rel=\"noopener\">Huntington disease<\/a> \u2014 and examine how it should work among different types of twins.<\/p>\n<p>Identical (monozygotic) twins share virtually 100% of their genetic code. Since Huntington disease is purely genetic, you\u2019d expect perfect concordance among identical twins, regardless of whether they were raised in the same environment or not. There are actually <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC1049168\/\" target=\"_blank\" rel=\"noopener\">a few case reports<\/a> of identical twins raised apart \u2014 a rare occurrence \u2014 where both developed Huntington disease, as expected.\u00a0<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"ck-editor__editable ck-editor__nested-editable\" src=\"https:\/\/www.europesays.com\/uk\/wp-content\/uploads\/2025\/06\/ou_250421_fig3_twin_study_689x388.jpg\" alt=\"photo of Twin study of Huntington Disease\" height=\"388\" width=\"689\" data-asset-description=\"Twin study of Huntington Disease\" data-asset-id=\"9ed4c41b-a7b8-46ee-8ea7-152dda2557a6\" data-asset-title=\"ou_250421_fig3_twin_study_689x388.jpg\" data-creditline=\"F. Perry Wilson\" data-keywords=\"\" data-path=\"\/vim\/live\/professional_assets\/medscape\/images\/thumbnail_library\/ou_250421_fig3_twin_study_689x388.jpg\" data-asset-url=\"https:\/\/www.europesays.com\/uk\/wp-content\/uploads\/2025\/06\/ou_250421_fig3_twin_study_689x388.jpg\" data-height=\"388\" data-width=\"689\" role=\"textbox\" tabindex=\"-1\" contenteditable=\"true\"\/><\/p>\n<p>In contrast, fraternal (dizygotic) twins, share 50% of the same genetic information (just like nontwin siblings). As such, we\u2019d expect some concordance between fraternal twins in terms of Huntington disease (since it\u2019s a dominant gene, 50% of children of an affected parent are affected), but not perfect concordance. And, similarly, it shouldn\u2019t matter whether these twins were raised in the same environment or not. I\u2019m including the in utero environment here \u2014 basically, \u201cenvironment\u201d means anything not genetic.<\/p>\n<p>Almost no disease is purely environmental, because genetic variation might change the susceptibility to environmental toxins, but as an example of the other side, let\u2019s take something like <a href=\"https:\/\/emedicine.medscape.com\/article\/1053253-overview\" class=\"cl_ref_article\" target=\"_blank\" rel=\"noopener\">scarlet fever<\/a>. <a href=\"https:\/\/www.cambridge.org\/core\/journals\/amg-acta-geneticae-medicae-et-gemellologiae-twin-research\/article\/heredity-and-infectious-diseases-a-twin-study\/4ECCE34E0BB8A144EEDB55CBC874E087\" target=\"_blank\" rel=\"noopener\">This twin study<\/a> examined concordance rates for scarlet fever between identical and fraternal twins and found them to be basically the same. In other words, susceptibility to scarlet fever is driven almost entirely by the environment kids are raised in, not by their genetic similarity.\u00a0<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"ck-editor__editable ck-editor__nested-editable\" src=\"https:\/\/www.europesays.com\/uk\/wp-content\/uploads\/2025\/06\/ou_250421_fig4_twin_study_689x388.jpg\" alt=\"photo of Twin study of Huntington Disease\" height=\"388\" width=\"689\" data-asset-description=\"Twin study of Huntington Disease\" data-asset-id=\"5afeb013-f63a-4968-8e2f-d285ac3f1a64\" data-asset-title=\"ou_250421_fig4_twin_study_689x388.jpg\" data-creditline=\"F. Perry Wilson\" data-keywords=\"\" data-path=\"\/vim\/live\/professional_assets\/medscape\/images\/thumbnail_library\/ou_250421_fig4_twin_study_689x388.jpg\" data-asset-url=\"https:\/\/www.europesays.com\/uk\/wp-content\/uploads\/2025\/06\/ou_250421_fig4_twin_study_689x388.jpg\" data-height=\"388\" data-width=\"689\" role=\"textbox\" tabindex=\"-1\" contenteditable=\"true\"\/><\/p>\n<p>Then consider something like <a href=\"https:\/\/emedicine.medscape.com\/article\/117853-overview\" class=\"cl_ref_article\" target=\"_blank\" rel=\"noopener\">type 2 diabetes<\/a>, which has both strong genetic and environmental components. Twin studies show a higher degree of concordance among identical twins than fraternal twins, but data from twins raised apart suggest a substantial environmental component. Studies vary, but <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/31124778\/\" target=\"_blank\" rel=\"noopener\">most end up attributing about 60% of the risk to genetic and 40% to environmental exposures<\/a>. And, to be clear, when I write \u201cenvironment\u201d here, I don\u2019t mean toxins in the air or something \u2014 I mean the environment we have created, which is replete with calorie-dense, ultra-processed foods.<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"ck-editor__editable ck-editor__nested-editable\" src=\"https:\/\/www.europesays.com\/uk\/wp-content\/uploads\/2025\/06\/ou_250421_fig5_twin_study_689x388.jpg\" alt=\"photo of Twin study of Huntington Disease\" height=\"388\" width=\"689\" data-asset-description=\"Twin study of Huntington Disease\" data-asset-id=\"7bd30165-cb3d-449b-8a6b-a46add0b57b7\" data-asset-title=\"ou_250421_fig5_twin_study_689x388.jpg\" data-creditline=\"F. If it is caused by some environmental exposure \u2014 a vaccine or vaccine ingredient, a pollutant, energy beams from space, whatever \u2014 we should see similar concordance between identical and fraternal twins, provided they were raised together. What do we actually see? <a href=\"https:\/\/www.seattlechildrens.org\/clinics\/autism-center\/the-autism-blog\/twins-autism\/\" target=\"_blank\" rel=\"noopener\">Concordance rates among identical twins range from 60%-90% and, for fraternal twins, from about 3% to 30%<\/a>. That\u2019s a strong genetic signal.\u00a0<\/p>\n<p>There are quite a few studies out there that dig in even deeper, but this one in <a href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2654804\" target=\"_blank\" rel=\"noopener\">JAMA is pretty solid<\/a> since it comes from Sweden where, as I have mentioned multiple times before, healthcare is universal and so are electronic medical records. For what it\u2019s worth, the estimates from this study are pretty similar to those from other studies.\u00a0<\/p>\n<p>Researchers examined about 40,000 twin pairs, as well as 2.5 million sibling pairs and about a million half-sibling pairs. The data suggested that 83% of autism diagnoses could be attributed to genetics, based on a high degree of concordance between identical twins vs fraternal twins. About 4% could be attributed to what are called \u201cshared environmental factors\u201d \u2014 these are things that siblings are both exposed to \u2014 for fraternal twins that includes everything from the womb on up until they leave the nest.\u00a0<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"ck-editor__editable ck-editor__nested-editable\" src=\"https:\/\/www.europesays.com\/uk\/wp-content\/uploads\/2025\/06\/ou_250421_fig6_twin_study_689x388.jpg\" alt=\"photo of Twin study of Huntington Disease\" height=\"388\" width=\"689\" data-asset-description=\"Twin study of Huntington Disease\" data-asset-id=\"707aa0da-b053-4055-b02a-4f783a03d9e5\" data-asset-title=\"ou_250421_fig6_twin_study_689x388.jpg\" data-creditline=\"F. Perry Wilson\" data-keywords=\"\" data-path=\"\/vim\/live\/professional_assets\/medscape\/images\/thumbnail_library\/ou_250421_fig6_twin_study_689x388.jpg\" data-asset-url=\"https:\/\/www.europesays.com\/uk\/wp-content\/uploads\/2025\/06\/ou_250421_fig6_twin_study_689x388.jpg\" data-height=\"388\" data-width=\"689\" role=\"textbox\" tabindex=\"-1\" contenteditable=\"true\"\/><\/p>\n<p>In that study, 16% of the diagnoses could be explained by non-shared environmental factors. This is interesting; it reflects a concordance in relatives after accounting for shared genetics even if they weren\u2019t raised in the same environment. This could reflect some interesting epigenetic causes of autism or exposures (like certain illnesses) that aren\u2019t highly shared between siblings.<\/p>\n<p>I think there is something for everyone in these data. It\u2019s fairly clear that the majority of cases of autism can be attributed to genetic factors, even if we don\u2019t yet know entirely what those factors are. That does support the hypothesis that the observed increase in cases is due to more or better diagnosis \u2014 we just missed these kids in the past, or <a href=\"https:\/\/www.science.org\/content\/article\/cold-parenting-childhood-schizophrenia-how-diagnosis-autism-has-evolved-over-time\" target=\"_blank\" rel=\"noopener\">diagnosed them with something else<\/a>.<\/p>\n<p>But <a href=\"https:\/\/emedicine.medscape.com\/article\/912781-overview\" class=\"cl_ref_article\" target=\"_blank\" rel=\"noopener\">autism spectrum disorder<\/a> is not Huntington disease. There clearly are environmental factors at play as well; not every pair of identical twins is concordant for autism, even if 90% are. And, even if there is concordance, the <a href=\"https:\/\/www.nih.gov\/news-events\/news-releases\/severity-autism-symptoms-varies-greatly-among-identical-twins\" target=\"_blank\" rel=\"noopener\">severity may differ<\/a> between identical twins, indicating some gene-environment interactions.\u00a0<\/p>\n<p>So, is something in the environment the major factor leading to the observed increase in cases of autism spectrum disorder? Probably not, but it should not be ignored just because it isn\u2019t the sole explanation. We should try to figure out what these other factors are.\u00a0<\/p>\n<p>Which means that now, the real work begins, because \u201cenvironmental factors\u201d is a big tent that includes essentially everything that isn\u2019t genetic: mom and dad\u2019s medical history, toxins and pollutants, infections, medications, and so on. Twin studies are not the way to figure out which of these is the culprit \u2014 more dedicated, cause-specific research is. I for one am happy to see some funding go in this area, but I will be quite surprised, and honestly skeptical, if we get an answer by September.\u00a0<\/p>\n<p>F.
