Finalised in 2022, it highlighted serious cultural issues as well as potential mismanagement of waiting lists. <\/p>\n<p class=\"c-paragraph paywall\">The organisation has said it cannot publish the report in full due to legal advice.<\/p>\n<p class=\"c-paragraph paywall\">The review, which has been leaked extensively in the media, is the latest in a series of controversies facing the body, following a review into the <a href=\"https:\/\/www.irishtimes.com\/health\/2025\/04\/08\/spinal-surgeries-report-the-story-behind-the-springs-used-on-children\/\" target=\"_self\" rel=\"noopener\" title=\"https:\/\/www.irishtimes.com\/health\/2025\/04\/08\/spinal-surgeries-report-the-story-behind-the-springs-used-on-children\/\">use of unauthorised springs in three children with scoliosis<\/a>, as well as an <a href=\"https:\/\/www.irishtimes.com\/health\/2025\/05\/23\/audit-finds-almost-70-per-cent-of-hip-operations-in-two-childrens-hospitals-unnecessary\/\" target=\"_self\" rel=\"noopener\" title=\"https:\/\/www.irishtimes.com\/health\/2025\/05\/23\/audit-finds-almost-70-per-cent-of-hip-operations-in-two-childrens-hospitals-unnecessary\/\">audit on the threshold for hip surgery<\/a>.<\/p>\n<p class=\"c-paragraph paywall\">According to the summary, a total of 45 individuals at the unnamed children\u2019s hospital were interviewed, with most describing a culture within their department where change was slow, lacked governance and robust processes, and was influenced by strong and challenging personalities. <\/p>\n<p class=\"c-paragraph paywall\">\u201cThese interpersonal difficulties among team members led to poor working relationships and a challenging working environment,\u201d the summary said, adding that there was a high attrition rate among support staff due to \u201cbullying issues\u201d.<\/p>\n<p class=\"c-paragraph paywall\">A \u201csignificant risk\u201d was identified where only one employee managed the needs of a complex tertiary speciality. <\/p>\n<p class=\"c-paragraph paywall\">\u201cThis level of dependency on one individual for a critical service is not in line with best practice and created a vulnerability in this speciality,\u201d the summary said.<\/p>\n<p class=\"c-paragraph paywall\">Half of medical trainees described the learning environment at the hospital as not conducive to learning. Consequently, the training body indicated there would be no intake of any new trainees or Specialist Registrars (SpRs) into the programme in 2022 due to concerns about the trainee experience which was \u201creputationally damaging\u201d for CHI.<\/p>\n<p class=\"c-paragraph paywall\">The report also stated that a negative culture can impact service delivery, department dynamics and staff experience and has the potential to put patients at risk, it added.<\/p>\n<p class=\"c-paragraph paywall\">On the topic of leadership and governance, the report said there was a \u201csubstantial and persistent message of concern\u201d regarding the lack of strong site leadership and good governance, along with poor operational oversight and accountability.<\/p>\n<p class=\"c-paragraph paywall\">\u201cThere was consistent feedback that the challenging behaviours of some staff were not adequately addressed by the site leadership team. This led to a number of unresolved personal issues between employees,\u201d it added.<\/p>\n<p class=\"c-paragraph paywall\">The report also said the management of a particular cohort of patients was inequitable across CHI Hospitals. <\/p>\n<p class=\"c-paragraph paywall\">According to the summary, issues were also identified with regard to <a href=\"https:\/\/www.irishtimes.com\/tags\/national-treatment-purchase-fund-board\/\" target=\"_self\" rel=\"noopener\" title=\"https:\/\/www.irishtimes.com\/tags\/national-treatment-purchase-fund-board\/\">National Treatment Purchase Fund (NTPF)<\/a> funded waiting list initiatives, in which some patients could have been seen by other departmental colleagues within the existing day to day service and \u201cpotentially managed in a more proactive way\u201d.<\/p>\n<p class=\"c-paragraph paywall\">In the summary, CHI outlined the actions it has taken in response to the review, including hiring an external facilitator to improve culture and teamwork in the department and implementing a \u201crobust process\u201d for NTPF funded waiting list initiatives, requiring multiple approvals.<\/p>\n<p class=\"c-paragraph b-it-article-body__interstitial-link\">[\u00a0<a aria-label=\"Open related story\" class=\"c-link\" href=\"https:\/\/www.irishtimes.com\/health\/2025\/05\/15\/mother-devastated-to-be-told-her-young-sons-hip-operation-was-not-necessary-after-all\/\" rel=\"noreferrer noopener\" target=\"_blank\">Mother \u2018devastated\u2019 to be told her young son\u2019s hip operation was not necessary after allOpens in new window<\/a>\u00a0]<\/p>\n<p class=\"c-paragraph paywall\">In a statement, CHI said it wants \u201cto be open and transparent\u201d with families.<\/p>\n<p class=\"c-paragraph paywall\">\u201cCHI takes accountability to the public and the families it serves extremely seriously.
