In clinical trials, the drug nearly eliminated HIV\u2019s spread among people given an injection every six months.<\/p>\n<p id=\"anchor-5e2d69\" class=\"body-graf\">Called Yeztugo, the <a href=\"https:\/\/www.nbcnews.com\/feature\/nbc-out\/injectable-hiv-prevention-drug-lencapavir-rcna170778\" target=\"_blank\" rel=\"noopener\">highly effective drug<\/a> has inspired feverish anticipation among advocates for HIV prevention. The hope is the medication could accelerate the <a href=\"https:\/\/www.nbcnews.com\/nbc-out\/out-health-and-wellness\/us-progress-hiv-fight-continues-trail-many-rich-nations-rcna85683\" target=\"_blank\" rel=\"noopener\">stubbornly slow decline of HIV transmission<\/a> in the United States.<\/p>\n<p id=\"anchor-69ee6d\" class=\"body-graf\">\u201cThis is the single best opportunity in 44 years of HIV prevention,\u201d said <a href=\"https:\/\/avac.org\/mitchell-warren\/\" target=\"_blank\" rel=\"noopener\">Mitchell Warren<\/a>, executive director of the HIV advocacy nonprofit group AVAC.<\/p>\n<p id=\"anchor-90a9ef\" class=\"body-graf\">Yeztugo (generic name lenacapavir), which is given by health care workers in clinics, is significantly more effective than the existing oral HIV-prevention drugs, experts said, because it appears to address the challenges of sticking to a daily pill regimen for people who are at high risk of HIV.<\/p>\n<p id=\"anchor-8c399e\" class=\"body-graf\">Gilead\u2019s chairman and CEO, Daniel O\u2019Day, suggested in a statement Wednesday that the drug could \u201cend the HIV epidemic once and for all.\u201d<\/p>\n<p id=\"anchor-289830\" class=\"body-graf\">It was approved <a href=\"https:\/\/www.nbcnews.com\/nbc-out\/out-health-and-wellness\/injectable-hiv-prevention-drug-lencapavir-rcna170778\" target=\"_blank\" rel=\"noopener\">after astounding results<\/a> in Gilead\u2019s large clinical trials of the drug. The trials randomly assigned people at risk of HIV to receive either lenacapavir injections every six months or daily Truvada, a pill form of pre-exposure prophylaxis, or PrEP. Among gay and bisexual men and transgender people, the Yeztugo group had an 89% lower HIV rate than a group taking Truvada and a 96% lower rate than Gilead estimated would have been expected absent any PrEP.<\/p>\n<p id=\"anchor-6c2930\" class=\"body-graf\">In a <a href=\"https:\/\/www.nejm.org\/doi\/10.1056\/NEJMoa2407001\" target=\"_blank\" rel=\"noopener\">similar trial<\/a> among cisgender women in sub-Saharan Africa, no one who received Yeztugo contracted HIV.<\/p>\n<p id=\"anchor-9ef8ac\" class=\"body-graf\">The shot is the first in a <a href=\"https:\/\/clinicalinfo.hiv.gov\/en\/glossary\/capsid-inhibitors#:~:text=Capsid%20inhibitors%20are%20a%20class,and%20enzymes%20needed%20for%20replication\" target=\"_blank\" rel=\"noopener\">new class<\/a> of antiretrovirals which block HIV from infecting and making new copies of itself inside the immune cells it targets. Lenacapavir was <a href=\"https:\/\/www.gilead.com\/news\/news-details\/2022\/sunlenca-lenacapavir-receives-fda-approval-as-a-first-in-class-twice-yearly-treatment-option-for-people-living-with-multi-drug-resistant-hiv\" target=\"_blank\" rel=\"noopener\">first approved in 2022<\/a>, under the brand name Sunleca, for use with other medications to treat highly drug resistant strains of the virus.<\/p>\n<p id=\"anchor-481e94\" class=\"body-graf\">All forms of PrEP work in the same way: If enough of the drug is present in the body when a person is exposed to HIV,  it&#8217;s highly effective at preventing the virus from taking hold and establishing a lifelong infection. Lenacapavir is so long-acting, it only needs to be injected twice a year.<\/p>\n<p id=\"anchor-61a454\" class=\"body-graf\">However, Yeztugo\u2019s use as PrEP against HIV emerges into a political climate that experts warn could <a href=\"https:\/\/www.kff.org\/policy-watch\/what-do-federal-staffing-cuts-and-hhs-restructuring-mean-for-the-nations-hiv-response\/\" target=\"_blank\" rel=\"noopener\">derail progress against the virus\u2019 spread<\/a>. The Trump administration\u2019s recent sweeping cancellations of research grants and its severe cuts to the Centers for Disease Control and Prevention\u2019s staffing <a href=\"https:\/\/www.nbcnews.com\/nbc-out\/out-news\/trump-administration-axes-125m-lgbtq-health-funding-upending-research-rcna199175\" target=\"_blank\" rel=\"noopener\">have dramatically affected the HIV-prevention field<\/a> in particular. HIV experts are concerned that Yeztugo\u2019s full potential might remain unrealized.<\/p>\n<p><img decoding=\"async\" loading=\"lazy\" src=\"https:\/\/www.europesays.com\/uk\/wp-content\/uploads\/2025\/06\/250618-yeztugo-ch-1436-6703c6.jpg\" alt=\"Yeztugo injection.\" height=\"1667\" width=\"2500\"\/>The shot is the first in a new class of antiretrovirals which block HIV from infecting and making new copies of itself inside the immune cells it targets.Gilead Sciences<\/p>\n<p id=\"anchor-00d071\" class=\"body-graf\">Priced at $14,109 per injection, or $2,352 per month, Yeztugo&#8217;s cost could be another hurdle. <\/p>\n<p id=\"anchor-954b8d\" class=\"body-graf\">Health insurers, at least in the near term, might decline to cover the shots in favor of other forms of PrEP, including the cheaper generic version of Truvada pills, which cost as little as $30 per month. Or insurers might impose higher copays for lenacapavir, said Elizabeth Kaplan, director of health care access at Harvard Law School\u2019s Health Law and Policy Clinic.<\/p>\n<p id=\"anchor-dac138\" class=\"body-graf\">A pending Supreme Court decision <a href=\"https:\/\/www.nbcnews.com\/nbc-out\/out-news\/obamacare-appeals-court-unconstitutional-texas-hiv-prep-rcna152813\" target=\"_blank\" rel=\"noopener\">concerning a pillar of the Affordable Care Act<\/a> could also be a barrier to insurance coverage for medications that prevent HIV. The ACA, or Obamacare, mandates no-cost coverage for <a href=\"https:\/\/www.lung.org\/getmedia\/7ee72d9c-ee78-4a77-a419-9bfa87e69acd\/Braidwood-Preventive-Services-Chart.pdf\" target=\"_blank\" rel=\"noopener\">certain preventive interventions<\/a> like PrEP. <\/p>\n<p><strong>Mixed success for PrEP drugs<\/strong><\/p>\n<p id=\"anchor-86421b\" class=\"body-graf\">Older forms of PrEP have<a href=\"https:\/\/www.nbcnews.com\/nbc-out\/out-health-and-wellness\/preps-promise-change-course-hiv-succeeded-only-white-gay-men-rcna75438\" target=\"_blank\" rel=\"noopener\"> had mixed success<\/a>.<\/p>\n<p id=\"anchor-67eb22\" class=\"body-graf\">Truvada, also made by Gilead, was the first approved PrEP drug <a href=\"https:\/\/www.gilead.com\/news\/news-details\/2012\/us-food-and-drug-administration-approves-gileads-truvada-for-reducing-the-risk-of-acquiring-hiv\" target=\"_blank\" rel=\"noopener\">in 2012<\/a>, followed by a similar oral medication from the company, Descovy, <a href=\"https:\/\/www.gilead.com\/news\/news-details\/2019\/us-food-and-drug-administration-approves-descovy-for-hiv-pre-exposure-prophylaxis-prep#:~:text=FOSTER%20CITY%2C%20Calif.,tablets%3B%20F%2FTAF).\" target=\"_blank\" rel=\"noopener\">in 2019<\/a>. <\/p>\n<p id=\"anchor-cb05bf\" class=\"body-graf\">They have steadily gained popularity among gay and bisexual men, who, <a href=\"https:\/\/www.cdc.gov\/hiv-data\/nhss\/hiv-diagnoses-deaths-and-prevalence-2025.html\" target=\"_blank\" rel=\"noopener\">according to the CDC<\/a>, account for 7 in 10 new HIV cases and comprise the overwhelming share of PrEP users.<\/p>\n<p id=\"anchor-d530fc\" class=\"body-graf\">However, considering <a href=\"https:\/\/www.cdc.gov\/hiv\/data-research\/facts-stats\/gay-bisexual-men.html\" target=\"_blank\" rel=\"noopener\">how much higher<\/a> the HIV rate is among Black and Latino gay men compared with their white peers, PrEP use among men of color <a href=\"https:\/\/www.nbcnews.com\/nbc-out\/out-health-and-wellness\/preps-promise-change-course-hiv-succeeded-only-white-gay-men-rcna75438\" target=\"_blank\" rel=\"noopener\">has remained disproportionately low<\/a>. Experts believe PrEP has most likely hastened HIV\u2019s considerable decline among white gay and bisexual men, but the HIV rate among Black and Latino gay and bisexual men has persisted at markedly high rates. In addition, when they are prescribed PrEP, Black gay men <a href=\"https:\/\/link.springer.com\/article\/10.1007\/s10461-019-02641-2\" target=\"_blank\" rel=\"noopener\">are less likely<\/a> <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/35662802\/\" target=\"_blank\" rel=\"noopener\">to take the pill<\/a> <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/35662802\/\" target=\"_blank\" rel=\"noopener\">regimen as prescribed<\/a>.<\/p>\n<p id=\"anchor-4f3234\" class=\"body-graf\">When they are taken daily, <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/22972843\/\" target=\"_blank\" rel=\"noopener\">Truvada<\/a> and <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/32711800\/\" target=\"_blank\" rel=\"noopener\">Descovy<\/a> each slash HIV risk by <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC6107918\/\" target=\"_blank\" rel=\"noopener\">at least 99%<\/a>. But inconsistent use diminishes PrEP\u2019s effectiveness.<\/p>\n<p id=\"anchor-5165d4\" class=\"body-graf\">In late 2021, ViiV Healthcare\u2019s Apretude <a href=\"https:\/\/www.nbcnews.com\/nbc-out\/out-health-and-wellness\/fda-approves-first-injectable-hiv-prevention-drug-rcna9426\" target=\"_blank\" rel=\"noopener\">was approved<\/a> as an injectable PrEP given every two months. <\/p>\n<p id=\"anchor-e21e27\" class=\"body-graf\">Despite <a href=\"https:\/\/www.nbcnews.com\/nbc-out\/out-health-and-wellness\/fda-approves-first-injectable-hiv-prevention-drug-rcna9426\" target=\"_blank\" rel=\"noopener\">being much more effective<\/a> than Truvada at lowering HIV cases across a population of gay and bisexual men and transgender women in a major clinical trial, Apretude has <a href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2825028\" target=\"_blank\" rel=\"noopener\">gained scant traction<\/a> until recently. About 21,000 people are taking it, ViiV reported. It\u2019s possible that, in part, people have been alienated by the requirement to visit a clinic every two months for Apretude, instead of every three months for an oral PrEP prescription.<\/p>\n<p><strong>Yeztugo has the potential, but will it deliver?<\/strong><\/p>\n<p id=\"anchor-886d55\" class=\"body-graf\">The twice-yearly injection offers an alternative for people who won\u2019t or have trouble remembering to take a pill every day, reducing the number of required annual clinic visits from four to two.<\/p>\n<p id=\"anchor-261125\" class=\"body-graf\"><a href=\"https:\/\/www.amfar.org\/news\/cuts-to-the-cdcs-division-of-hiv\/\" target=\"_blank\" rel=\"noopener\">According to CDC<\/a> <a href=\"https:\/\/stacks.cdc.gov\/view\/cdc\/156513\" target=\"_blank\" rel=\"noopener\">estimates<\/a>, during the decade after PrEP\u2019s debut, the annual HIV transmission rate declined by only 17%, from 38,300 cases in 2012 to 31,800 cases in 2022. Much of the decline occurred since 2018, with case numbers <a href=\"https:\/\/www.hiv.gov\/hiv-basics\/overview\/data-and-trends\/statistics#:~:text=CDC%20estimates%20show%20new%20HIV,likely%20contributed%20to%20the%20decline.\" target=\"_blank\" rel=\"noopener\">falling by 12%<\/a> during that four-year period. <\/p>\n<p id=\"anchor-055cb1\" class=\"body-graf\">In 2019, the Trump administration launched the <a href=\"https:\/\/www.hiv.gov\/federal-response\/ending-the-hiv-epidemic\/overview\" target=\"_blank\" rel=\"noopener\">Ending the HIV Epidemic<\/a>, or EHE, initiative, which <a href=\"https:\/\/www.kff.org\/hivaids\/issue-brief\/ending-the-hiv-epidemic-ehe-funding-tracker\/\" target=\"_blank\" rel=\"noopener\">has since<\/a> <a href=\"https:\/\/www.hiv.gov\/federal-response\/ending-the-hiv-epidemic\/funding\/fiscal-year\" target=\"_blank\" rel=\"noopener\">targeted nearly $3 billion<\/a> in new spending to combat HIV, including by promoting PrEP, in 48 hot-spot counties in particular. The target jurisdictions had a swifter <a href=\"https:\/\/www.cdc.gov\/nchhstp\/director-letters\/cdc-publishes-new-hiv-surveillance-reports.html\" target=\"_blank\" rel=\"noopener\">21% decline<\/a> in estimated HIV transmissions from 2018 to 2022.<\/p>\n<p id=\"anchor-be2309\" class=\"body-graf\">In a study <a href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2825028\" target=\"_blank\" rel=\"noopener\">published in October<\/a>, CDC investigators reported that PrEP use has risen steadily since 2014 and that about 200,000 people were receiving some form of it during any month of 2023. <\/p>\n<p id=\"anchor-adf37f\" class=\"body-graf\">But that is only a small fraction of the 1.5 million gay and bisexual men who are good PrEP candidates, according to estimates in a new CDC study <a href=\"https:\/\/www.sciencedirect.com\/science\/article\/abs\/pii\/S1047279725000900\" target=\"_blank\" rel=\"noopener\">published in May<\/a>.<\/p>\n<p><img decoding=\"async\" loading=\"lazy\" src=\"https:\/\/www.europesays.com\/uk\/wp-content\/uploads\/2025\/06\/250618-yeztugo-ch-1437-ee292d.jpg\" alt=\"Yeztugo injection.\" height=\"1667\" width=\"2500\"\/>Yeztugo injection.Gilead Sciences<\/p>\n<p id=\"anchor-aba295\" class=\"body-graf\">The crucial question is whether a critical mass of people who otherwise wouldn\u2019t have stuck to an oral PrEP regimen can routinely be provided lenacapavir while they are at risk of HIV.<\/p>\n<p id=\"anchor-d9322a\" class=\"body-graf\">Dr. Susanne Doblecki-Lewis, chief of the division of infectious diseases at the University of Miami Miller School of Medicine, is the site principal investigator for the lenacapavir clinical trial in gay men and a separate trial in people who inject drugs. She said lenacapavir could mitigate HIV-related racial disparities, provided it is made \u201cavailable easily and at low or no cost to people who can benefit from it who are uninsured or underinsured.\u201d <\/p>\n<p id=\"anchor-b18353\" class=\"body-graf\">But, she said, \u201cif there are barriers, like complicated prior authorizations or high copays that will prevent people from easily starting it, we could see disparities just get worse.\u201d<\/p>\n<p><strong>Political challenges <\/strong><\/p>\n<p id=\"anchor-151c00\" class=\"body-graf\">The Trump administration\u2019s <a href=\"https:\/\/www.whitehouse.gov\/omb\/information-resources\/budget\/the-presidents-fy-2026-discretionary-budget-request\/\" target=\"_blank\" rel=\"noopener\">2026 budget request<\/a> seeks a <a href=\"https:\/\/www.kff.org\/hivaids\/issue-brief\/domestic-hiv-funding-in-the-white-house-fy2026-budget-request\/\" target=\"_blank\" rel=\"noopener\">$1.5 billion, or 35%, cut<\/a> for domestic HIV funding. That includes shuttering the CDC\u2019s $794 million <a href=\"https:\/\/www.kff.org\/quick-take\/cutting-hiv-prevention-funding-at-cdc-what-would-it-mean\/\" target=\"_blank\" rel=\"noopener\">HIV-prevention division<\/a>, although some remnants of it are apparently meant to be folded into a new federal health department, along with a maintained $220 million in EHE funding. <\/p>\n<p id=\"anchor-1cb4f9\" class=\"body-graf\">Much of the CDC\u2019s HIV-prevention and -surveillance budget is <a href=\"https:\/\/www.nbcnews.com\/feature\/nbc-out\/tennessee-axed-millions-hiv-funds-scrutiny-far-right-provocateurs-rcna67769\" target=\"_blank\" rel=\"noopener\">distributed in grants<\/a> to state and local health departments and nonprofit organizations. Severe federal budget cuts would most likely hobble those entities\u2019 efforts to educate at-risk populations and doctors about lenacapavir and provide services to facilitate its use. <\/p>\n<p id=\"anchor-da9205\" class=\"body-graf\">Another hurdle for prescribers is whether they can ensure that people at risk of HIV return for lenacapavir shots twice yearly. Two recent studies, one <a href=\"https:\/\/link.springer.com\/article\/10.1007\/s11606-025-09574-8#xd_co_f=MWI4NjMzNzAtYmExOS00ZTI5LWJhNzAtMjEwMmM5NzdmNmRl~\" target=\"_blank\" rel=\"noopener\">published in May<\/a> and another <a href=\"https:\/\/www.croiconference.org\/abstract\/37-2025\/\" target=\"_blank\" rel=\"noopener\">presented at a conference<\/a> in March, found that fewer than half of oral PrEP users stay on it for six months or longer. <\/p>\n<p id=\"anchor-04108f\" class=\"body-graf\">Historically, the National Institutes of Health has funded research to help implement new forms of PrEP, examining weak spots and devising interventions. But the Trump administration\u2019s campaign to slash NIH spending <a href=\"https:\/\/www.nbcnews.com\/nbc-out\/out-news\/trump-administration-axes-125m-lgbtq-health-funding-upending-research-rcna199175\" target=\"_blank\" rel=\"noopener\">has had a severe impact on HIV research<\/a>, including PrEP, according to a dossier of canceled grants <a href=\"https:\/\/hsph.harvard.edu\/news\/documenting-grant-cancellations-that-represent-a-lifes-worth-of-work\/\" target=\"_blank\" rel=\"noopener\">compiled by<\/a> a <a href=\"https:\/\/hsph.harvard.edu\/profile\/scott-delaney\/\" target=\"_blank\" rel=\"noopener\">Harvard researcher<\/a>. <\/p>\n<p id=\"anchor-df3c90\" class=\"body-graf\">A <a href=\"https:\/\/www.aclu.org\/press-releases\/federal-district-court-strikes-down-nihs-unlawful-directives-that-led-to-the-elimination-of-critical-research\" target=\"_blank\" rel=\"noopener\">federal judge ruled<\/a> Monday that some such NIH grant terminations were \u201carbitrary and capricious\u201d and \u201cvoid and illegal.\u201d But the temporary win for researchers doesn\u2019t mean the NIH under Trump will necessarily become newly hospitable to grant proposals concerning PrEP implementation.<\/p>\n<p id=\"anchor-53c881\" class=\"body-graf\">Johanna Mercier, Gilead\u2019s chief commercial officer, said in an interview this month that the company has been pushing insurers to cover lenacapavir. She expressed optimism for ultimately widespread coverage of the drug. <\/p>\n<p id=\"anchor-1777c9\" class=\"body-graf\">Gilead will cover up to $7,200 annually in out-of-pocket costs for insured people receiving Yeztugo. And its patient assistance program will provide it free to low-income uninsured people. There is otherwise a patchwork system nationally that can often aid the uninsured in covering the required clinic visits and lab tests. <\/p>\n<p id=\"anchor-53c594\" class=\"body-graf\">Tristan Schukraft, CEO of the popular PrEP-focused telehealth company <a href=\"https:\/\/heymistr.com\/\" target=\"_blank\" rel=\"noopener\">Mistr<\/a>, said it intends to begin offering the drug immediately through its storefronts in seven major urban gay neighborhoods. The company has also formed partnerships with a nationwide network of community-based clinics that can provide the shots, including to people without insurance.<\/p>\n<p id=\"anchor-f25273\" class=\"endmark body-graf\">\u201cWe\u2019re ready,\u201d Schukraft said.<\/p>\n","protected":false},"excerpt":{"rendered":"The Food and Drug Administration has approved a highly effective new HIV-prevention medication, Gilead Sciences reported Wednesday.
