Waiting lists are falling a bit faster: May\u2019s figures, out yesterday, show <a href=\"https:\/\/www.england.nhs.uk\/2025\/07\/nhs-delivers-record-numbers-of-treatments-as-waiting-list-drops-to-26-month-low\/\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\">the lowest waiting list total<\/a> in two years, as well as the highest number of treatments<strong> <\/strong>ever recorded. But yes, lists are still long, at 7.36 million, with 40% of people waiting longer than 18 weeks.<\/p>\n<p class=\"dcr-16w5gq9\">More good news today: patients <a href=\"https:\/\/www.england.nhs.uk\/statistics\/statistical-work-areas\/gp-patient-survey\/\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\">are more satisfied<\/a>, or less dissatisfied, with GPs: 70% satisfied with contacting the GP and 75% reported a good overall experience. The <a href=\"https:\/\/www.gov.uk\/government\/news\/pm-launches-new-era-for-nhs-with-easier-care-in-neighbourhoods\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\">extra 1,900 GPs<\/a> and the increased rollout of phone and NHS app bookings are reducing the <a href=\"https:\/\/www.bbc.co.uk\/news\/articles\/cd7ee895nr0o\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\">8am fastest-finger scramble<\/a>.<\/p>\n<p class=\"dcr-16w5gq9\">The bad news is England\u2019s resident (junior) doctors\u2019 <a href=\"https:\/\/www.theguardian.com\/society\/2025\/jul\/08\/resident-doctors-strikes-labour-nhs\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\">90% vote to strike<\/a>. The British Medical Association\u2019s <a href=\"https:\/\/www.theguardian.com\/society\/2025\/jul\/08\/resident-doctors-in-england-vote-to-strike-over-pay\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\">rush to strike action<\/a>, with only two weeks\u2019 notice, is hard on patients with rapid cancellations and managers preparing for the five-day chaos, with doctors out from 25 to 30 July. If any voted just to strengthen their union\u2019s negotiating position, they find themselves called out fast. Why did 45% fail to vote at all, when turnout last time was 71%? Picket lines may be thinner but the NHS, in slow recovery, will be thrown backwards again: with <a href=\"https:\/\/www.bbc.co.uk\/news\/articles\/cren3l8nj8wo\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\">11 separate walkouts<\/a> between 2022 and 2024, <a href=\"https:\/\/www.theguardian.com\/society\/2024\/jan\/10\/junior-doctors-strike-led-to-130000-patients-in-england-having-care-cancelled\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\">1.3m healthcare appointments<\/a> were lost, <a href=\"https:\/\/www.england.nhs.uk\/long-read\/financial-performance-update-1-feb-24\/\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\">costing \u00a31.5bn<\/a>.<\/p>\n<p class=\"dcr-16w5gq9\">The BMA may find only a grudging solidarity from some unions. Immediately after the election, the health secretary, Wes Streeting, settled with resident doctors, giving a <a href=\"https:\/\/www.theguardian.com\/society\/article\/2024\/jul\/29\/junior-doctors-leaders-agree-pay-deal-over-two-years\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\">22.3% rise over two years<\/a>, and an above-inflation <a href=\"https:\/\/www.bbc.co.uk\/news\/articles\/clyv1vxkdjyo\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\">5.4% this year<\/a>: that\u2019s an <a href=\"https:\/\/www.gov.uk\/government\/publications\/nhs-pay-awards-2025-to-2026-resident-doctors\/resident-doctors-pay-award-2025-to-2026-investing-in-our-medical-workforce\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\">average increase in pay of 28.9%<\/a> over three years, meaning <a href=\"https:\/\/www.theguardian.com\/society\/2025\/jul\/09\/the-bma-wants-a-29-pay-rise-for-resident-doctors-but-how-did-it-calculate-these-figures\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\">starting salaries will be up \u00a39,500<\/a>. Other NHS staff in England \u2013 nurses, midwives and physiotherapists \u2013 <a href=\"https:\/\/www.gov.uk\/government\/publications\/nhs-pay-awards-2025-to-2026-agenda-for-change-staff\/nhs-pay-award-2025-to-2026-a-fair-deal-for-nhs-staff\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\">are offered 3.6%<\/a>. It\u2019s impossible to imagine Streeting conceding the doctors\u2019 <a href=\"https:\/\/www.theguardian.com\/society\/2025\/jul\/09\/the-bma-wants-a-29-pay-rise-for-resident-doctors-but-how-did-it-calculate-these-figures\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\">new 29% claim<\/a> \u2013 which, the BMA says, is needed to reverse cuts to the real-terms value of their pay since 2008. But that sum is disputed, as the BMA uses the retail prices index (RPI) to measure inflation since 2008. The consumer price index (CPI), used by the Office for National Statistics (ONS) since 2013, puts their real loss at 4.7%.<\/p>\n<p class=\"dcr-16w5gq9\">Everyone wants to get back to 2008, before the bankers crash; <a href=\"https:\/\/www.tes.com\/magazine\/news\/general\/teacher-pay-rise-2024-25\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\">that\u2019s true of teachers, too<\/a>, whose pay has fallen behind, like the entire public sector. But this is a poorer, Brexit- stricken country, with growth practically stagnant, battered like the rest by world events. Public and private sector pay has <a href=\"https:\/\/ifs.org.uk\/publications\/recent-trends-public-sector-pay\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\">fallen over the past few years<\/a>, despite a slight uptick recently. People feel it in their shopping. If wages grew at pre-crash rates, public pay would be <a href=\"https:\/\/ifs.org.uk\/sites\/default\/files\/2024-03\/Recent-trends-in-public-sector-pay-IFS-REPORT.pdf\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\">56% higher, with the private sector 40% better off<\/a>. I know it\u2019s an absurd measurement, but the <a href=\"https:\/\/www.icaew.com\/insights\/viewpoints-on-the-news\/2024\/sep-2024\/chart-of-the-week-prime-ministers-salary\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\">prime minister\u2019s salary<\/a> is 57% lower than had it kept pace with inflation since 2009.<\/p>\n<p class=\"dcr-16w5gq9\">In this pay climate, the doctors may lack sympathy. Their last pay rise came out of general NHS funds, paid in part by abolishing NHS England. Dipping into the service again for a 29% claim looks unlikely. Even if more taxes were raised, the NHS already takes <a href=\"https:\/\/www.kingsfund.org.uk\/insight-and-analysis\/blogs\/comprehensive-spending-review-2025-mean-nhs-health-care\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\">40% of departmental spending<\/a>.<\/p>\n<p class=\"dcr-16w5gq9\">The BMA\u2019s language is out of kilter with the times, as it calls the offer \u201c<a href=\"https:\/\/www.theguardian.com\/society\/2025\/may\/22\/teachers-doctors-england-pay-rise-nhs\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\">derisory<\/a>\u201d and \u201cinsulting\u201d, with its co-chair saying people are \u201c<a href=\"https:\/\/www.independent.co.uk\/news\/uk\/home-news\/wes-streeting-bma-junior-doctors-health-secretary-downing-street-b2776734.html\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\">excited to go again<\/a>\u201d with strikes. Last time, over half the public backed the doctors strikes: a May YouGov poll finds <a href=\"https:\/\/yougov.co.uk\/topics\/politics\/survey-results\/daily\/2025\/05\/27\/afee1\/2\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\">48% opposed<\/a> to further strike action, with 39% of voters in favour.<\/p>\n<p class=\"dcr-16w5gq9\">I hear there is much complaint about the strike among doctors, but expect very few to dare speak out in public. Clare Gerada, former president of the Royal College of GPs, <a href=\"https:\/\/www.theguardian.com\/society\/2025\/jun\/08\/resident-doctors-should-vote-against-strike-action\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\">is one<\/a>. But her polite objections draw poison on social media. Incidentally, she tells me the high cost of the strike last time was money spent on paying overtime to other doctors, with \u201cconsultants \u2026 getting above \u00a33,000 to fill in for a weekend\u201d. Consultants are also balloting over pay.<\/p>\n<p class=\"dcr-16w5gq9\">England\u2019s 77,000 resident doctors have good reasons to protest \u2013 but less over pay than their careers. Their training is gruelling compared with any other students. Due to abysmal stop-go planning by previous health secretaries, 20,000 of them are stuck in a bottleneck, with <a href=\"https:\/\/www.bma.org.uk\/bma-media-centre\/resident-doctors-demand-expansion-of-nhs-training-posts-to-ensure-jobs-for-uk-medics#:~:text=Due%20to%20failures%20of%20the,in%20the%20UK%20are%20protected\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\">five applying for every training post<\/a>, up from 2:1 five years ago. Streeting says he wants to talk to the BMA about conditions: he has just ordered <a href=\"https:\/\/www.theguardian.com\/society\/2025\/jul\/02\/nhs-in-england-told-to-slash-recruitment-of-overseas-trained-medics\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\">UK-trained doctors<\/a> to take precedence for these posts over their foreign-trained peers, and created <a href=\"https:\/\/pulsepcn.co.uk\/news\/almost-2000-gps-recruited-through-arrs\/\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\">almost 2,000 more GP posts<\/a> by changing practices\u2019 hiring finances.<\/p>\n<p class=\"dcr-16w5gq9\">Meanwhile, Streeting\u2019s 10-year plan has been greeted with enthusiasm for its principles \u2013 prevention, neighbourhood and electronic records \u2013 if doubts about delivery. Those of us of who have been following the NHS for decades haven\u2019t enough fingers to count the \u201clong-term plans\u201d and \u201cplans big enough to see from space\u201d that cause acronym storms of administrative upheaval, before another one comes along. But this gets a fair wind for its local clinics starting in poorest places, and <a href=\"https:\/\/www.goodgrowthfoundation.co.uk\/press\/to-win-the-next-election-labour-must-fix-the-nhs-good-growth-foundation\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\">public support in polling<\/a>.<\/p>\n<p class=\"dcr-16w5gq9\">Popular demand has brought back <a href=\"https:\/\/www.nationaltheatre.org.uk\/productions\/nye\/\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\">Nye<\/a>, the National Theatre\u2019s play about the founding of the NHS. By chance I saw it on Wednesday, the day after the strike was announced. Herbert Morrison, a sceptic, warns Nye Bevan (Michael Sheen) that \u201cno health minister has ever persuaded the British Medical Association to agree anything\u201d, greeted by knowing gales of laughter from the audience.<\/p>\n<p class=\"dcr-16w5gq9\">Bevan replies: \u201cExactly. So we have to break the union.\u201d Morrison is astounded at this from a leftist. \u201cThey\u2019re middle class, that\u2019s fine,\u201d says Bevan, to even more laughter from an audience that seemed NHS-heavy, and they weren\u2019t laughing in support of doctors. True, battles between <a href=\"https:\/\/www.theguardian.com\/society\/nhs\" data-link-name=\"in body link\" data-component=\"auto-linked-tag\" target=\"_blank\" rel=\"noopener\">NHS<\/a> staff and politicians rarely end well for health secretaries, but this time the BMA may have overreached.<\/p>\n<p class=\"dcr-16w5gq9\">The play ends with a list of facts loudly applauded: \u201cWithin 10 years of the NHS being launched, infant mortality fell by 50%.\u201d Support for the NHS is as strong as ever, but anxiety about the state of it will only worsen if its improvement is blocked by strikes.<\/p>\n","protected":false},"excerpt":{"rendered":"Good news and bad news for the NHS this week.
