file extensively noted a clear risk he he may abscond or wander due to his condition.<\/p>\n<p>His care plan also said staff had to check on him every single hour to make sure he was safe.<\/p>\n<p>The probe by the HSE made the shocking revelation that senior care assistant and a care assistant who had responsibility for Mr Kearins\u2019 care had falsified records.<\/p>\n<p>Car bursts into flames in busy Glasgow street sparking rush hour chaos<\/p>\n<p>They had said they performed vital tasks involving the OAP when he wasn&#8217;t even in the building.<\/p>\n<p>Both care workers weren&#8217;t aware he wasn&#8217;t in his room or he had made it outside.<\/p>\n<p>They had no idea the tragic pensioner had left the building until his <a href=\"https:\/\/www.thescottishsun.co.uk\/topic\/body\/\" target=\"_blank\" rel=\"noopener\">body<\/a> was found later in the morning.<\/p>\n<p>The probe found the company had failed to have a safe system of <a href=\"https:\/\/www.thescottishsun.co.uk\/topic\/work\/\" target=\"_blank\" rel=\"noopener\">work<\/a> in place.<\/p>\n<p>They also found the fire alarm for the internal fire door had been deactivated.<\/p>\n<p>The probe also revealed a catalogue of failures at the facility.<\/p>\n<p>CATALOGUE OF FAILURES<\/p>\n<p class=\"article__content--intro\">The Health and Safety Executive found:<\/p>\n<p>The management failures in respect of the alarm door reactivation were not causative of Mr Kearins\u2019 death and would likely not have even come to light but for four individual errors:<\/p>\n<ul>\n<li>The unidentified member of staff who closed the internal fire door without further action;<\/li>\n<li>The fire alarm for the internal fire door which had been deactivated<\/li>\n<li>The unidentified member of staff who left the unalarmed external fire door insecure; and<\/li>\n<li>The actions of both the senior care assistant and the care assistant.<\/li>\n<\/ul>\n<p>Oakminster Healthcare Limited pled guilty to Health and Safety offences at Glasgow Sheriff <a href=\"https:\/\/www.thescottishsun.co.uk\/topic\/courts\/\" target=\"_blank\" rel=\"noopener\">Court<\/a>.<\/p>\n<p>The firm was fined \u00a353,750 on July 23.<\/p>\n<p>A <a href=\"https:\/\/www.thescottishsun.co.uk\/topic\/health-and-safety\/\" target=\"_blank\" rel=\"noopener\">Health and Safety<\/a> Inspector blasted the failings and said Mr Keavins&#8217;
