Placental histopathology and fetal autopsy have the highest utility in identifying the cause of a stillbirth, and are recommended by the American College of Obstetricians and Gynecologists (ACOG) and the Society for Maternal-Fetal Medicine (SMFM) [<a data-track=\"click\" data-track-action=\"reference anchor\" data-track-label=\"link\" data-test=\"citation-ref\" aria-label=\"Reference 2\" title=\"Metz TD, Snipe Berry R, Fretts RC, Reddy UM, Turrentine MA. Information on pathology following stillbirth was added to the U.S. Fetal Death Data Files in 2014, collected from fetal death certificates nationwide. It was previously reported using those data files that the prevalence of fetal autopsy was 20.9% among stillbirths between 2014 and 2016 [<a data-track=\"click\" data-track-action=\"reference anchor\" data-track-label=\"link\" data-test=\"citation-ref\" aria-label=\"Reference 3\" title=\"Oliver EA, Finneran MM, Rood KM, Ma&#x2019;ayeh M, Berghella V, Silver RM. Fetal autopsy rates in the United States: analysis of national vital statistics. Obstet Gynecol. 2022;140:869&#x2013;73.\" href=\"http:\/\/www.nature.com\/articles\/s41372-025-02381-3#ref-CR3\" id=\"ref-link-section-d66346069e381\" target=\"_blank\" rel=\"noopener\">3<\/a>], with the prevalence of placental histopathology not reported.<\/p>\n<p>Evidence on the use of fetal autopsy and placental histopathology following stillbirth in recent years and how utilization has potentially changed over time is needed and currently lacking. Despite the prevalence of stillbirth in the United States, the causes of death continue to be understudied, likely due in part to the barriers in the use of postmortem exams [<a data-track=\"click\" data-track-action=\"reference anchor\" data-track-label=\"link\" data-test=\"citation-ref\" aria-label=\"Reference 4\" title=\"Gibbins KJ, Vora NL, Subramaniam A, Page JM, Riches NO, Rothwell E.
