The VA has experienced a net loss of 2,000 registered nurses since the start of this fiscal year, the data show, along with approximately 1,300 medical assistants, 1,100 nursing assistants and licensed practical nurses, 800 doctors, 500 social workers and 150 psychologists.<\/p>\n<p class=\"dcr-130mj7b\">The numbers are at odds with claims by the VA secretary, Doug Collins, that veterans\u2019 healthcare would<a href=\"https:\/\/www.npr.org\/2025\/05\/10\/nx-s1-5391962\/va-job-cuts-limit-doctor-nurse-resignations-veterans\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\"> not<\/a> be impacted by an agency-wide<a href=\"https:\/\/news.va.gov\/press-room\/va-to-reduce-staff-by-nearly-30k-by-end-of-fy2025\/\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\"> reduction<\/a> of 30,000 workers to be completed this year through a combination of attrition, a hiring freeze and deferred resignation program.<\/p>\n<p class=\"dcr-130mj7b\">The reduction in medical staff is also feeding fears that the <a href=\"https:\/\/www.theguardian.com\/us-news\/trump-administration\" data-link-name=\"in body link\" data-component=\"auto-linked-tag\" target=\"_blank\" rel=\"noopener\">Trump administration<\/a> is seeking to transform the VA, which currently operates the largest integrated healthcare system in the United States, into a private voucher program.<\/p>\n<p class=\"dcr-130mj7b\">\u201cIt\u2019s a betrayal,\u201d said Manuel Santamaria, 42, a disabled veteran who served as a US army medic and paratrooper in Iraq and Afghanistan. \u201cIt takes away the government\u2019s accountability to veterans who have sacrificed for them.\u201d<\/p>\n<p class=\"dcr-130mj7b\">The VA said in a statement to the Guardian that the fear of privatization \u201cis a far-left canard\u201d and that \u201canyone who says VA is cutting health care and benefits is not being honest\u201d.<\/p>\n<p>Unite for Veterans, Unite for America Rally at the National Mall on 6 June 2025. Photograph: The Washington Post\/Getty Images<\/p>\n<p class=\"dcr-130mj7b\">The decline in the number of health workers, <a href=\"https:\/\/www.va.gov\/EMPLOYEE\/docs\/workforce\/VA-Workforce-Dashboard-Issue-27.pdf\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\">documented<\/a> by the VA\u2019s monthly workforce dashboard,<strong> <\/strong>comes as the agency has experienced an influx of nearly<a href=\"https:\/\/department.va.gov\/pactdata\/interactive-dashboard\/\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\"> 500,000 patients<\/a> with complex medical issues linked to war-time toxic exposure. <a href=\"https:\/\/www.va.gov\/resources\/the-pact-act-and-your-va-benefits\/\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\">Bipartisan legislation<\/a> passed by Congress in 2022 dramatically expanded veterans\u2019 eligibility for care for toxic exposure.<\/p>\n<p class=\"dcr-130mj7b\">Patricia Fieldings, a registered nurse who works in the spinal cord injury unit at the VA hospital in Augusta, Georgia, said the departures are creating \u201cvery unsafe\u201d conditions for her patients, many of whom lost the use of their neck or became paraplegic due to war wounds.<\/p>\n<p class=\"dcr-130mj7b\">Because of staffing shortages, nurses are being pulled from other parts of the hospital, who lack adequate training to \u201csafely care for a spinal cord patient\u201d, she said.<\/p>\n<p class=\"dcr-130mj7b\">Veterans and employee unions note that even before these departures the VA was already understaffed. Last August, the<a href=\"https:\/\/www.vaoig.gov\/reports\/national-healthcare-review\/oig-determination-veterans-health-administrations-severe-0\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\"> VA\u2019s inspector general reported<\/a> that 86% of the agency\u2019s 170 medical centers and more than 1,000 clinics reported a \u201csevere\u201d shortage of doctors, while 82% of facilities reported severe shortages of nurses.<\/p>\n<p class=\"dcr-130mj7b\">The staff reductions reported by VA this year represent an additional 2% decrease in the number of nurses and 3% decline in the number of doctors, exacerbating the existing shortages.<\/p>\n<p class=\"dcr-130mj7b\">In response to a detailed list of questions, the VA press secretary, Peter Kasperowicz, did not dispute that the VA employed fewer doctors and nurses under President Trump, but said \u201ca nationwide shortage of healthcare workers\u201d had made \u201chiring and retention difficult for the last 15 years across the entire healthcare sector, including at some VA locations\u201d.<\/p>\n<p class=\"dcr-130mj7b\">However, agency data shows that under Biden, the VA was able to attract doctors and nurses. The VA added approximately 10,000 \u201cmission-critical\u201d healthcare professionals, between <a href=\"https:\/\/www.va.gov\/EMPLOYEE\/docs\/workforce\/VA-Workforce-Dashboard-Issue-01.pdf\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\">May 2023<\/a> and <a href=\"https:\/\/www.va.gov\/EMPLOYEE\/docs\/workforce\/VA-Workforce-Dashboard-Issue-21.pdf\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\">January 2025<\/a>, agency dashboards show.<\/p>\n<p class=\"dcr-130mj7b\">Kasperowicz said the VA \u201chas several strategies to navigate shortages while ensuring veterans continue to receive timely, high-quality health care\u201d, which include referrals to private healthcare providers and telehealth. He dismissed the \u201csevere shortages\u201d of doctors and nurses reported by the VA inspector general saying the watchdog\u2019s congressionally-mandated report was \u201ccompletely subjective, not standardized and unreliable\u201d.<\/p>\n<p class=\"dcr-130mj7b\">Collins has made<strong> <\/strong>cutting<strong> <\/strong>employee headcount a priority, arguing the agency is bloated and can improve healthcare quality while shedding 30,000 jobs. Collins<a href=\"https:\/\/news.va.gov\/press-room\/va-to-reduce-staff-by-nearly-30k-by-end-of-fy2025\/\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\"> said<\/a> his goal is \u201creducing bureaucracy\u201d by consolidating \u201ccostly administrative functions\u201d.<\/p>\n<p class=\"dcr-130mj7b\">Documents reviewed by the Guardian, however, show the agency is also losing healthcare providers.<\/p>\n<p class=\"dcr-130mj7b\">So far this year, VA nurses represented by the labor union National Nurses United have filed 116 notices, known as \u201cassignment despite objection\u201d, the union said, to protest situations they believe put patient health and safety at risk.<\/p>\n<p>Manuel Santamaria was deployed to Kunar Province in Afghanistan, with the 173rd airborne brigade in 2007. Photograph: Courtesy Manuel Santamaria<\/p>\n<p class=\"dcr-130mj7b\">One written by a shift supervisor in the intensive care unit in the Bronx in June, reported three nurses had been tasked with treating three patients each, contrary to<a href=\"https:\/\/www.aacn.org\/newsroom\/standards-published-for-critical-care-nurse-staffing\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\"> established safety standards<\/a> that stipulate critical care nurses should not care for more than two patients at a time. Six nurses cared for 15 patients at the ICU in the Bronx<strong> <\/strong>during the nurse\u2019s shift, they wrote: 10 had been placed in isolation, all were listed as \u201chigh risk\u201d of falls.<strong> <\/strong>The nurse also reported that equipment was broken and that nurses had to work through breaks and meal times.<\/p>\n<p class=\"dcr-130mj7b\">At the VA in Augusta, Georgia, Fieldings filed five reports alleging conditions that \u201cposed a serious threat to health and safety of patients\u201d in the spinal cord unit between April and July. On 6 June, she wrote that two registered nurses had been tasked with caring for 10 patients, seven of whom were on ventilators and all of whom were at high risk of falls. On 24 July, she reported three nurses were charged with taking care of nine veteran patients, all of whom had been placed in isolation. \u201cStaff were not properly trained,\u201d she indicated, and the unit was staffed with \u201cunqualified\u201d or \u201cinappropriate personnel\u201d.<\/p>\n<p class=\"dcr-130mj7b\">\u201cThese are some of our most vulnerable patients,\u201d Fieldings said in an interview. \u201cThey have difficulty eating. Many have broken their necks.\u201d<\/p>\n<p class=\"dcr-130mj7b\">She said departures had also led to regular forced overtime for nurses, leaving them exhausted. \u201cNurses stay until their moral distress is so great that they can\u2019t take it anymore and then they leave,\u201d she said.<\/p>\n<p class=\"dcr-130mj7b\">In its statement, the VA said reports of unsafe conditions in Augusta\u2019s spinal cord unit were \u201cfalse\u201dand that \u201cthe unit is fully staffed\u201d.<strong> <\/strong>The VA did not respond to reports alleging unsafe conditions at the agency\u2019s hospital in the Bronx. It said the Guardian was \u201ccherry picking issues that are mostly routine, limited to a handful of sites and in many cases were worse under the Biden administration\u201d.<\/p>\n<p class=\"dcr-130mj7b\">On Wednesday, the agency said it was<a href=\"https:\/\/news.va.gov\/press-room\/va-terminates-union-contracts-for-most-bargaining-unit-employees\/\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\"> terminating labor agreements<\/a> with unions that represent most of its health workers, citing a March<a href=\"https:\/\/www.whitehouse.gov\/presidential-actions\/2025\/03\/exclusions-from-federal-labor-management-relations-programs\/\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\"> executive order<\/a> from President Trump that sought to eliminate collective bargaining rights on \u201cnational security\u201d grounds.<\/p>\n<p class=\"dcr-130mj7b\">The termination of union agreements eliminates the VA\u2019s obligation to respond to these reports of unsafe working conditions.<a href=\"https:\/\/www.nationalnursesunited.org\/sites\/default\/files\/nnu\/documents\/VA-and-NNOCNNU-2023-Master-Agreement%201223.pdf\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\"> Contract language<\/a> requiring nurses be provided professional training has also been voided, along with rules that require management to \u201cfloat\u201d, or temporarily reassign nurses to cover parts of the hospital experiencing staffing shortages, \u201cbased on qualifications and competencies of the RN\u201d.<\/p>\n<p class=\"dcr-130mj7b\">In statements, the AFL-CIO, National Nurses United and the American Federation of Government Employees, which represents 320,000 VA workers, said the VA is<a href=\"https:\/\/www.afge.org\/publication\/largest-veteran-affairs-employee-union-outraged-after-secretary-collins-terminates-afge-collective-bargaining-agreement\/\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\"> retaliating<\/a> against workers for speaking up for patients. The agency<a href=\"https:\/\/news.va.gov\/press-room\/va-terminates-union-contracts-for-most-bargaining-unit-employees\/\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\"> says<\/a> the end of collective bargaining means \u201cstaff will spend more time with Veterans\u201d and allow the agency to \u201cmanage its staff according to Veterans\u2019 needs, not union demands\u201d.<\/p>\n<p class=\"dcr-130mj7b\">The unions, who say their right to bargain collectively cannot be voided on \u201cnational security\u201d grounds, said they would press their case in the courts.<\/p>\n<p>The seal of the Department of Veterans Affairs. Photograph: NurPhoto\/Getty Images<\/p>\n<p class=\"dcr-130mj7b\">At the VA Northport Medical Center on Long Island, a virtual town hall on 30 July with hospital director Antonio Sanchez devolved; the chat function in Microsoft Teams burst with complaints. \u201cWe are desperate. We are getting burnt out,\u201d a clinician in the facility\u2019s inpatient medical unit wrote. \u201cWe are not able to keep pace with our cut in staff.\u201d<\/p>\n<p class=\"dcr-130mj7b\">Others raised concerns about short staffing and patient care across other critical care units at Northport, the Teams chat shows, including the intensive care unit, post-anesthesia care unit and community living centers, the VA\u2019s version of nursing home care.<\/p>\n<p class=\"dcr-130mj7b\">Near the end of the meeting, Sanchez responded that he was \u201cworking hard\u201d to bring staff on board as soon as possible.<\/p>\n<p class=\"dcr-130mj7b\">Veterans advocates and employee unions allege the Trump administration is consciously seeking to starve the veterans healthcare system so it can be turned into a private voucher program. The administration\u2019s budget proposal for 2026 includes a major re-alignment of priorities \u2013 a 50% increase in taxpayer funding for private healthcare for veterans, $11bn, paid for by a corresponding cut to the existing public system.<\/p>\n<p class=\"dcr-130mj7b\">During his Senate<a href=\"https:\/\/www.veterans.senate.gov\/2025\/7\/hearing-to-consider-pending-nominations\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\"> confirmation hearing<\/a> to be the VA\u2019s undersecretary for health on 23 July, John Bartrum, a retired air force major general, said it was time to \u201crevisit the balance\u201d between private and government veterans healthcare. In a statement to the Guardian, the VA rejected the idea that it was seeking to privatize the agency and said it had opened 13 healthcare clinics since January. Advocates said those facilities had been in the works for years.<\/p>\n<p class=\"dcr-130mj7b\">The same day as Bartrum\u2019s hearing, the Access Act, a bill that would make it dramatically easier for veterans to seek private healthcare with taxpayer support,<a href=\"https:\/\/www.military.com\/daily-news\/2025\/07\/23\/proposal-expand-private-health-care-veterans-advanced-house-panel.html?amp\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\"> cleared a key House committee<\/a> on a 12-11 party-line vote.<\/p>\n<p class=\"dcr-130mj7b\">A blue-ribbon commission established by the agency<a href=\"https:\/\/www.grassley.senate.gov\/imo\/media\/doc\/va_red_team_report.pdf\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\"> last year found<\/a> veterans received significantly better care at lower cost from the public system. Private providers operated with little oversight, they wrote, and \u201care not required to demonstrate competency in diagnosing and treating the complex care needs of veterans nor in understanding military culture, which is often critical to providing quality care for veterans\u201d.<\/p>\n<p class=\"dcr-130mj7b\">Independent studies from the<a href=\"https:\/\/www.cbo.gov\/publication\/57583#footnote-019-backlink\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\"> Congressional Budget Office<\/a> and<a href=\"https:\/\/www.rand.org\/pubs\/periodicals\/health-quarterly\/issues\/v10\/n3\/09.html\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\"> Rand Corporation<\/a> have similarly found that veterans in the private system face longer wait times and poorly coordinated healthcare at higher costs to the taxpayers.<\/p>\n<p>Santamaria\u2019s hand without medication to treat his psoriasis, a skin disease that causes a rash with itchy scaly patches. Photograph: Courtesy Manuel Santamaria<\/p>\n<p class=\"dcr-130mj7b\">In Las Vegas,<strong> <\/strong>Santamaria bristled at the suggestion the VA was bloated. He said difficulties scheduling appointments have \u201cgotten much worse\u201d under Trump \u2013 and that he now often spends more than two hours on hold.<\/p>\n<p class=\"dcr-130mj7b\">In addition to post traumatic stress disorder, Santamaria suffers from psoriasis, a skin disease that causes a rash with itchy scaly patches that disrupt his sleep and make it hard to concentrate. In February, when his medication ran out, Santamaria sought to switch to a different drug,<strong> <\/strong>but was unable to schedule an appointment with the VA until May. By then his entire body was covered by cracked, bleeding welts.<\/p>\n<p class=\"dcr-130mj7b\">Psoriasis is among the conditions presumed to be connected to war-time toxic exposure under the Pact Act, the 2022 law that extended healthcare and benefits eligibility. According to the VA, the wait time for new dermatology patients at the VA hospital in southern Nevada was<a href=\"https:\/\/www.accesstocare.va.gov\/FacilityPerformanceData\/FacilityDataResults?LocationText=Las%20Vegas,%20Nevada,%20United%20States&amp;Radius=50&amp;UserLatitude=-1&amp;UserLongitude=-1&amp;f=593&amp;sd=1,2,3,99&amp;sc=1,2,3,4,5,6,7,8,9,10\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\"> 137 days<\/a> as of August 7.<\/p>\n<p class=\"dcr-130mj7b\">In its statement, the VA blamed the delays Santamaria faced on a lack of communication from the private dermatologist he saw through the agency\u2019s \u201ccommunity care\u201d voucher program. Once that doctor filled out the proper paperwork, \u201cnew medication was mailed out just five days later\u201d, the agency said.<\/p>\n<p class=\"dcr-130mj7b\">The agency said the average wait time on phone calls placed to the VA in southern Nevada was one minute and 47 seconds. Santamaria said VA workers typically answer the phone relatively quickly, before putting him on hold for two hours or more.<\/p>\n<p class=\"dcr-130mj7b\">The Guardian asked the VA for data in order to compare patient wait times during the Trump and Biden administrations at each VA facility. The VA did not provide it, but shared a chart that indicated wait times for some health services had marginally improved this fiscal year, while others had worsened. The fiscal year covers the final three and-a-half months of the Biden administration and first six months of Trump\u2019s second term.<\/p>\n<p class=\"dcr-130mj7b\">The VA\u2019s workforce dashboards show the decline in staffing was accelerating. Photograph: The Washington Post\/Getty Images<\/p>\n<p class=\"dcr-130mj7b\">The VA declined to answer questions on the acceleration in the reduction of \u201cmission-critical\u201d medical workers employed in agency hospitals and clinics.<\/p>\n<p class=\"dcr-130mj7b\">Those staffing losses do not include 10,000 VA workers who accepted deferred resignation offers early in Trump\u2019s second term, whose last day on the books will be 30 September.<\/p>\n<p class=\"dcr-130mj7b\">In response to a Freedom of Information Act request filed by The War Horse, a non-profit news outlet focused on veterans issues, the VA<a href=\"https:\/\/thewarhorse.org\/wp-content\/uploads\/2025\/07\/VA-Deferred-Resignation-January-Through-May-2025.pdf\" data-link-name=\"in body link\" target=\"_blank\" rel=\"noopener\"> provided<\/a> a list of occupations held by these workers.
