(ABC News: Patrick Stone)<\/p>\n<p class=\"paragraph_paragraph__iYReA\">He said the longer appointments many of his patients needed did not attract Medicare rebates proportionate to the time he spent with them, and he could earn significantly more if he squeezed in lots of shorter appointments back-to-back.<\/p>\n<p class=\"paragraph_paragraph__iYReA\">&#8220;[But] there is simply no way to sustain empathy, compassion and concentration seeing that many patients in a day,&#8221; he said.<\/p>\n<p class=\"paragraph_paragraph__iYReA\">&#8220;Burnout or more superficial care is almost guaranteed.&#8221;<\/p>\n<p>GPs taking on role of specialists<\/p>\n<p class=\"paragraph_paragraph__iYReA\">The Royal Australian College of GPs (RACGP) released its annual survey of members and GPs in training on Tuesday, and like Dr Harris, many of the thousands of doctors surveyed said they were spending longer with patients, particularly for mental health.<\/p>\n<p class=\"paragraph_paragraph__iYReA\">The college has urged the federal government to increase Medicare rebates for longer consultations by 40 per cent.<\/p>\n<p class=\"paragraph_paragraph__iYReA\">As it stands, the rebate a patient receives per minute generally decreases the longer the appointment is.<\/p>\n<blockquote class=\"EmphasisedText_quote__TE6kn\"><p>&#8220;We are living in an ageing society with more chronic health conditions and more complex healthcare needs, which means patients require more time with GPs,&#8221;<\/p><\/blockquote>\n<p>  president Michael Wright said.<img decoding=\"async\" alt=\"A middle-aged white man with short greying hair and a crisp white shirt sits at a computer in a brightly lit workspace.\" class=\"Image_image__5tFYM ContentImage_image__DQ_cq\"  src=\"https:\/\/www.europesays.com\/uk\/wp-content\/uploads\/2025\/10\/11eedc243d12343f29a77e6461389a10\" loading=\"lazy\" data-component=\"Image\" data-lazy=\"true\"\/><\/p>\n<p class=\"Typography_base__sj2RP FigureCaption_text__zDxQ5 Typography_sizeMobile12__w_FPC Typography_lineHeightMobile20___U7Vr Typography_regular__WeIG6 Typography_colourInherit__dfnUx\" data-component=\"Typography\">Royal Australian College of General Practitioners president Michael Wright says patients require more time with GPs. (ABC News: John Gunn)<\/p>\n<p>GP survey at a glance<\/p>\n<ul class=\"ContentAlignment_marginBottom__4H_6E ContentAlignment_overflowVisible__N2zKU\">\n<li>The average fee for a six\u201320-minute standard consultation has increased from just over $78 to $82 (which means the average gap fee is around $39)<\/li>\n<li>28 per cent of people say they are waiting longer for a GP appointment than they felt acceptable<\/li>\n<li>8.8 per cent of Australians delayed care due to cost in 2024, compared to 7 per cent in 2023<\/li>\n<\/ul>\n<p class=\"paragraph_paragraph__iYReA\">As the cost of seeing a GP continues to increase, half of practising GPs said they now charged $90 or more for a six-to-20-minute consultation, which left patients $47 out of pocket after the Medicare rebate.<\/p>\n<p class=\"paragraph_paragraph__iYReA\">Only 12 per cent said they were able to bulk-bill all patients compared to 13 per cent last year.<\/p>\n<p class=\"paragraph_paragraph__iYReA\">Most GPs said they were also regularly managing conditions typically treated by specialists, such as psychologists or psychiatrists.<\/p>\n<p class=\"paragraph_paragraph__iYReA\">Even in metropolitan Melbourne where he practised, Dr Harris said it could be near impossible to find a private psychiatrist to refer his patients to, so he had to do his best to provide that level of care.<\/p>\n<p class=\"paragraph_paragraph__iYReA\">Dr Harris said knowing you could not get someone into a specialist or a public hospital mental health unit could be a lot of pressure.<\/p>\n<p class=\"paragraph_paragraph__iYReA\">&#8220;Particularly for junior doctors, that can be pretty scary,&#8221; he said.<\/p>\n<p><img decoding=\"async\" alt=\"\" class=\"Image_image__5tFYM ContentImage_image__DQ_cq\"  src=\"https:\/\/www.europesays.com\/uk\/wp-content\/uploads\/2025\/10\/45e195cbe8317d73f4f5795b63761c82\" loading=\"lazy\" data-component=\"Image\" data-lazy=\"true\"\/><\/p>\n<p class=\"Typography_base__sj2RP FigureCaption_text__zDxQ5 Typography_sizeMobile12__w_FPC Typography_lineHeightMobile20___U7Vr Typography_regular__WeIG6 Typography_colourInherit__dfnUx\" data-component=\"Typography\">Doctors&#8217; biggest challenge is the rising number of complex presentations, particularly with mental health, according to the survey. (ABC News: Kate Nickels)<\/p>\n<p class=\"paragraph_paragraph__iYReA\">The report said funding for GP care had remained stagnant for the past decade and in 2023-24 the government spent $452 on GP care per person compared to $3,649 on public hospital care per person.<\/p>\n<p>Will the bulk-billing changes help?<\/p>\n<p class=\"paragraph_paragraph__iYReA\">The RACGP report comes just weeks before new bulk-billing changes promised at the last election come into effect.<\/p>\n<p class=\"paragraph_paragraph__iYReA\">From November 1 the government will allow GPs to receive a bulk-billing incentive for any patient. Currently they are only eligible for the incentive if they bulk-bill a patient who is under 16 or a concession card holder.<\/p>\n<p class=\"paragraph_paragraph__iYReA\">There will also be an additional 12.5 per cent incentive payment, split between the GP and practice, when all GPs at the same clinic bulk-bill all patients.<\/p>\n<p class=\"paragraph_paragraph__iYReA\">Health Minister Mark Butler said these incentives meant nine out of 10 GP visits would be bulk-billed by 2030.<\/p>\n<p class=\"paragraph_paragraph__iYReA\">&#8220;GP practices around the country are already preparing to sign up to be fully bulk-billing practices ahead of November,&#8221; he told the ABC.<\/p>\n<p class=\"paragraph_paragraph__iYReA\">&#8220;We know this investment will work because it has already worked for the patients the incentive already applies to: pensioners, concession cardholders, and families with kids. When they went to the GP last year, more than nine out of 10 GP visits were bulk-billed for them.&#8221;<\/p>\n<p><a href=\"https:\/\/www.abc.net.au\/news\/2025-04-24\/election-2025-bulk-billing-incentives-surveys\/105188924\" data-component=\"FullBleedLink\" class=\"RelatedCard_link__rsgR9 FullBleedLink_root__lTw_U interactive_focusContext__yRhc_ interactive_defaults__AKxUU FullBleedLink_showVisited__g3Xvz\" target=\"_blank\" rel=\"noopener\">We\u2019ve been promised more bulk bulling but doctors say they can\u2019t deliver<\/a><\/p>\n<p class=\"Typography_base__sj2RP RelatedCard_synopsis__cFwMW Typography_sizeMobile14__u7TGe Typography_lineHeightMobile20___U7Vr Typography_regular__WeIG6 Typography_colourInherit__dfnUx\" data-component=\"Typography\">Back in April, we reported that two new surveys cast doubt on whether the signature health policy of the 2025 election campaign could be delivered.\u00a0<\/p>\n<p class=\"paragraph_paragraph__iYReA\">Mental health consultation and treatment plan items will also be removed on November 1. Instead, GPs will have to use standard time-based item numbers for mental health concerns.<\/p>\n<p class=\"paragraph_paragraph__iYReA\">Dr Harris said this meant many of his patients would need to pay more because he would not be able to do something called co-billing.<\/p>\n<p class=\"paragraph_paragraph__iYReA\">As an example, if a patient came in for two needs \u2014 depression and diabetes \u2014 Dr Harris could currently charge them privately for diabetes management and bulk-bill them for depression management by using a specific mental health item number.<\/p>\n<p class=\"paragraph_paragraph__iYReA\">But when mental health is charged by time, this co-billing is not possible and the patient will end up paying more for a long appointment for both issues to be addressed.<\/p>\n<p class=\"paragraph_paragraph__iYReA\">&#8220;Just relying on those standard time-based items won&#8217;t allow the clinic to be viable, so [we] will be charging significantly more for those longer appointments,&#8221; Dr Harris said.<\/p>\n<p class=\"paragraph_paragraph__iYReA\"><strong>&#8220;<\/strong>I know many of my patients won&#8217;t be able to afford it and will go elsewhere or we&#8217;ll have to try and cram everything into a short appointment.&#8221;<\/p>\n<p class=\"paragraph_paragraph__iYReA\">The government argues it could work out better for the patient as the rebates for the mental health items were $83.65, whereas the rebate for an appointment that is at least 20 minutes is $84.90, which then increases to $125.10 for a 40-minute appointment and $202.65 for 60 minutes.<\/p>\n<p class=\"paragraph_paragraph__iYReA\">Despite the challenges highlighted by the RACGP survey, 71 per cent of GPs said they were satisfied with their job.<\/p>\n<p class=\"paragraph_paragraph__iYReA\">However, noticeably more doctors said they were dissatisfied with their hours of work, pay and administrative load.<\/p>\n<p class=\"paragraph_paragraph__iYReA\">For the first time, doctors were asked about their experiences with racism, and the survey indicated more than one in 10 had witnessed racism towards a patient from staff or fellow patients in their own practice.<\/p>\n<p class=\"paragraph_paragraph__iYReA\">Nearly one in five GPs have personally experienced racism from patients in the past 12 months and one in 10 have personally experienced racism from a colleague.<\/p>\n<p>Loading&#8230;<\/p>\n","protected":false},"excerpt":{"rendered":"Melbourne GP Owen Harris would love to bulk-bill more of his patients.
