The police announced that it is investigating itself Wednesday morning.
The City of Phoenix released the disciplinary records for two fired officers arrested for an unreported violent traffic stop that made national headlines. The department also announced an internal audit of their histories and a broader investigation into their former squads and precincts.
“The allegations against the officers are serious, and we recognize this investigation is an important issue to the community,” a post on the city’s website announcing the records release and investigation reads.
Former Officers Antonio Felix and Luis Vasquez Jr. were arrested on Aug. 22 for multiple felony charges including kidnapping and aggravated assault. Both officers were assigned to the South Mountain Precinct. Felix was with the department for eight years and Vasquez for nine.

The department will conduct an audit of the officers’ service with the department by going through their reports, body camera footage and more to identify any other previous misconduct that needs investigating, according to the city. It will also investigate the officers’ squad and precinct for any systemic misconduct.
“Both the community and the Police Department deserve confidence that this conduct is not widespread,” the city wrote.
The two officers or their attorneys could not be reached for comment in time for publication of this story.
The city provided a summary of the officers’ disciplinary backgrounds. Both were disciplined multiple times. Vasquez was also found to be within policy for an officer involved shooting in 2025 and was given 14 commendations between 2019 and 2026. Felix was given five commendations between 2020 and 2022.

According to the police department Vasquez’s disciplinary history includes:
- Failure to issue a citation in a collision investigation in 2021; resulted in supervisory coaching.
- Four missed court appearances between 2021-2025; resulted in supervisory counseling.
- Failed to thoroughly investigate a collision and failed to properly activate his body-worn camera in 2025; resulted in a Written Reprimand.
According to the police department Felix’s disciplinary history includes:
- Failure to notify his Field Training Officer of a use of force during an incident in 2018 until they returned to the precinct to write the report; resulted in supervisory coaching.
- Three missed court appearances between 2018-2023; resulted in supervisory counseling.
- Three minor police vehicle collisions between 2018-2022, all determined to be out of policy; resulted in supervisory counseling.
- Received a community complaint about rudeness during a call for service in 2021; resulted in no policy violations found, however he did receive coaching on his communication.
- Failure to complete two annual AZ Post requirements in 2024 due to industrial leave; qualifications were completed upon return in 2025.
- Failed to complete a report on a call for service in 2025 regarding theft and threatening messages; resulted in a Written Reprimand.
- Failed to complete an incident report for a call for service in 2026 involving a missing juvenile by the end of shift as required; would have resulted in discipline, but the case was pending at the time of termination.
