One Fort Worth restaurant received more than 30 demerits during June 15-21 inspections, and roaches were found at others, according to data from the city compiled by the Star-Telegram.

Tokyo Cafe at 5121 Pershing Ave. scored 35 on June 15. Restaurants that receive more than 30 demerits are required to immediately fix the most critical violations and start corrective procedures on all the other violations within 48 hours.

The inspector found evidence of insects or other pests; employees hadn’t completed an accredited food-handler course, and chicken and other foods weren’t being held at a safe temperature.

Donut Express at 2606 Cherry Lane scored 26 on June 16. The inspector found unlabeled icing containers, kolaches that weren’t marked with the preparation date and an accumulation of food residue inside a cabinet and reach-in cooler.

The establishment scored 7 during a follow-up inspection the next day.

Bee-Zee Food Mart at 2749 Las Vegas Trail scored 25 on June 16. Chicken and pasta were left cooling at room temperature, no food manager certification was available and the microwave was dirty.

The food mart scored 12 on the June 17 follow-up inspection.

No evidence of rodents was found in the 103 establishments inspected, but roaches were found at two locations.

Birrieria Jalisco at 1201 E. Long Ave. scored 16 on June 16. The inspector noticed a live cockroach near one of the hand sinks. Some food was stored uncovered and one of the refrigerators wasn’t cooling properly.

Carniceria Los Arcos at 6809 McCart Ave. scored 9 on June 19. The inspector found several dead roaches, meat stored on the floor and an expired food-handler certificate.
