PHOENIX (AZFamily) — Every week, Arizona’s Family looks up restaurants recently inspected by Maricopa County’s Environmental Services Department. Restaurants with some of the highest numbers of “risk factors,” considered major health code violations by inspectors, are selected for our Dirty Dining Report. Here are the Valley restaurants that did not make the grade for Oct. 8, 2025:
The Villa at Bella Vista – 1825 E Emelita Avenue, Mesa
3 violations:
- Employee drink without lid on top of dishwasher
- Employee not washing hands properly
- Mixed melon not kept cold enough
Eggstasy – 2510 W Happy Valley Rd, Phoenix
4 violations:
- Liquid raw eggs not at proper temperature
- Tzatziki sauce with yogurt not kept cold enough
- Cooked sausage kept past discard date
- Medicine stored above prep table
Fu Belly – 5055 W Ray Rd, Chandler
4 violations:
- Employee drink stored on counter with food
- Employee handled raw beef then didn’t wash hands
- Bowl of meats thawing in hand wash sink
- Raw chicken thawing with raw shrimp
Bahara India Kitchen – 9920 S Rural Rd, Tempe
4 violations:
- Pan of raw chicken stored above spinach
- Cooked rice not kept hot enough
- Toxic chemicals not labeled
- Raw meat stored in to-go boxes
Hot Bagels Deli – 34640 N North Valley Parkway, Phoenix
4 violations:
- No certified food manager on duty
- Cook cracked an egg then touched a bagel
- Raw eggs stored on top of ready-to-eat bacon
- Medicine bottle stored above deli meat
