The Office of the Inspector General report says this is important because discipline is meant to be progressive.

That means discipline for new misconduct should be based on previous disciplinary actions.
The report goes on to say gaps in coordination leads to an unclear picture of CPD misconduct, impedes on applying the proper discipline, and hinders CPD from identifying at-risk members for intervention.
The report covers data from 2016 to 2021. CPD says since then, it has fully implemented a new case management system.
