PHOENIX (AZFamily) — Every week, Arizona’s Family looks up restaurants recently inspected by Maricopa County’s Environmental Services Department. Restaurants with some of the highest numbers of “risk factors,” considered major health code violations by inspectors, are selected for our Dirty Dining Report. Here are the Valley restaurants that did not make the grade for Dec. 17, 2025:
Eggstasy – 20736 E Maya Rd, Queen Creek
3 violations:
- Raw beef stored over cooked beef
- Cooked chicken breasts not kept hot enough
- Bloody Mary mix kept past discard date
Hash Kitchen – 3919 E Williams Field Rd, Gilbert
3 violations:
- No soap at hand wash sink
- Eggs stored above cooked bacon
- Multiple fruit flies in restaurant
Country Manor Assisted Living – 2815 N 48th Street, Phoenix
3 violations:
- No certified food manager
- Staff member failed to wash hands before working in kitchen
- Eggs stored above ready-to-eat muffins
South Mountain Boarding Home – 6420 S 22nd Street, Phoenix
3 violations:
- Lettuce, grapes and bread with organic gray/white material
- Raw eggs above ready to eat salad and pasta
- Food debris and trash behind kitchen equipment
Evo Restaurant – 15507 N Scottsdale Road, Scottsdale
5 violations:
- No certified food manager
- Bartender grabbed ice cube with bare hands
- Dishwasher not cleaning properly
- Cooked short rib and lasagna not kept cold enough
- Multiple glass bottles stored in ice well
