PHOENIX (AZFamily) — Every week, Arizona’s Family looks up restaurants recently inspected by Maricopa County’s Environmental Services Department. Restaurants with some of the highest numbers of “risk factors,” considered major health code violations by inspectors, are selected for our Dirty Dining Report. Here are the Valley restaurants that did not make the grade for Jan. 14, 2026:
Coop’s Bar and Grill – 8455 W Peoria Ave, Peoria
3 violations:
- Build-up debris in ice maker
- Deli meats not date marked
- Toxic substances not labeled
Carniceria Guasave – 3632 W McDowell Rd, Phoenix
4 violations:
- No bodily fluid clean-up kit
- Hand wash sink not working
- Raw chicken stored next to ready-to-eat foods
- Food debris and organic matter in a pan
Carniceria El Toro Charolais – 6402 W Glendale Ave, Glendale
4 violations:
- Cooked chicken wings not kept hot enough
- Containers of lard and cooked meats not stored cold enough
- No date marks on deli meats
- Restaurant using prep sink as a mop sink
Chilaquiles Modern Kitchen – 1804 S Signal Butte Rd, Mesa
7 violations:
- No certified food manager
- Raw eggs stored above ready-to-eat foods
- Dishwasher not working properly
- Cooked meats not cooling properly
- Hollandaise sauce not held hot enough
- Salsa kept past discard date
- Food not kept at proper temperature
