PHOENIX (AZFamily) — Every week, Arizona’s Family looks up restaurants recently inspected by Maricopa County’s Environmental Services Department. Restaurants with some of the highest numbers of “risk factors,” considered major health code violations by inspectors, are selected for our Dirty Dining Report. Here are the Valley restaurants that did not make the grade for June 3, 2026:
Black Bear Diner – 2805 S Alma School Rd, Chandler
3 violations:
- Cook washing hands without soap
- No soap at hand wash sink
- Raw sausage stored beneath egg whites and ham
Filiberto’s Mexican Food – 4532 E Thomas Rd, Phoenix
3 violations:
- No certified food manager
- Employee handling bag of raw beef, then not washing hands
- Containers of raw shrimp stored above lettuce and onions
Chilaquiles Modern Kitchen – 1804 S Signal Butte Rd, Mesa
4 violations:
- Food left out at room temperature
- Worker handling dirty dishes then clean dishes
- No hot water at hand wash sink
- Meat slicer with food debris
Farm and Craft – 4302 N Scottsdale Rd, Scottsdale
4 violations:
- Employee rinsing dishes, taking trash out, then not washing hands
- Cooked chicken not kept hot enough
- Garlic/oil mixture not kept cold enough
- Dishwashing machine not cleaning properly
Thiru Kuppusamy – 955 W Chandler Heights Rd, Chandler
5 violations:
- Employee using cell phone then working on prep line
- Raw chicken stored above package of celery
- Dishwasher without enough sanitizer
- Container of yogurt not kept cold enough
- Chicken curry and mutton not date marked
